Focus needs to be placed on strategies to improve graduate nurse retention. Graduate nurse retention impacts organizations financially and through direct patient care. The financial impacts are salary, hiring expenses, orientation expenses, and replacement costs. Direct patient care is affected through patient satisfaction, job satisfaction, and quality of care. A quantitative, non-experimental, descriptive correlation study was conducted using questionnaires to determine if a nurse transition program improved graduate nurse retention during the first year of employment. Benner's Novice to Expert Model was used as the framework for the study. A convenience sample of graduate nurses from a midwestern teaching hospital with a nurse transition program was used. Chi-square test analysis was performed and determined no significant difference in retention rates. However, the data collected using the Casey-Fink Graduate Nurse Experience Survey (2004) did show that the nurse transition program had a significant positive impact on perceived experiences of graduate nurses such as increased comfort/confidence, work relationships, skills/procedures performance, and an easier transition to the work environment. Nurse transition programs have a potential positive impact on graduate nurses' perceived experiences.

Book excerpt: Newly licensed graduate nurses are transitioning to practice with low self-efficacy and competency gaps leading to poor patient outcomes and low retention rates within this population of nurses. These low rates of retention create an environment in which newly licensed graduate nurses have failed or are delayed in their transition from novice to expert. The purpose of this study was to evaluate the impact of the implementation of the Vizient/AACN nurse residency program on the first-year retention rate of newly licensed graduate nurses within an organization in central Texas. The program was evaluated utilizing newly licensed graduate nurse pre- and postimplementation data related to retention rates of the organization and the Casey Fink Graduate Nurse Experience Survey. The study design was a quantitative approach utilizing a cross-sectional survey. A convenience sample of newly licensed graduate nurses hired from November 2017 to September 2018 was used. All 48 residents enrolled in the nurse residency program were included in the study. Data were analyzed utilizing a power analysis and a paired- samples t test. The sample size was lower than 43, indicating a low confidence level. A strong correlation was noted for a standardized NRP and self-efficacy (r = .822), but there was no significant difference identified between pretest and posttest means (p > .05). Retention rates improved by 11.65% following implementation of the NRP. Findings of this study can influence the development of standardized NRPs which could increase self-efficacy and reduce turnover rates among newly licensed graduate nurses.

Design: An integrative literature review was conducted of research studies on nurse residency programs in acute care settings. Kramer's Reality Shock Theory guided this review. Methods: Research published in the U.S. between 2007-2013, was obtained from electronic data searches using the key works "new graduate nurses", "novice nurse", "reality shock", and "nurse residency programs." Outcomes were compared and synthesized into major themes. Findings: Major themes include an increase in competency, satisfaction, peer support, confidence, commitment to the profession, and retention of nurses who completed yearlong residency programs. Organizational cost savings was also found. Conclusions/Relevance: The findings support previous research recommendations that the ideal length of nurse residency programs is one year; and the essential program components include preceptors or mentors and didactic education. This study findings show advance evidence of the important effects that nurse residency programs have on new graduate nurses' transition into practice, including a reduction in reality shock.

While organizations have implemented structures and processes to support new nurse transition into practice, new nurse turnover continues. The retention of new nurses is important to sustaining a stable workforce, as they are utilized to fill vacancy gaps in acute care settings. In addition, the retention of new nurses is important to patient safety and quality of care. Current research suggests that turnover rates among new nurses have decreased in some organizations, namely those that have implemented residency programs. Questions remain as to whether the decrease in turnover rates is due to the implementation of these innovative programs or a response to the current economic environment. Both qualitative and quantitative studies identify factors that contribute to the difficulties of the transition. These studies have focused primarily on developmental factors such as competence, confidence and support. This qualitative study utilized van Manen's (1990) phenomenological approach to gain meaningful insight into and understanding of the difficulties new nurses face as they transition from the role of student to practicing nurse. This study is unique in that it provides a comprehensive perspective to fully examine the difficulty of the transition for new nurses. Four major themes emerged from this study's findings: transitioning into the role of professional nurse, applying knowledge to practice, navigating the organization, and building relationships. Nurse executives and leaders as well as nurse educators in academia may find these results useful to further develop and implement innovative strategies that address new nurse transitions from a broader perspective and with a more comprehensive lens.

Book excerpt: The purpose of this study was to investigate the transition experience of new graduate nurses during their first work experience after graduating from a baccalaureate undergraduate nursing program. A case study approach was used to examine a community hospital in Connecticut. A random sample of baccalaureate prepared new graduate nurses who graduated in May 2007, and were in the first year of working as registered nurses, was chosen. The research question examined how new graduate nurses perceived the supports provided within their transition experience. The study examined the supports through Benner's (1982) developmental theory of nursing. The four supports included: support during orientation, support from experienced nurses, supports to develop time management skills and support from their undergraduate nursing education. The four research questions addressed each of these supports. The study used three methods of data collection. Twelve new graduate nurses completed the quantitative survey, eight participated in the individual interview and four new graduate nurses participated in the focus group interview. The conclusions included the following: new graduate nurses were supported by the learning environment of the orientation; new graduate nurses perceived a supportive network was available to them within the hospital environment; time management skills are essential for the transition to entry nursing practice; new graduate nurses were assisted by their undergraduate school experiences during the transition period. Recommendations for practice have implications for making new graduates successful in a nursing career. Hospital orientations should provide preceptors who encourage autonomy and are consistent and provide emotional support and need to provide support in the transition period by mentoring and encouraging guidance from experienced nurses. Hospitals also need to provide clinician guidance and organizational resources to assist with time management, and offer intern programs to students prior to graduation. Undergraduate nursing programs should incorporate more clinical time to help the new graduate nurse transition into nursing practice. Recommendations for further research might involve a qualitative in-depth study regarding time management as well as an in depth case study examining new graduate nurses perceived level of support during the orientation period.

Book excerpt: The first year of professional employment as a Registered Nurse is an exciting yet somewhat stressful time for the new graduate nurse. Most health care institutions provide opportunities to make the transition from student nurse to professional nurse through a new graduate nurse orientation. These orientations vary widely in the amount of time allowed, the resources provided the new graduate nurses, and the experiences afforded the new graduate nurses. Much of the research of these orientation programs looks at the impact of the orientation on retention of new graduate nurses. The body of research in this dissertation attempts to identify how and education-based new graduate nurse orientation contributes to retention by identifying essential elements of that orientation. Through a single case, explanatory, holistic case study multiple sources of evidence including archival data, one-on-one interviews with new graduate nurses who completed the orientation program, and a focus group interview of new graduate nurses who completed the orientation program were examined to answer the question of how the orientation impacts retention and which elements are essential for retention, satisfaction, increased competency levels, and ease of transition for the new graduate nurse. Using a theoretical framework including Chickering's psychosocial model of development and Romiszowski's analysis of knowledge and skill development, it is concluded that preceptorship is the essential element of new graduate nurse orientation that promotes new graduate nurse retention, satisfaction, increased competency levels, and ease of transition. Preceptorships facilitate the socialization necessary in orientation for new graduate nurses' success by affecting all elements of young adult development as well as providing the necessary continuation of acquisition of nursing knowledge and skills for the novice nurse. Implications of these findings as well as suggestions for future research are provided.
