Book excerpt: Excerpt from Edinburgh Medical Journal, 1895 On Sunday morning, June 11th, about 8 a.m., two hurried mes sages arrived to go at once to the patient's house; and as the second of these indicated what had happened, I sent a messenger in a cab to Prof. Annandale, so that we arrived almost simultaneously. His mother, the only responsible person in the house, informed us that it was his usual custom to rise early on a Sunday morning and go over to his garden, and then return to breakfast. Being a little later than usual, she knocked at his door to inquire how he was, but was informed that he only wanted to be left alone. A minute or two after she heard a report, and entering the room found her son lying stretched out on bed with a smoking revolver in his hand, and a hole in his forehead, from which a tremendous amount of blood was rapidly flowing. With extraordinary presence of mind she removed the weapon and bound up the head with a pledget of cotton wool and a bandage, and sent first for a friend and then for medical assistance. Before this gentleman arrived, without any aid twice did the patient walk to the bath-room, lock the door, and attend to himself; and when the revolver was lifted, cried out to his friend, For God's sake, man, be careful that no one is hurt, for it is loaded. The day previously he had been at his work as usual, had attended to the payment of the men under him, and on the afternoon went a drive into the country with some friends, when he seemed perfectly happy and in his ordinary health and spirits. In every one's opinion he was the last person in the world to associate with suicide; and except for a growing irritability in temper, was the sanest man in the world, with no delusions whatever other than that his work was too heavy for him. Book excerpt: Excerpt from Edinburgh Medical Journal, 1898, Vol. 4 Though reliance cannot be placed on the finger for the advancement of knowledge, and though it may prove now and then somewhat fallacious in the course of work, it nevertheless remains the most valuable and indispensable instrument the physician possesses for the examination of the pulse; and I am persuaded it is capable of a higher degree of efficiency, when trained by the side of methods which amplify and render more definite all the practical aspects of pulse feeling. The sphygmograph has likewise served its day, and is no longer likely to reveal anything beyond the many facts it has brought to light, now incorporated in current clinical knowledge. Like a scaffolding, it was necessary to construction. From the clinical standpoint it is of little or no practical value; and even as an instrument of research it fails to record two important facts pertaining to the arterial system, namely, the blood pressure and the varying calibre of the arteries. During the past few years I have endeavoured to study more particularly these aspects of the arterial circulation; and in this series of articles I propose to furnish some of the results of my observations, and preferably those results which have a practical bearing on clinical work. Wherever the elements of that personality are suspended or modified, justice requires a corresponding suspension or modifica tion of the law. In every case, therefore, to which we apply the ordinary rules of law, we assume the existence of the ordinary mental functions and their uninterrupted development up to a period of life at which we presume the individual to have learned to appreciate sufficiently his relations to others, and to have acquired sufficient power to regulate his acts by his experience. When these conditions are absent or notably imperfect, or when they are for the time in abeyance, the law has ever refused to permit the exercise of rights and has abstained from imposing responsibility for crime. The individual is subjected to new rules, and his acts lose their ordinary legal significance. The cases in which the law is thus suspended by abnormal conditions of the mental functions depend almost wholly on conditions of disease; and as these fall peculiarly within the province of medicine, it plainly belongs to that science to investi gate the nature and extent of these aberrations. The subject from this point of view is a branch of forensic medicine in which the results of medical science are arranged for the information of the lawyer, with only so much scientific theory and proof as is necessary to enable him to judge of the application of the general principles to the individual case. Book excerpt: Excerpt from Edinburgh Medical Journal, 1916, Vol. 16 Although the year which has just passed has not been distinguished by any great advance in medicine - unless it be reckoned an achievement that, thanks to science, millions of men have lived in camps through the seasons without being decimated by infectious disease - research has not stood still. A survey of the periodical literature of the year reveals many contrasts with 1914. The patching and mending the physically less fit no doubt goes on, but there has been less writing about it - about the surgery of the over-civilised colon, for instance, or about the diagnosis of fortuitous incurable lesions of the nervous system, or the toxic effects of eating wrongly or too much. The most prominent place in medical literature has been occupied by work designed to prevent, or repair, the ravages of war. Speaking only in the most general way, it may be said that the teaching of military surgery and medicine in previous wars has proved of comparatively small value, except as regards organisation, in the present conflict. Many of the lessons of the South African \var, in particular, have had to be unlearned. There our army was small, the ri e was the principal weapon, transport was bad, typhoid was rife, and sepsis was trifling; here the armies are immense, the weapon is the high explosive, transport is unsurpassed, typhoid has been abolished, and sepsis stands out as the great danger. It very quickly became apparent, to the surprise, we imagine, of those whose experience depended on the results of civil surgery at home, that our present methods of dealing with sepsis in war are far from effectual.
