Book excerpt: Introduction: Pediatric overweight and obesity is a public health crisis, affecting one third of children and over two thirds of adults in the United States (US). The US Preventive Services Task Force (USPSTF) recommends screening for obesity and offering intensive or specialty lifestyle intervention or referral. Recent analyses revealed a family-based social facilitation weight loss maintenance intervention delivered at high intensity ("SFM-High") resulted in significantly greater change in children's percent overweight at 1-year compared to lower intensity conditions (Wilfley et al., in preparation). Thus, the purpose of this dissertation is to conduct the first cost-effectiveness analysis of the recommended treatment strategy, SFM-High, compared to standard care for obesity (i.e., no treatment control, "NTC") with a lifetime horizon. It was hypothesized that SFM-High would be cost-effective for the treatment of obesity in children and parents. Methods: Using TreeAge software, two cost-effectiveness decision trees were built to evaluate the incremental cost effectiveness of SFM-High (N=59 parent-child dyads) compared to NTC for the treatment of obesity. The incremental costs of SFM-High compared to NTC were divided by the incremental effectiveness to estimate the incremental cost-effectiveness ratio (ICER), which provides a metric for the resources required (i.e., dollars) to obtain a unit of effectiveness (i.e., life years gained). If a treatment is considered dominant, it is less costly and more effective than the comparative strategy. Results: The ICER for treating children with SFM-High compared to NTC was $17,877 per life year gained. The ICER for treating adults with SFM-High compared to NTC was dominant. Approximating the cost-effectiveness of treating children and parents using the family-based SFM-High intervention showed that the ICERs for treating the child and treating the parent were dominant compared to NTC. Conclusions: Results of the present analyses show that the family-based SFM-High intervention is cost-saving for the treatment of obesity in children and parents. As such, the present results provide an important, initial quantification of the costs and benefits of obesity intervention, offer preliminary optimism regarding the cost savings of a family-based social-facilitation weight loss maintenance intervention approach, and indicate areas for future study.

Intervention: 'Families for Health' is a 12-week programme, with parallel groups for parents and children, combining support for parenting, lifestyle change, as well as social & emotional development. Design of the Evaluation: Pilot study using mixed-methods comprising: process evaluation; outcome evaluation involving a 'before and after' evaluation and triangulation with interview data; economic evaluation (cost-outcome description); users and providers perspectives. Setting: Leisure Centre, Coventry, England. Participants: 27 overweight or obese children aged 7-13 years (18 girls, 9 boys) and their parents, from 21 families. Process Evaluation: Two groups were run, and were delivered as planned. Recruitment was difficult, although most effective via the media. Attendance rate was 62%, with 18 (67%) children completing the programme. Outcome Evaluation: Primary outcome was change in the BMI z-score from baseline. For 22 children with follow-up data, BMI z-score was significantly reduced by -0.18 (95% CI -0.30 to -0.05, p=0.008) at the end of the programme, and was sustained to 9 months ( -0.21) and 2 years ( -0.23). There were also significant improvements in the children's quality-of-life, eating and activity environment, child-parent relationships and parents' mental health. Fruit and vegetable consumption, participation in moderate/vigorous exercise, and children's self-esteem did not change significantly. Interview data illustrated the changes made by the families, particularly to their eating environment. User and Provider Perspectives: The group-based parenting approach was received well, providing the 'tools' for parents to become 'agents of change' in the family. Suggested changes to the programme include providing follow-up sessions and a greater focus on physical activity. Economic Evaluation: Costs to run 'Families for Health' were £517 per family or £402 per child, in-line with other group-based obesity management or parenting interventions. Conclusion: 'Families for Health' is a promising new intervention for the management of childhood obesity.

Children with obesity are more likely to have poorer health-related quality of life (HRQOL) and poorer weight-specific quality of life (WSQOL) compared to their peers. The impact of weight management interventions on children’s HRQOL and WSQOL is not fully understood. Whether and how weight loss changes parents’ and children’s perceptions of the children’s quality of life can provide insight into interventions’ effectiveness and highlight opportunities for adapting interventions.Objective: To determine the association between pediatric participants’ weight loss and change in their quality of life, specifically HRQOL and WSQOL, among participants enrolled in family-based behavioral weight management interventions. This study used a one-arm quasi-experimental design and combined data from two previous pediatric weight management intervention studies. Parents with overweight/obesity and their children ages 7 to 11 with overweight/obesity participated in a 20-week, family-based weight management intervention focused on increasing healthier lifestyle eating and activity behaviors. Child height, child weight, and child self-reported and parent proxy-reported HRQOL and WSQOL were collected. Child self-reported and parent proxy-reported HRQOL and WSQOL scores were compared. Multiple regression was used to assess the association between change in child BMI z-score and change in child HRQOL and WSQOL. Parent proxy-reported WSQOL was significantly lower than child self-reported WSQOL at all timepoints, but quality of life significantly increased with treatment. Change in child BMI z-score was significantly negatively associated with change in child self-reported total WSQOL (improvements in child weight status associated with improvements in weight-related quality of life) but not child self-reported total HRQOL. Change in child BMI z-score was also significantly negatively associated with parent proxy-reported HRQOL and WSQOL. The change in children’s self-reported quality of life does not track with changes in their weight status as much as changes in parents’ perceptions of child quality of life improvements. Parent perceptions of child HRQOL and WSQOL indicate significant improvement with intervention that is more linked to child weight status changes. Pediatric weight management interventions may need modification to show children how their weight loss is related to their improving quality of life.
