Book excerpt: The localized muscular fatigue, acute loss of glenohumeral strength and passive range of motion (PROM), and delayed onset muscle soreness (DOMS) that intercollegiate and professional baseball pitchers experience following a game must be minimized or eliminated before they can safely pitch in their next outing. The combination of light shoulder exercise, stretching, and cryotherapy is commonly used after baseball pitching as the recovery method to hasten the restoration of shoulder muscular strength and PROM, and reduce the symptoms of DOMS. However, the effectiveness of the light shoulder exercise program for recovery after baseball pitching remains unclear. Objective: To compare the effectiveness of two recovery methods, stretch and cryotherapy (SC), and a light shoulder exercise program, stretch and cryotherapy (LSC), after simulated game pitching by collegiate baseball pitchers.

Book excerpt: Decreases in glenohumeral (GH) rotation motion and rotator cuff strength have been linked to multiple pathologies in the throwing arms of baseball pitchers, but a full understanding of the shoulder mechanisms affected by a total arc of motion deficit (TAMD) has not yet been achieved. This study was designed to quantify the effects that this loss in range of motion has on GH joint laxity and eccentric external and internal rotation strength measures. We recruited 47 intercollegiate baseball pitchers with no history of shoulder or elbow surgery, and assigned them to either a TAMD group or a Non-TAMD group based on the extent of their loss of rotational motion at the shoulder. Eccentric internal and external rotation peak force values were evaluated in the middle 70% of the pitchers' available rotational range of motion with an isokinetic dynamometer, while anterior and posterior GH joint laxity and stiffness were assessed with a multijoint arthrometer under a 150N load applied at a rate of 15-19 N/s, with stiffness calculated as the slope of the force displacement curve following the inflection point representing the point of soft tissue compression. We found significant differences in the stiffness measures when comparing the anterior to the posterior directions in both the dominant (22.64 ± 5.02 N/mm and 28.72 ± 7.18 N/mm, p

Available in PDF, EPUB and Kindle. Book excerpt: Treat your own rotator cuff? Who needs to worry about that? According to the medical research, a lot of people. The rotator cuff, a group of four, flat tendons that connect to the critical muscles that stabilize your shoulder, can cause a lot more problems than you might think. Consider a few of these statistics from the published literature: .It's simply just a matter of time until the majority of shoulders get a rotator cuff tear. According to Magnetic Resonance Imaging (MRI) scans, approximately 4% of people under forty years of age have a torn rotator cuff. After age sixty, however, 54% of people have one (Sher 1995). .Once the rotator cuff gets torn, it doesn't look good either. One study followed a group of patients with tears in their rotator cuffs and found that 80% of the them went on to either enlarge or turn into full thickness tears-in less than a two-year period (Yamanaka 1994). As you can tell, rotator cuff problems aren't just for elite athletes. Seriously consider investing just a few minutes a week doing the simple exercises in this book if you: .have been diagnosed with either a partial or full thickness rotator cuff tear (yes, many studies show that even full thickness tears can be helped with exercise ) .experience shoulder pain .do upper body weight lifting .have a job or play a sport where you do a lot of work with your arms above shoulder level .have been diagnosed with "impingement syndrome" .want a healthy and properly functioning rotator cuff So whether you already suffer from a rotator cuff problem, or simply want to prevent one, Treat Your Own Rotator Cuff will guide you step-by-step through an evidence-based program that can iron-plate your shoulders in just minutes a week. Jim Johnson, P.T., is a physical therapist who has spent over fifteen years treating both inpatients and outpatients with a wide range of pain and mobility problems. He has written many books based completely on published research and controlled trials including The Multifidus Back Pain Solution, Treat Your Own Knees, The No-Beach, No-Zone, No-Nonsense Weight Loss Plan: A Pocket Guide to What Works, and The Sixty-Second Motivator. Book excerpt: ABSTRACT: The purpose of this study was to examine the effects of a 6-week Postural Restoration (PRI) based strength training protocol versus a 6-week control group on strength, power, and flexibility in collegiate baseball players. Collegiate baseball players (N = 28; 20.36 ± 1.35) at a Division III college in New England participated in this study. Participants were divided into a treatment group (n = 15) and a control (n = 13) group. Pretesting was conducted for an estimated 1-repetition max Front Squat (strength), a bilateral vertical jump (power), and glenohumeral (GH) and femoroacetabular (FA) range of motion (flexibility) measurements. Twelve strength training sessions were conducted for both groups with the treatment group performing pre-exercise PRI based exercises as well as additional PRI based exercises during the training session. Posttesting was completed following the 12 training sessions. Postural Restoration techniques were found to have no statistically significant effect on strength and power when compared to a standard 6-week strength training protocol. Although not statistically significant, Postural Restoration techniques exhibited an additional 1.7% increase in strength measurements when compared to a standard strength training protocol. A significant interaction exists between the PRI-treatment group vs the control group with respect to Right Arm Internal Rotation measurements. The 1.7% increase in the treatment group could be from improvements in hip and trunk strength, posture and core control when bracing a loaded spine during the front sguat exercise. Primary improvements in Right Arm Internal Rotation measurements may be a result of a primary right hand dominant population in a time of year where throwing adaptations cannot be monitored. PRI-based protocols were hypothesized to produce greater improvements in strength, power, and flexibility to a greater extent in comparison to the strength training-control group following the six-week training program, there were no indications of a more favorable prescription method. Thus, both protocols could provide egual benefit to improving strength, power, and flexibility with respect to training collegiate baseball players during the preseason training months.

Book excerpt: The purpose of this study was to provide information concerning the effects of a posterior capsule stretching program on the internal range of motion (R-O-M) of overhead athletes as well as to determine if there was any impact on the number of players experiencing shoulder injuries. Thirty-five Division I collegiate baseball players had the degree of shoulder internal rotation (IROT) assessed in both their dominant and nondominant arms (pretest) and were subsequently divided into those who exhibited GIRD (glenohumeral internal rotation deficit; n=27) and those who did not (n=8). Then the Sleeper Stretch was taught to each player and utilized over the course of a 12-week period. Intermittent (every 4 weeks) as well as posttreatment reassessments were performed to determine changes in R-O-M across the length of the study. Parametric and nonparametric analyses indicated a significant gain of 9° of IROT over the course of the study, with the most prominent (6°) gain occurring between weeks 8 and 12. No differences between the GIRD and non-GIRD groups were noted. In addition, no shoulder injuries occurred during the 2010 season, although the comparison to the injury rates of the previous three seasons failed to be statistically significant. Clinically, an increase in R-O-M, coupled with the absence of shoulder injuries, suggests that the Sleeper Stretch could be a promising preventative measure for overhead athletes.
