METHODE : La construction du TCS repose sur les recommandations fournies par le site de la faculté des sciences de Montréal. Le panel d'experts est constitué de maîtres de stage universitaire ayant au moins 10 ans d'expérience. RESULTATS : Nous présentons les différents formats des vinettes retenues, les étapes successives du travail de rédaction (définition des situations cliniques, rédaction des questions, correction et validation, et construction des grilles de réponses). Nous évoquons les difficultés rencontrées lors de la conception et les modalités d'application du test. CONCLUSION : Le respect d'un processus de rédaction bien défini permet la production d'un est de qualité constituant un bon outil d'auto-évaluation

Buts: Apprécier la fidélité inter-juge des TCS utilisés en évaluation sommative en DES de médecine générale à Rouen et discuter de l’impact de certains critères d’élaboration du test sur le classement des étudiants. Méthodes : Une analyse quantitative rétrospective des résultats aux TCS en DES de médecine générale à Rouen entre 2011 et 2016 a été effectuée. La fidélité inter-juge a été évaluée via le calcul de la corrélation intra-classe (ICC). L’impact sur le classement des étudiants a été évalué après amélioration du coefficient alpha, exclusion des experts déviants, éviction des réponses extrêmes, et utilisation des stratégies de réponses au hasard. Résultats : La fidélité inter-juge était excellente (ICC=0.984). L’amélioration de la cohérence interne du test modifiait le classement des étudiants mais pas l’exclusion des experts déviants. Les stratégies d’éviction des réponses extrêmes et de réponse au hasard se révélaient parfois efficaces. Conclusion : Le panel d’experts utilisé à Rouen en DES de médecine générale semble quantitativement et qualitativement adapté. Certaines améliorations concernant la construction des items pourraient être réalisées. Une méfiance devrait être accordée concernant l’interprétation du coefficient alpha. D’autres études sont nécessaires afin d’estimer quelles dimensions du raisonnement clinique sont mesurées par le TCS.

L'objectif a été d'évaluer à travers les scores obtenus par les trois groupes de médecins testés, le degré d'expertise en médecine d'urgence, et de faire percevoir la pertinence du test pouvant décliner sur une formation clinique appropriée.

Le test de concordance de script (TCS), méthode innovante d’évaluation du raisonnement clinique (RC) en santé, semble sous-utilisé pour les étudiants dijonnais et notamment en stage en médecine d’urgence (MU). Objectifs. Ce travail tente de montrer la faisabilité et la pertinence de cet outil en termes de progression du RC de l’étudiant entre le début et la fin de son stage (environ 24 journées) aux urgences du CHU de Dijon. Matériel et méthodes. Un questionnaire de 20 TCS concernant la MU a été soumis à 64 étudiants au début et fin de stage pour 28, 14 et 22 étudiants (MM1, MM2, MM3, respectivement) et comparé aux réponses d’un panel d’experts seniors urgentistes. La progression de chaque étudiant est analysée par les tests de rangs signés de Wilcoxon. Résultats. Les scores moyens (sur 60 points) passent de 37,1 plus/moins 3,80 ; 38,5plus/moins 5,06 et 42,74 plus/moins 3,67 au pré-test à 39,3 plus/moins 3,53 ; 41,2 plus/moins 2,68 et 42,7 plus/moins 2,95 au post-test, respectivement pour les MM1, MM2 et MM3, traduisant une amélioration significative des scores entre le début et la fin de stage (p=0,016). En outre, l’ANOVA met en évidence une différence significative des scores entre les promotions en pré-test (p=0,0001) et post-test (p=0.0029), avec un avantage pour les promotions plus âgées. Cependant, la marge de progression n’est pas influencée par la promotion ou le sexe. Conclusions. Ces résultats confirment la faisabilité et l’intérêt d’une évaluation par TCS pour apprécier le RC des étudiants en médecine sur des problématiques d’urgence. La recherche de facteurs influençant la progression du RC et la généralisation des TCS en tant qu’outil validant un stage pratique fait l’objet d’un travail ultérieur.
