L'objectif principal de cette étude qualitative était de faire l'état des lieux des moyens utilisés par les médecins généralistes picards pour soulager les douleurs résistantes aux thérapeutiques conventionnelles. Les objectifs secondaires étaient l'évaluation de leur ressenti quant à l'utilisation des thérapeutiques non-conventionnelles et l'échec des thérapeutiques qui avait menés à leur éventuel recours. Dans de rares situations, les patients ne voyaient pas leurs douleurs soulagées malgré l'utilisation d'antalgiques, le recours à des médecins spécialistes ou des techniques de rééducation. Si certains praticiens s'en tenaient aux thérapeutiques validées par les autorités sanitaires, d'autres avaient recours à des thérapeutiques dites non-conventionnelles afin de compléter le traitement mais ne percevaient pas d'efficacité supplémentaire à celle d'un effet placebo. Le recours aux thérapeutiques non-conventionnelles peut s'expliquer par le fait que la réponse à la douleur ne saurait être toujours médicamenteuse, c'est notamment le cas dans les états de souffrance. Des médecins pointaient le manque de formation sur la gestion de l'échec thérapeutique. Il est aussi influencé par des difficultés d'accès aux soins de par des facteurs socio-économiques ou par les représentations culturelles. Dans les rares situations où la douleur était insuffisamment soulagée, des médecins confiaient avoir recours à des thérapeutiques non-conventionnelles.

Book excerpt: La douleur est un thème qui depuis quelques années est au centre des préoccupations de nombreuses personnes (particuliers, média, organismes médicaux). Beaucoup d'études font état de la situation en milieu hospitalier. Nous nous sommes intéressés à la démarche des médecins généralistes dans ce domaine. La douleur est un phénomène difficile à définir. Elle repose sur plusieurs composantes (sensorielle, affective). Elle peut être aiguë ou chronique. Plusieurs classes de médicaments sont utilisés pour la soulager : les trois niveaux d'antalgiques établis par l'Organisation Mondiale pour la santé qui vont du paracétamol et de l'acide acétylsalicylique aux antalgiques opïodes majeurs, ainsi que des médicaments dits co-antalgiques. Le phénomène douloureux peut-être évalué grâce à des échelles de la douleur. Notre étude porte sur la description de la douleur par les patients, sur une évaluation de l'intensité douloureuse par le patient et par le médecin ainsi que sur l'attitude adoptée par les médecins généralistes en présence de cette douleur. Nous avons interrogé 126 patients parmi lesquels 76 présentaient un phénomène douloureux. Cette douleur était plutôt aiguë, présente de jour comme de nuit. Les localisations le plus souvent citées étaient la gorge, le rachis lombaire et l'abdomen. L'intensité maximale de la douleur était cotée le plus souvent comme forte (53 % par le patient et 44 % par le médecin) . Les principales affections responsables de la douleur étaient l'arthrose, les angines et les lumbagos. Le traitement prescrit reposait en majorité sur les antalgiques de niveau 1 de l'OMS. Les chiffres retrouvés dans la description de la douleur sont comparables à ceux de la littérature. La cotation de la douleur met en évidence une disparité entre le patient et le médecin (" sous cotation " par le médecin). Un diagnostic étiologique est donné au patient dans 69 cas sur 76. La description d'un arrêt de travail ne semble pas corrélé avec l'intensité de la douleur. Le traitement prescrit fait ressortir la non-prescription des antalgiques majeurs. Cette constatation pourrait être liée aux modalités de prescription des antalgiques majeurs au moment de l'étude (carnet à souche, " douleur " administrative) ainsi qu'à l'absence d'étude dans notre enquête des visites du médecin au domicile du patient, écartant probablement les douleurs les plus intenses. Nous pouvons aussi noter que la réponse du médecin ne paraît pas toujours adaptée à l'intensité de la douleur exprimée par le patient.

The distribution and seriousness of the disease depend on the pathogens and vectors involved, as welt as environmental conditions. Of the four parasites (Plasmodium), only P. falciparum kilts although the other three cause debilitating disease with regular relapses and recrudescence. More than fifty species of Anopheles can fulfil the vector rote which is essential in transmission of the parasite between human beings. Climatic factors (temperature and rainfall), the environment and biogeographical particularities dictate the distribution of anopheline species and determine transmission rates. This is why it makes sense to tank about the biodiversity of malaria. Today, more than 90% of deaths from malaria occur in Tropical Africa which is home to only 10% of humanity. Every 30 seconds a child dies from malaria. This continent harbours the most effective vectors (An. gambiae and An. funestus, in particular) and the climate is highly conducive to transmission of the disease. Severe malaria is also seen in forest foci in Southeast Asia, Papua-New Guinea and the Amazon. In the rest of the tropical and subtropical world, P. vivax and/or P malariae cause less severe disease.
