The experimental group, which was composed of 4 women (age range=63-72 years, x=65.9 years), completed the 7 week aquatic exercise program. Six women (age range=34-77 years, x=52.9 years) who were members of the Arthritis Foundation's support group and were not participating in regular aquatic exercise, comprised the control group. All women were volunteers and were diagnosed with some form of arthritis. Pretests measured the following variables: hand strength using a baseline hand dynamometer; functional ability with a 150 foot timed walking test and a 13 stair climb which was timed and scored with a Functional Independence Measure (FIM) score; and pain measured with a horizontal visual analogue scale. For 7 weeks the experimental group participated in the Arthritis Foundation YMCA Aquatic Program and the control group maintained normal activities of daily living. Posttests for the same variables were conducted at the end of 7 weeks. A repeated measured ANOVA at the .05 a level was used to find differences between the control group and the experimental group for each of the variables. There was a significantly higher pain rating for the experimental group throughout the study. No other significant differences were found. Although there were no significant physical improvements, the subjective reports from the experimental groups indicated a positive physical and emotional benefit.

Book excerpt: The purpose of this study was to objectively investigate the physical and psychological outcomes following aquatic exercise in individuals with TKA. Introduction: Previous studies have served as a good starting point to assess the potential advantages of water-based rehabilitation. However, most current studies have failed to analyze an all-inclusive assessment of knee range of motion, strength, functional balance, and psychological measures following aquatic exercise in TKA. Therefore, this case study is unique in that it will look at comprehensive rehabilitation outcomes, both physical and psychological measures, and analyze the trend of recovery in each outcome. Methods: 3 participants status post TKA were recruited. One participant participated in conventional treatment with additional aquatic exercise (C+A), one participant participated in conventional treatment with additional land exercise (C+L), and one participant participated in conventional treatment only (C). Each aquatic intervention lasted approximately 1 hour in the aquatic therapy pool. Pre-and post intervention as well as bi-weekly data collection included knee range of motion using a goniometer, strength assessments using a handheld dynamometer, functional balance measurements using the Balance Master, psychological measures using 2 questionnaires: Assessment of Quality of Life, Knee Osteoarthritis Outcome Score, and subjective pain level assessed using a Numeric Pain Rating Scale. Microsoft Excel was used for data analysis and Time-Series Graphs were illustrated to make visual comparisons of changes over time between each participant. Results: C+A achieved earlier improvements in knee extension ROM and balance outcomes compared to the other two participants. C+A and C+L achieved greater improvements in most outcome measures compared to C participant. However, positive trends were found in all participants on most outcome variables. Discussion and Conclusion: Results indicate that exercising in water is safe, effective, and is an appropriate rehabilitative method for individuals post TKA. Although this was a case study with no statistical significance, clinical meaningfulness was shown through positive trends of recovery in knee ROM, balance, strength, and quality of life. While the improvements in this study cannot be directly attributed from this novice program, the positive results suggest that this rehabilitation approach should be further explored in future trials.

Book excerpt: Due to recent advances in aquatic research, technology, and facilities, many modes of aquatic therapy now exist. These aquatic modes assist individuals (e.g., osteoarthritis patients) in the performance of activities that may be too difficult to complete on land. However, the biomechanical requirements of each aquatic therapy mode may elicit different physiological and functional responses. Therefore, the purpose of this thesis was to: (a) provide a review of the physiological and biomechanical differences between aquatic and land based exercises, and (b) examine the acute effects of underwater and land treadmill exercise on oxygen consumption (VO2), rating of perceived exertion (RPE), perceived pain, mobility, and gait kinematics for patients with osteoarthritis (OA). Methods consisted of the retrieval of experimental studies examining the physiological and biomechanical effects of deep water running (DWR), shallow water running (SWR), water calisthenics, and underwater treadmill therapy. The methods also examined the physiological and biomechanical effects on 19 participants during and after three consecutive exercise sessions on an underwater treadmill and on a land-based treadmill. Based on the studies reviewed, when compared to a similar land-based mode, VO2 values are lower during both DWR and SWR, but can be higher during water calisthenics and underwater treadmill exercise. RPE responses during DWR are similar during max effort, and stride frequency and stride length are both lower in all four aquatic modes than on land. Pain levels are no different between most water calisthenics, and most studies reported improvements in mobility after aquatic therapy, but no difference between the aquatic and land-based modes. The OA participants achieved VO2 values that were not different between conditions during moderate intensities, but were 37% greater during low intensity exercise on land than in water (p = 0.001). Perceived pain and Time Up & Go scores were 140% and 240% greater, respectively, for land than underwater treadmill exercise (p = 0.01). Patients diagnosed with OA may walk on an underwater treadmill at a moderate intensity with less pain and equivalent energy expenditures compared to walking on a land-based treadmill.
