Cela a engendré une surconsommation et un mauvais usage de cette classe médicamenteuse à travers le monde provoquant l'émergence et la diffusion de résistances bactériennes. Les infections à bactéries résistantes ont été décrites comme un enjeu sanitaire mondial urgent par l'Organisation Mondiale de la Santé. Si rien n'est fait, certains spécialistes infectiologues s'accordent à dire que ces infections deviendront d'ici une trentaine d'année l'une des premières causes de décès dans le monde. En effet, il n'est plus rare de voir apparaître des bactéries multi-résistantes provoquant des situations d'impasse thérapeutiques entraînant une hausse de la mortalité dans les hôpitaux. La France s'est donc décidée au début des années 2000, comme différents pays, à mettre en place un ensemble de mesures sanitaires pour tenter de lutter contre cette menace. Cela a permis une réduction de 20% de la consommation française en vingt ans mais la France reste malgré tout le quatrième plus gros consommateur d'antibiotiques en Europe. L'arsenal thérapeutique s'amenuise et les antibiotiques classés comme critiques sont de plus en plus consommés. En 2015 a été lancée l'expérimentation de la dispensation à l'unité des antibiotiques en officine de ville. Entre autres résultats, l'expérimentation a permis de diminuer la consommation d'antibiotiques et réduit l'automédication qui reste un élément majeur du mésusage des antibiotiques. Cependant, des limites ont été révélées au cours de l'expérimentation, comme par exemple, pour les patients, une complexification de la prise du traitement, notamment pour des personnes fragiles et une réduction de la traçabilité des médicaments. Pour le pharmacien, le coût d'opportunité est double : il devra prendre plus de temps pour délivrer moins de médicaments. Si cette mesure venait à être généralisée en France, elle permettrait de lutter efficacement contre le phénomène des résistances bactériennes et contribuerait, à court terme, à faire significativement baisser la dépense en antibiotiques en volume et en valeur pour l'Assurance-Maladie et à moyen-long terme, à réduire considérablement l'empreinte environnementale des antibiotiques. Mais la réussite de la dispensation à l'unité des antibiotiques suppose une mutation conséquente de l'activité officinale, requérant une nouvelle organisation et de nouveaux moyens, qui pourrait accentuer des inégalités déjà présentes au sein du système officinal.
