Book excerpt: Colorectal cancer is one of the major malignancies affecting westernized societies, both in terms of incidence and as a cause of mortality: it is, however, a preventable and a detectable disease. Although most countries do not actively promote preventive policies for their general population, this trend is gradually changing. In line with this current development, Colorectal Cancer in Clinical Practice examines prevention and early detection of this disease, as well as discussion of management issues. It will provide the clinician with an accessible and up-to-date guide on the primary and secondary prevention of colorectal cancer, and will be of particular interest to primary care physicians and specialists in gastroenterology, oncology and surgery.

Book excerpt: Cancer ranks second only to heart disease as a leading cause of death in the United States, making it a tremendous burden in years of life lost, patient suffering, and economic costs. Fulfilling the Potential for Cancer Prevention and Early Detection reviews the proof that we can dramatically reduce cancer rates. The National Cancer Policy Board, part of the Institute of Medicine, outlines a national strategy to realize the promise of cancer prevention and early detection, including specific and wide-ranging recommendations. Offering a wealth of information and directly addressing major controversies, the book includes: A detailed look at how significantly cancer could be reduced through lifestyle changes, evaluating approaches used to alter eating, smoking, and exercise habits. An analysis of the intuitive notion that screening for cancer leads to improved health outcomes, including a discussion of screening methods, potential risks, and current recommendations. An examination of cancer prevention and control opportunities in primary health care delivery settings, including a review of interventions aimed at improving provider performance. Reviews of professional education and training programs, research trends and opportunities, and federal programs that support cancer prevention and early detection. This in-depth volume will be of interest to policy analysts, cancer and public health specialists, health care administrators and providers, researchers, insurers, medical journalists, and patient advocates.

Book excerpt: A Working Group of 23 independent experts from 15 countries, convened by the International Agency for Research on Cancer (IARC) in November 2017, reviewed the scientific evidence and assessed the cancer-preventive and adverse effects of various methods of screening for colorectal cancer. Colorectal cancer is the third most common cancer in men and the second most common in women worldwide, and represents more than 10% of the global cancer burden. This publication provides evidence-based evaluations of the effectiveness of colorectal cancer screening in reducing colorectal cancer incidence and mortality. The Working Group also reviewed the body of evidence on the comparison of endoscopic and stool-based techniques, on the determinants of participation in screening programs, and on the most mature emerging techniques that may be alternatives to current practices for colorectal cancer screening, as well as presenting the different categories of high-risk populations and the surveillance strategies for such individuals.

Book excerpt: Colorectal Cancer Screening provides a complete overview of colorectal cancer screening, from epidemiology and molecular abnormalities, to the latest screening techniques such as stool DNA and FIT, Computerized Tomography (CT) Colonography, High Definition Colonoscopes and Narrow Band Imaging. As the text is devoted entirely to CRC screening, it features many facts, principles, guidelines and figures related to screening in an easy access format. This volume provides a complete guide to colorectal cancer screening which will be informative to the subspecialist as well as the primary care practitioner. It represents the only text that provides this up to date information about a subject that is continually changing. For the primary practitioner, information on the guidelines for screening as well as increasing patient participation is presentedd. For the subspecialist, information regarding the latest imaging techniques as well as flat adenomas and chromoendoscopy are covered. The section on the molecular changes in CRC will appeal to both groups. The text includes up to date information about colorectal screening that encompasses the entire spectrum of the topic and features photographs of polyps as well as diagrams of the morphology of polyps as well as photographs of CT colonography images. Algorithms are presented for all the suggested guidelines. Chapters are devoted to patient participation in screening and risk factors as well as new imaging technology. This useful volume explains the rationale behind screening for CRC. In addition, it covers the different screening options as well as the performance characteristics, when available in the literature, for each test. This volume will be used by the sub specialists who perform screening tests as well as primary care practitioners who refer patients to be screened for colorectal cancer.

Book excerpt: The IOM's National Cancer Policy Board estimated in 2003 that even modest efforts to implement known tactics for cancer prevention and early detection could result in up to a 29 percent drop in cancer deaths in about 20 years. The IOM's National Cancer Policy Forum, which succeeded the Board after it was disbanded in 2005, continued the Board's work to outline ways to increase screening in the U.S. On February 25 and 26, 2008, the Forum convened a workshop to discuss screening for colorectal cancer. Colorectal cancer screening remains low, despite strong evidence that screening prevents deaths. With the aim to make recommended colorectal cancer screening more widespread, the workshop discussed steps to be taken at the clinic, community, and health system levels. Workshop speakers, representing a broad spectrum of leaders in the field, identified major barriers to increased screening and described strategies to overcome these obstacles. This workshop summary highlights the information presented, as well as the subsequent discussion about actions needed to increase colorectal screening and, ultimately, to prevent more colorectal cancer deaths.

Book excerpt: Colorectal cancer remains a major health issue for many developed regions around the world. The good news is that early detection has significantly improved overall survival rates and continues to do so. A number of prevention strategies contribute to this positive trend, and today a patient who undergoes a colonoscopy for screening purposes stands a much better chance of being effectively surveyed for prevention of colorectal cancer. Patients can rely increasingly on the improved datasets and technical advances that are being made in screening approaches and skills. With continued progress, particularly in the partnership between clinicians and computer scientists, the future for colorectal cancer surveillance looks increasingly positive for the development of improved tools and methods.

Practical, authoritative, and highly-illustrated, this volume in the brand new Early Detection and Treatment of Cancer series covers current protocols and the latest advances in diagnostic imaging and molecular and serologic markers for colon cancer. Apply expert advice on the best “next-step plan for different presentations and tips for less invasive protocols. Get clinical, hands-on tools to help you make informed decisions on precisely what tests and imaging studies are needed for accurate diagnosis and staging. Clear figures, tables, and boxes illustrate step-by-step care of the full range of problems encountered. The small size and convenient format make this an ideal purchase for diagnostic reference. Outlines the steps after diagnosis to guide you through formulating a treatment or patient care plan. Emphasizes important points—such as risk-adjusted screening for staging and the use of promising new gene therapies—with “key points boxes at the beginning of each chapter and pedagogic features throughout. Summarizes the process of accurately diagnosing and staging cancer in a logical, almost algorithmic, approach for easy reference. Book excerpt: Causes and Control of Colorectal Cancer: A Model for Cancer Prevention is a ground- breaking monograph which takes a global, multidisciplinary approach to the causes, carcinogenesis and control of colorectal cancer. Over 1000 key studies were analyzed on colorectal cancer cause, carcinogenesis, primary prevention, early detection using modern screening techniques, and surveillance after tumor excision. All major research results are included up to the date of publication. The key chapters include those on morphology, molecular biology and evolution of tumors, etiology chapters on heredity, diet, alcohol, smoking, physical activity, life stress and others, while major chapters in the area of colorectal cancer control are on primary prevention, early detection by screening and surveillance, and post-tumor excision surveillance. Important recommendations on primary prevention, screening and surveillance of colorectal tumors are made in keeping with the most current research data. Causes and Control of Colorectal Cancer: A Model for Cancer Prevention takes colorectal cancer control into the 21st century, and suggests this to be the first cancer in which control will be largely achieved within the next generation, as a result of the unprecedented multidisciplinary research into causation, primary prevention, screening and surveillance over the past 30 years. The book also serves as a model for the control of other common malignancies, particularly cancers of the breast and prostate, the causes and prevention of which are less well understood.
