Book excerpt: The field of physical therapy has seen significant change over the years with the continuously evolving health care system. The increase in knowledge and responsibilities required by physical therapists has led to the progression of education requirements to a doctoral degree for entry-level practice. Physical therapist assistants have not seen a similar elevation in education, and degree requirements remain at the associate level. Limitations on the PTA program length hinder the addition of informational material. Many program directors feel the current time constraints create a challenge to complete the education standards. Progressing educational requirements to a baccalaureate level may be challenging with many of the current programs located in community colleges. A discussion on the community college baccalaureate degree and articulation agreements will serve as a potential solution to facilitate degree progression. An analysis of the current PTA program curriculum at Washburn University will lead to recommendations for curricular expansion for degree progression. Utilizing the respiratory therapy program as a model for the transition of degree requirements from an associate's degree to a baccalaureate, recommendations will be made to guide the conversion of the PTA program to the baccalaureate level. Reviewing the course work requirements in DPT education will serve as a guide to enhance the foundational knowledge delivered in PTA education.

Book excerpt: The Commission of Accreditation of Physical Therapy Education (CAPTE) mandates the integration of critical thinking into physical therapy curriculum. However, it has been unclear what contributes to a physical therapist's critical thinking skills. The purpose of this study was to examine if years of experience and level of education affect critical thinking skills of physical therapists. Participants included 100 pre- and post-MPT graduates. Ages ranged from 20-60 years old with 40% of the participants holding a bachelor degree, 48% an entry-level masters degree in physical therapy (MPT), and 18% an advanced graduate degree in addition to the MPT. Years of experience ranged from 0-33 years. Participants were recruited through electronic mail. Accepted study participants completed both an online demographics questionnaire and the Watson-Glaser Critical Thinking Appraisal (WGCTA), a measure of critical thinking skills. The WGCTA has been shown to have both validity and reliability, in addition to being a widely used critical thinking appraisal tool. Two one-way ANOVA (analysis of variance) revealed a significant difference between critical thinking scores of those with less than one year experience compared to those with greater than 10 years of experience, and a significant difference between critical thinking scores of those with a bachelor degree compared to those with an MPT degree. A 3 x 3 ANOVA revealed no significant difference when looking at the interaction between participants with a bachelor degree and less than 10 years of experience compared to participants with a doctorate and greater than 10 years of experience. This suggests years of experience and level of education alone are not enough to affect critical thinking scores. Further, the WGCTA may not be an appropriate assessment tool when assessing critical thinking skills of persons with post graduate degrees.

Book excerpt: In 2000, the American Physical Therapy Association presented their Vision 2020 statement that includes the premise that all physical therapists will be at the doctoral level by 2020. The purpose of this qualitative study was to identify the perceptions and perspectives of practicing physical therapists in Western New York State relative to the change in entry-level requirements to the doctoral level, to establish an enhanced understanding of the needs of the profession as articulated by practicing clinicians, and to assess whether education is driving the practice of physical therapy or whether practice is driving physical therapy education. Data were analyzed using the grounded theory of analysis, which led to the core category or central concept of this study, coined Being-Out-of-the-Loop . The Basic Social Process that emerged, which captured the essence of the participants' perspectives and perceptions relative to doctoral entry-level requirements was also Being-Out-of-the-Loop. Participant responses clearly demonstrate that they were unaware, uniformed, and Out-of-the-Loop when addressing the doctoral entry-level initiative. The Basic Social Process identified in the study is represented in a theoretical model made up of six elements (sub-categories), which frame the respondents' overall perspectives and perceptions of doctoral entry-level requirements. Additionally, findings of this study suggest that participants believe that there is a need to expand the entry-level curriculum and that there are benefits and liabilities associated with moving to doctoral preparation. Overwhelmingly, participants support a residency type clinical training model. They share the belief that doctoral preparation provides the best opportunity to improve the status and future of the profession, and from their responses, it is apparent that practice is driving education. Implications of this study include the need for the members of the profession at-large to recognize that the majority of physical therapists are unaware of the doctoral initiative and the Vision (2020) of the profession; moreover, the lack of clinical support for doctoral preparation seems to be due to their degree of uncertainty and not related to their clinical judgment.

Book excerpt: Introduction: Leadership has been identified as an important attribute for health professionals, including physical therapists. The American Physical Therapy Association (APTA) Health Policy and Administration Section (formally the Section on Administration) developed a position and model on Leadership, Administration, Management, and Professionalism (LAMP) processes. There are several studies revealing leadership as a needed skill for the new graduate physical therapist. Leadership is described in this study using the transformational leadership theory. Kouzes and Posner, after extensive investigation using the transformational leadership theory, developed the Leadership Practices Inventory: Self Instrument, 3rd edition (Self LPI) to identify five key exemplary leadership behaviors. Literature Review: There are several studies that reported the importance of leadership in new graduate physical therapists and the physical therapy profession. However, there are no studies that reported the development of the leadership skills in professional entry-level doctor of physical therapy (DPT) students. The purpose of this study was to describe the leadership practices of year one (YR 1) student physical therapists enrolled in a professional entry-level DPT program. The identification of leadership practice of YR 1 DPT student physical therapists may provide a baseline measurement of leadership practices of students entering the physical therapy field. Subjects: Students in the first semester of the first year of a professional entry-level DPT program enrolled in six Commission on Accreditation for Physical Therapy Education (CAPTE) accredited Midwestern universities. Methods: This was a descriptive study using the Self LPI and a demographic survey. Meetings were held at the six universities with a monetary incentive offered for students who participated in the study. Results: The mean leadership practices of YR 1 DPT students in highest to lowest rank order were Enable, Encourage, Model, Challenge, and Inspire. These results were similar to the rank of the general population, including health care providers, with a rank order of Enable, Model, Encourage, Challenge and Inspire according to Posner. Students were found to be Middle High assessors of the leadership behaviors on the Self LPI. The YR 1 DPT students' top three Self LPI statements mirrored those of the general population. Age was found to be a significant predictor for all five leadership practices with the greatest impact on the Challenge leadership practice. There was also a significant difference with university affiliation and the Encourage leadership practice. Discussion and Conclusion: The rank of the five leadership practices provided insight about the leadership practices of students who have just entered a professional entry-level education program for physical therapy. The results of this study provided baseline leadership practices of YR 1 DPT students and recommendations to assist educators with curriculum content focused on leadership development.

The occupation, now a profession, complete with autonomy, direct access, and great responsibility for decision-making for those we care for has changed; however, the need for physical therapists to care for those with physical disabilities has not changed. In order for physical therapists to manage care, responsibility, and autonomy in the ever-changing economic and healthcare environment, physical therapist education programs need, as part of the admission process, some standardization to help them select the best students who have the potential to succeed academically, as well as, pass the national licensure examination. Therefore, the purpose of this study was to determine which critical thinking skills and higher order constructs would be essential to assess on an admission examination for entrance into physical therapist education programs. A modified e-Delphi was used to answer this question by surveying a group of experts determined by the criteria of being a licensed physical therapist who has been published for their research on critical thinking and higher order thinking, as well as, program directors of physical therapist education programs. The critical thinking skills of clarifying meaning, assessing claims, assessing arguments, stating results, self-examination, and self-correction, and the higher order thinking constructs of critical thinking and logical thinking emerged as the consensus items to be assessed in a discipline-specific examination prior to entrance into a physical therapist education program.
