Some of these interventions are evidence-based; others are not. However, previous research has also found that parents select interventions for their children with ASD on the basis of several factors other than the evidence base, including their emotional representations of ASD and their perceptions of the timeline, consequences, and causes of ASD. Other research has found that people's cultural worldviews influence their decision-making on a variety of social and political issues. The current study examined the relationships between these factors and parents' perceptions of the efficacy of evidence-based and non-evidence-based interventions. Contrary to previous research, for parents of children with ASD, perceptions of intervention efficacy were not affected by their emotional representations of ASD or their perceptions of the timeline or consequences of ASD. There was a relationship between parents' perceptions of the causes of ASD and their perceptions of the efficacy of some interventions. Parents' cultural worldview was not related to their perceptions of the efficacy of evidence-based interventions, but was related to their perceptions of non-evidence-based interventions. Further research is needed to determine other factors that influence how parents perceive ASD interventions.

Bivariate correlations supported hypotheses 2, 3, and 4. Multiple regression was run to test hypothesis 5, which showed that positive perceptions were not significant moderators between the parent's stress and family functioning. The findings of the post-hoc analysis revealed that parenting stress and family functioning were significantly mediated by the parent's positive perceptions. The perception of children being a source of happiness and fulfillment showed partial mediation, and the perception of the children being a source of family strength and closeness showed full mediation. These findings are important in understanding the impact of positive perceptions on parenting stress and family functioning, having implications for future research, intervention approaches, and support group settings for parents of children with ASDs.

Book excerpt: Concurrent with the rising prevalence of autism spectrum disorders (ASDs) is the growing number of children with an ASD receiving community-based services. Several treatment methods, many of which are parent-mediated, have demonstrated efficacy in controlled research settings. Despite efficacy data, little is known about the effectiveness of such methods in community service settings. Specifically, limitations of this research include lack of a control comparison group and follow-up assessments to measure maintenance of treatment gains, and examining the influence of parent factors on treatment outcomes. To address these limitations, this prospective study examined the effectiveness of a 12-week, research-based, parent-mediated intervention delivered in a community service setting to 30 young children with ASD (mean age = 54.83 months; SD = 25.44) and their primary caregiver. This study had two primary aims: 1) compare child and parent outcomes for dyads in the intervention and community comparison groups from baseline to 12 weeks (immediately post-intervention) and 24 weeks (followup), and 2) examine parent factors as predictors of child outcomes for all dyads at 12 weeks (post-intervention) and 24 weeks (follow-up). Results indicated a strong, positive trend in improvement of child communication skills over six months for parent-child dyads in the intervention group compared to dyads in the community comparison group. Higher baseline parenting stress was significantly related to smaller improvements in child social skills from baseline to 12 weeks. Overall, the enhanced methodological rigor of this study provides further support for the utility of delivering research-based parent-mediated interventions in community service settings to children with ASD and their caregivers. Clinical implications of this study include the need for increased targeted screening of parenting stress levels upon entry into parent-mediated intervention services.

Book excerpt: Treatment outcomes in parent-mediated interventions are associated with the parent's engagement and acceptance, treatment acceptability, of the intervention. Research within the field of behavioral interventions has found that parental attributions, the causal explanation's parents make to understand the meaning of their child's behavior, are associated with treatment acceptability and parental engagement for behavioral parent-focused intervention. However, this relationship has not been investigated for parent-mediated interventions focused on core symptoms of autism spectrum disorder (ASD), such as social and communication skills. The primary goal of this exploratory study was to investigate the relationship between parental attributions for ASD-related behavior and treatment acceptability and engagement for parent-mediated intervention for ASD. N = 107 parents or primary caregivers of a child aged 3-6 years with an ASD diagnosis were asked to rate their treatment acceptability and engagement of a parent-mediated intervention and complete the Parental Attribution Questionnaire, among other measures. Parental attributions for all dimensions of child-referent attributions (internality, controllability, and stability) negatively predicted parent's ratings of treatment acceptability for parent-mediated intervention. No associations were found for any dimensions of parent-referent attributions. Results revealed that only the parental attribution of child-referent stability was related to engagement for parent-mediated intervention. Given that parental attributions can be readily changed prior to the start of the intervention, a better understanding of the role of parental attributions in parent-mediated interventions could help improve treatment outcomes for children with ASD.

Book excerpt: Parent-mediated interventions (PMI) for toddlers with Autism Spectrum Disorder (ASD) are becoming increasingly more common due to their cost effectiveness and opportunities for skills generalization. Parent intervention fidelity is critical for child outcomes in PMI; however, little is known about which parent characteristics may impact parent intervention fidelity. Parenting stress may be an important first factor to explore as it has been well documented that parenting stress is higher in parents of children with ASD, and it has close links to other important factors including socio-economic status and overall parent well-being. This study, using 49 parents of children at elevated risk for developing ASD (i.e., have an older sibling with an ASD diagnosis), sought to examine if parenting stress impacted one component of parent intervention fidelity, namely parents’ use of intervention strategies, following participation in a parent-mediated intervention. Regression analyses revealed no significant impact of parenting stress on parents’ use of intervention strategies. Potential reasons that a significant relation was not found are discussed. Given the increased shift towards PMI, research should continue to examine how the role of parent characteristics interact with parent intervention fidelity so we can understand how to best serve parents as they support their children, through parent-mediated interventions.
