Cependant il y a peu d’études récentes d’ampleur concernant ce symptôme dans ce contexte. L’objectif est de déterminer la prévalence de la douleur aiguë de l’adulte en médecine pré-hospitalière et d’identifier les facteurs associés à l’oligoanalgésie. Il s’agit d’une enquête prospective descriptive menée en 2007 sur une période de 11 mois au sein d’un service mobile d’urgence et de réanimation (SMUR) du service d’aide médical d’urgence (SAMU) de Seine Saint Denis. Les patients âgés de 16 ans ou plus et capables d’auto évaluer leur douleur ont été inclus dans l’étude. Pour chaque patient, le médecin transporteur devait remplir une fiche comprenant le diagnostic retenu, la présence ou non d’une douleur, l’intensité de la douleur et le traitement effectué.Sur les 2279 patients inclus, 947 (42%) ont été évalués comme douloureux. La douleur était intense à sévère pour 64% des patients. La traumatologie (82%) et la gynécologie-obstétrique (66%) étaient les catégories diagnostiques associées à un risque plus élevé de douleur. Ces mêmes pathologies ainsi que la cardiologie étaient associées à un risque plus élevé de douleur intense. Parmi les 1364 patients transportés par une unité mobile hospitalière (UMH), 48% étaient douloureux (71% présentaient une douleur sévère à intense). Un analgésique a été administré dans 73% des cas. Malgré tout 51% ont été soulagé et le taux de soulagement a été inférieur pour la traumatologie et la gynécologie obstétrique.Cette étude a mis en évidence une prévalence de la douleur pré- hospitalière de 42%, cependant ce taux varie beaucoup en fonction de l’origine de la douleur. La prise en charge de la douleur reste insuffisante et le taux de soulagement reste encore trop faible.

Si l'antalgie doit être optimale, de nombreux freins persistent et l'oligoanalgésie perdure chez un tiers des patients à la sortie des urgences. L'objectif principal de notre étude est donc de décrire l'efficacité et la qualité de la prise en charge de la douleur aigüe après la mise en place d'un protocole de prescriptions anticipées chez les patients de plus de 18 ans, afin de comparer les résultats obtenus à ceux de l'étude réalisée en 2018. Matériel et méthode: il s'agit d'une étude observationnelle rétrospective monocentrique avant après, réalisée au Centre Hospitalier de Périgueux (CHP). Les critères d'inclusion regroupaient les patients adultes communicants admis avec une douleur d'intensité modérée à sévère.Les critères d'inclusion regroupaient les patients adultes communicants admis avec une douleur d'intensité modérée à sévère. Le critère de jugement principal était l'efficacité de l'antalgie effectué aux urgences représentée par l'évaluation de la douleur à la sortie du service. L'analgésie était considérée comme suffisante ou de qualité lorsque l'échelle numérique (EN) était inférieure ou égale à 3/10 à la sortie des urgences. Résultats: 1048 patients ont été inclus du 19 mars 2018 au 16 juin 2019. On note une très légère augmentation du taux de soulagement des patients (62,6% IC95% [58,4- 66,8] pour la période I et 64,9% IC95% [60,6-69,2] pour la période II) mais cette différence n'est pas significative. Conclusion: cette étude a permis de constater un changement dans les pratiques, sans que cela soit traduit par une amélioration significative de l'efficacité globale de l'antalgie dans le service. Une nouvelle étude serait nécessaire à distance pour évaluer avec plus de recul l'impact potentiel de ce protocole.

Nous voulons comparer 2 protocoles d’analgésie. Nous avons donc réalisé une 1ere étude dont l’objectif était de faire un état des lieux de la prise en charge de la douleur aiguë au sein du SMUR d’Amiens. Les patients majeurs sur la période du 11/01 au 11/02/10 ont été inclus dans cette étude observationnelle rétrospective. Une revue de dossiers des patients a été réalisée. Les principaux paramètres observés étaient la prévalence de la douleur aiguë, l’utilisation de l’END pour son évaluation, l’existence d’un traitement antalgique adapté et celle d’un soulagement (END ≤ 3) en fin de prise en charge. 363 patients ont été inclus. L’END était notée pour 46% d’entre eux et pour 28% en fin de prise en charge. La prévalence de la douleur aiguë était de 54% et la douleur intense à sévère était de 35%. 71% des patients ayant une pathologie traumatique exprimaient une douleur END ≥ 6 contre 16% ayant une pathologie médicale. 67% présentant une douleur sévère et 26% des patients présentant une douleur intense ont reçu un antalgique de palier 3. 90% des patients présentant une douleur sévère ont reçu un traitement mais seuls 19% étaient soulagés lors de la réévaluation de la douleur et 61% l’étaient en fin de prise en charge. La mise en place d’une nouvelle stratégie antalgique semble nécessaire. Notre travail vient compléter les rares travaux ayant étudié la prévalence et la prise en charge de la douleur en préhospitalier. Nous avons rédigé un protocole de recherche visant à montrer les bénéfices de l’utilisation de la kétamine en coanalgésie avec le sufentanyl pour soulager les patients.

Nous avons pu montrer que la douleur thoracique due à un infarctus du myocarde avec sus-décalage du segment ST avait 3,7 fois plus de risque d’être intense à sévère comparativement à des douleurs non dues à un infarctus. Dans l’infarctus du myocarde sans sus-décalage du ST par contre, l’intensité de la douleur thoracique n’était pas significativement plus importante. Nous avons pu montrer que la morphine n’était pas moins efficace que le fentanyl lors de douleurs aiguës sévères.
