Book excerpt: Competition and prospective payment systems have been widely used to attempt to control health care costs. Though much of the increase in medical costs over the past half-century has been concentrated among a few high-cost users of health care, prospective payment systems may provide incentives to selectively reduce expenditures on high-cost users relative to low-cost users and this pressure may be increased by competition. We use data on hospital charges and cost-to-charge ratios from California in 1983 and 1993 to examine the effects of competition on costs for high and low cost admissions before and after the establishment of the Medicare Prospective Payment System (PPS). Comparing persons above and below age 65 before and after the establishment of PPS, we find that competition is associated with increased costs before PPS in both age groups, but decreased costs afterwards, especially among those above age 65 with the highest costs. We conclude that the combination of competition and prospective payment systems may result in incentives to selectively reduce spending among the most expensive patients. This raises important issues relevant to the use Of competition and prospective payment to control costs and implies at minimum the need to carefully monitor outcomes for the sickest patients under prospective payment systems in competitive environments.

Book excerpt: Health Economics: An International Perspective is the only textbook to provide a truly international, comparative treatment of health economics. Offering an analysis of health systems across borders, the fourth edition of this key text has been updated and revised to take account of changes in a host of countries. This edition features an expanded introduction, providing better grounding for many of the examples that come in subsequent chapters and making it easier for non-health care experts to see the links between the theory, the examples and the health care system components. It also boasts a restructured format, dividing the book into two broad sections: the first focuses on ideas and principles, along with evidence on their applications in the health sector, whereas the second focuses on introducing core tools and techniques used in applied health economics research. Further updates to this edition include: two new chapters on applied econometrics; a new chapter on equity, focusing on equity in access to health care, paying particular attention to how access and need for health care are defined and measured in applied research; a new chapter on emerging issues for health systems that are emanating from a series of global transitions both within (e.g. demographic change, epidemiological change, the global resolution on universal health coverage) and without the health sector (e.g. economic transitions). Throughout the text, examples and illustrations are taken from a wide range of settings and world regions, providing a unique overview of the performance of different health systems.

Available in PDF, EPUB and Kindle. Book excerpt: The LSE Companion to Health Policy covers a wide range of conceptual and practical issues from a number of different perspectives introducing the reader to, and summarising, the vast literature that analyses the complexities of health policy. the Companion also assesses the current state of the art. Health policy is a wide-ranging subject covering many academic disciplines, but what most studies in health policy have in common is an interest in applying theory to improve practice. This Companion brings academic rigour to bear evidence on a range of central areas within health policy. It covers key issues on the quality, access and inequalities in health and health care; supply and health markets; insurance and expenditures; pharmaceuticals and new technologies: and aging and long-term care. This unique Companion on health policy contains the most important features for health system reform at a time of funding constraints and will therefore hold great appeal for policy analysts and makers, students, academics and management professionals.
