Book excerpt: The American health care industry has undergone such dizzying transformations since the 1960s that many patients have lost confidence in a system they find too impersonal and ineffectual. Is their distrust justified and can confidence be restored? David Dranove, a leading health care economist, tackles these and other key questions in the first major economic and historical investigation of the field. Focusing on the doctor-patient relationship, he begins with the era of the independently practicing physician--epitomized by Marcus Welby, the beloved father figure/doctor in the 1960s television show of the same name--who disappeared with the growth of managed care. Dranove guides consumers in understanding the rapid developments of the health care industry and offers timely policy recommendations for reforming managed care as well as advice for patients making health care decisions. The book covers everything from start-up troubles with the first managed care organizations to attempts at government regulation to the mergers and quality control issues facing MCOs today. It also reflects on how difficult it is for patients to shop for medical care. Up until the 1970s, patients looked to autonomous physicians for recommendations on procedures and hospitals--a process that relied more on the patient's trust of the physician than on facts, and resulted in skyrocketing medical costs. Newly emerging MCOs have tried to solve the shopping problem by tracking the performance of care providers while obtaining discounts for their clients. Many observers accuse MCOs of caring more about cost than quality, and argue for government regulation. Dranove, however, believes that market forces can eventually achieve quality care and cost control. But first, MCOs must improve their ways of measuring provider performance, medical records must be made more complete and accessible (a task that need not compromise patient confidentiality), and patients must be willing to seek and act on information about the best care available. Dranove argues that patients can regain confidence in the medical system, and even come to trust MCOs, but they will need to rely on both their individual doctors and their own consumer awareness.

Book excerpt: Care work, both paid and unpaid, contributes to well-being, social development and economic growth. But the costs of providing care are unequally borne across gender and social class. Feminist scholarship on the gendered construction of welfare provisioning and welfare regimes has produced a conceptually strong and empirically grounded analysis of care, reinforcing the necessity of rethinking the distinctions between "the public" and "the private" as well as the links between them. Yet this analysis, premised on post-industrial contexts, does not travel easily to other parts of the world. Many of its core assumptions – about family structures, labor markets, state capacities, and public social provisioning – do not hold for a wider range of countries. Drawing on original research on the care economy in three developing regions (Africa, Asia, Latin America), this volume addresses a major empirical lacuna while facilitating a conversation across the North-South divide.

Book excerpt: Originally published in 1984, this book attempted to fill a gap by providing a broad-ranging structural analysis of the health care sector and the political and economic forces which influence its shape and contents, both in the western world and developing countries. The contributors examine the relationships of capitalism to health care, in terms of its influence on the physical environment, the incidence of social diseases and the prevailing (20th Century) view of what constitutes health itself; and in terms of the consequences of the new medical industrial complex it has created, such as the declining provision of health care for the poor and disadvantaged and the growing power of the pharmaceutical industry.

Society wants care to be allocated equitably; it wants the financial burden to be kept within bounds; it wants treatments to be both medically effective and economically efficient. These shared concerns lead to a demand for State intervention which this book seeks impartially to appraise and evaluate.

Book excerpt: This volume closely analyses women’s role and experiences in migration (internal and international) and its interlinkages with the care economy in their functions as nurses and paid domestic workers as well as unpaid carers. Bringing together case studies from across India and other parts of the world, the essays in the volume capture the characteristics and specificities of female migration in different settings — be it for economic or associational reasons, or as left behind members. The book also looks at gender-specific discriminations and vulnerabilities along with the empowering aspects of migration. This volume will be of great interest to scholars and researchers of migration, gender studies, sociology, and social anthropology, as well as development studies, demography, and economics.
