A 1992 study found 2 million children in the United States routinely used the emergency department as their primary source of care (Halfon, Newacheck, Wood & Peter, 1996). Studies have shown 50-90% of all patients presenting to emergency departments have minor complaints which could easily be treated in primary care settings (Dowling & Dudley, 1995; Grumbach et al., 1993). The non-urgent use of emergency departments has led to overcrowding, long waiting times and patient dissatisfaction (Grumbach et al., 1993; Derlet & Nisho, 1990, Liggins, 1993). Overcrowding and inappropriate use of the emergency department also interferes with the care of critically ill patients (Dowling & Dudley, 1995). Civilian hospitals have developed various strategies to address patients with minor complaints varying from outright refusal of care, to triaging to off-site clinics, developing non-urgent care clinics and establishing Fast Track Clinics also known as Minor Emergency Areas (MEAs) within existing emergency departments. These Fast Track clinics have been staffed with a variety of non-physician, mid-level health care providers including nurse practitioners.

Book excerpt: In the mid 1960's the role of the nurse practitioner (NP) was created and developed as a result of a shortage and maldistribution of physicians. The need for emergency nurse practitioners was recognized as a response to increased patient volume in the emergency department (ED) (Hayden, Davies, & Clore, 1992). Lack of understanding for the scope of practice and issues of implementation restricted the proliferation of NPs. Acute care NPs offer promise of increasing the efficiency of care while maintaining quality. Their use is important to the future of health care and the needs of patients in a system where care is often fragmented and costly. The purpose of this study was to analyze the responses of fifty emergency physicians on their perceptions of NP roles and responsibilities, willingness to hire, and incentives to become associated with a nurse practitioner. Roger's Innovation-Diffusion Theory was the theoretical framework used to analyze the data. A convenient sample was obtained from eight participating emergency departments. A researcher developed questionnaire was implemented with a return rate of 44% (50 out of 112) responding to questions of physicians working with NPs in the acute care setting. Demographic data revealed that only 26% (n-13) have had prior experience with an NP, the majority of all respondents were in favor of utilizing the skills of nurse practitioners in the ED. These results may suggest an overall acceptance of NPs related to increased exposure and use of NPs in the area.

While there has been some literature published regarding patient satisfaction with nurse practitioners (NPs), especially in the outpatient setting, very little literature could be located on patient satisfaction with Emergency Nurse Practitioners (ENPs). This study attempted to answer the following research question: What is the difference in Emergency Department (ED) patient satisfaction when cared for by an ENP vs. Medical Doctors (MDs)? A descriptive study was used to identiiy the differences in ED patient satisfaction. It was conducted at a rural emergency department with a population of patients residing in Fayette and surrounding counties of Texas who presented to the ED. A convenience sample of patients was obtained. The inclusion criteria were patients who were at least 18 years of age, who were discharged to home from the ED, who could read and write English, and who could complete the survey without help from another person. Collection of data was accomplished through a 12 item patient satisfaction survey (PSS) developed by the investigator. The PSS asked patients to rate satisfaction by indicating the degree to which their expectations were met. Descriptive statistics were used to describe the demographic characteristics of the sample. Due to unequal sample sizes (ENP n = 26 , MD n = 12), Welch approximate t-test for independent samples was used to compare the ENP and MDs on their grand mean satisfaction score. There were no statistically significant differences between the ENP and MDs on the basis of the grand mean satisfaction score. A Mann-Whitney U test was used to compare the two groups on the basis of the patients' responses to individual satisfaction questions.
