For these low birth weight, premature infants, the future is uncertain, since they are at risk for a variety of serious medical and developmental problems—including behavioral and learning disorders that may have damaging effects for the rest of their lives. The extent to which a comprehensive early intervention program could improve or prevent these adverse outcomes was examined in the Infant Health and Development Program, a randomized controlled trial involving almost 1,000 infants in eight cities in the United States. This book describes in detail the program, its research methodology, the progress of the program, and the results of the clinical trial. The program was administered by an interdisciplinary team composed of physicians, biostatisticians, child development specialists, and researchers from several disciplines. It was instituted upon the discharge of the infants from the neonatal nursery and was maintained for three years. One-third of the infants were randomly assigned to an intervention group, the remainder to a follow-up group. Infants in both groups received pediatric care and community referral services, but only those in the intervention group participated in a program that included extensive home visits, attendance at a child development center, and group meetings for parents. The results of the program proved to be clinically important; at age three, the children in the intervention group had significantly higher IQ scores, greater cognitive development, and fewer behavioral problems. The implications of the findings for public policy are equally important, for there is increasing interest in the prevention, early detection, and management of developmental disabilities in children, as evidenced by such legislation as the Education for All Children Act. Strategies to minimize the problems of low birth weight children, with their potential for long-term savings through the prevention of disabilities and their attendant costs, could have significant repercussions in such governmental areas as medical care, education, and social welfare.

Book excerpt: Adaptive emotion regulation begins with infants operating jointly with their parents to regulate their emotions, which fosters the development of independent regulation. Little is known about when or how this transition occurs, or the impact of factors such as parental availability or premature birth status. The current study examined the use of self-, parent-, and environment-reliant emotion regulation behaviours in full-term and healthy very-low-birthweight (VLBW) preterm infant-mother dyads at 5 ½, 12, and 18 months of age. At 5 ½ months, dyads participated in the Still-Face procedure (two normal interactions and one in which mothers are non-responsive and expressionless). At 12 and 18 months, dyads participated in two free-play interactions, a puzzle task, and an interference task. Emotion regulation behaviours were coded using two systematic, observational systems. Results indicated that infants used fewer self- and environment-reliant strategies as they aged, but more mother-reliant strategies. Increased use of self-reliant strategies at earlier ages predicted increased use of mother-reliant strategies at subsequent ages. Toddlers used more independent, attention-seeking, and escape behaviour during periods of maternal unavailability. There were no significant differences between full-term and VLBW/preterm toddlers' emotion regulation behaviours. The current study contributes to the understanding of normative development of emotion regulation and the risk associated with prematurity.

Book excerpt: This book presents the experience from a study that investigated executive function (EF) and sustained attention (SA) in preterm and full-term infants at 8 months after expected date of delivery and at 1011 months of age. Executive function and sustained attention emerge in infancy and continues to develop throughout childhood. Executive function and sustained attention is believed to underlie some learning problems in children at school age. Although numerous studies have reported that the overall development of preterm infants is comparable to that of full-term infants at the same corrected age, it is unclear to what extent the development of specific cognitive abilities is affected by prematurity and/or other factors such as medical complications. As preterm infants have a high rate of learning difficulties, it is possible that factors associated with prematurity specifically affect the development of some regions of the brain associated with the regulation of executive function and sustained attention. Thirty-seven preterm infants without identified disabilities, and 74 due date and gender matched healthy full-term infants, participated in the study. The preterm infants were all less than 32 weeks gestation and less than 1500 grams birth weight. The findings of the study showed that preterm infants performed significantly more poorly than full-term infants at both 8 months after the expected date of delivery and 1011 months chronological age on all measures of executive function and sustained attention. However the difference between preterm and full-term infants at 8 months after expected date of delivery was much less than at 1011 months chronological age. The results suggested that the effects of maturation are greater than the effects of exposure to extra uterine environmental stimuli on the development of executive function and sustained attention.
