Book excerpt: Diffuse optical tomography (DOT) and fluorescence molecule tomography (FMT) are emerging as important optical imaging tools for clinical and biomedical research. Two major outstanding challenge in DOT and FMT are i) the relatively low imaging resolution, which results from the high degree of photon scatter through biological tissue, and ii) the inability to image multiple fluorescent targets in deep tissue ("multiplexing") due to the relatively narrow wavelength region within which light is less attenuated and effectively transmitted through bulk tissue. In this dissertation, we explored the use of time-domain DOT and FMT imaging to address these problems. First, time-resolved (TR) measurement of early arriving photons from a pulse laser was studied as a method for improving resolution in diffuse optical imaging. The underlying concept is photons that arrive first ("early") at a detector from a pulsed laser incident on bulk tissue have experienced fewer scattering events and taken relatively straight paths. We developed and tested a time-domain diffuse optical imaging system consisting of a femetosecond titanium: sapphire laser and fast, single photon avalanche photodiode (SPAD) detectors, that had an extremely fast temporal response time of 59 ps. We validated that the SPAD allowed measurement of photon-density sensitivity function that were 65% narrower than the temporally un-gated case at very early times. This exceeded the performance achieved previously with photomultiplier-based systems and approached the theoretical maximum predicted by time-resolved Monte Carlo simulations. Following this work, we then developed a complete SPAD-based DOT scanner, in which an array of SPADs were used. We validated performance of the system in cylindrical and irregular shaped optical phantoms of approximately small animal size. We were able to accurately reconstruct the size and position of up to 4 absorbing inclusions in diffusive media for the first time, with increasing image quality at earlier times. Next, we developed a time-resolved multiplexed fluorescence tomography system, which allowed us to measure extremely rich multi-spectral and temporal fluorescence data sets from diffusing media. In combination with a recently developed two-stage de-mixing and image reconstruction algorithm we are able to simultaneously image as many as four (and in theory 5) red and near-infrared fluorophores with closely overlapping spectral and temporal profiles, doubling the information throughput of FMT systems. This instrument also allowed us to account for the wavelength-dependency of optical properties in bulk biological tissue, which is a major challenge in biological imaging. The unique multiplexing imaging capabilities of this technology will allow us to study development of gliomas in small animal models in ongoing work in our group.

Book excerpt: Fluorescence tomography (FT) is an emerging non-invasive in vivo molecular imaging modality that aims at quantification and three-dimensional (3D) localization of fluorescent tagged inclusions, such as cancer lesions and drug molecules, buried deep in human and animal subjects. Depth-resolved 3D reconstruction of fluorescent inclusions distributed over the volume of optically turbid biological tissue using the diffuse fluorescent photons detected on the skin poses a highly ill-conditioned problem, as depth information must be extracted from boundary data. Due to this ill-posed nature of FT reconstructions, noise and errors in the data can severely impair the accuracy of the 3D reconstructions. Consequently, improvements in the signal-to-noise ratio (SNR) of the data significantly enhance the quality of the FT reconstructions. Furthermore, enhancing the SNR of the FT data can greatly contribute to the speed of FT scans. The pivotal factor in the SNR of the FT data is the power of the radiation illuminating the subject and exciting the administered fluorescent agents. In existing single-point illumination FT systems, the illumination power level is limited by the skin maximum radiation exposure levels. In this research, a multiplexed architecture governed by the Hadamard transform was conceptualized, developed, and experimentally implemented for orders-of-magnitude enhancement of the SNR and the robustness of FT reconstructions. The multiplexed FT system allows for Hadamard-coded multi-point illumination of the subject while maintaining the maximal information content of the FT data. The significant improvements offered by the multiplexed FT system were validated by numerical and experimental studies carried out using a custom-built multiplexed FT system developed exclusively in this work. The studies indicate that Hadamard multiplexing offers significantly enhanced robustness in reconstructing deep fluorescent inclusions from low-SNR FT data.

Book excerpt: This thesis concerns the image processing of fluorescence diffuse optical tomography (fDOT), following two axes: FDOT image co-registration with PET (positron emission tomography) image and improvement of fDOT image reconstructions using mirrors to collect additional projections. It is presented in two parts:In the first part, an automatic method to co-register the fDOT images with PET images has been developed to correlate all the information from each modality. This co-registration method is based on automatic detection of fiducial markers (FM) present in both modalities. The particularity of this method is the use of optical surface image obtained in fDOT imaging system, which serves to identify the Z position of FM in optical images. We tested this method on a model of mice bearing tumor xenografts of MEN2A cancer cells that mimic a human medullary thyroid carcinoma, after a double injection of radiotracer [18F] 2-fluoro-2-Deoxy-D-glucose ( FDG) for PET imaging and optical fluorescent infrared tracer Sentidye. With the accuracy of our method, we can demonstrate that the signal of Sentidye is present both in the tumor and surrounding vessels.The fDOT reconstruction image quality is degraded along the Z axis due to a limited number of projections for reconstruction. In the second part, the work is oriented towards a new method of fDOT image reconstruction with a new multi-angle data acquisition system in placing two mirrors on each side of the animal. This work was conducted in collaboration with the CS Department of University College London (UCL), a partner of the European project FMT-XCT. TOAST software developed by this team was used as source code for the reconstruction algorithm, and was modified to adapt to the concerned problem. After several tests on the adjustment of program parameters, we applied this method on a phantom that simulating the biological tissue and on mice. The results showed an improvement in the reconstructed image of a semi-cylindrical phantom and the image of mouse kidney, for which the reconstruction of the mirrors geometry is better than that of conventional geometry without mirror. Nevertheless, we observed that the results were very sensitive to certain parameters, where the performance of reconstruction varies from one case to another. Future prospectives concern the optimization of parameters in order to generalize the multi-angle approach.

This new reconstruction algorithm is called Geometric Sensitivity Difference (GSD) method which effectively reduces the variation of reconstruction sensitivity with respect to imaging depth. Here the GSD method for source sharing detector pairs is demonstrated using simulated continuous-wave measurements in an outer circular-array imaging geometry, of which the native sensitivity varies strongly with respect to the depth. The results of the GSD method are compared to that of two methods: one is the conventional baseline method which utilizes the native sensitivity and the second method is depth compensation algorithm (DCA) which employs active depth adaptive scheme which counteracts the dependence of reconstruction sensitivity with respect to imaging depth.

Indeed, in vivo fluorescence optical imaging extends across a wide range of applications, from cellular to organ levels. Unfortunately, in organ level applications, fluorescence imaging suffers from low spatial resolution due to the high scattering nature of biological tissue, especially in deep tissue (>1cm). Extensive effort has been spent to improve the spatial resolution of fluorescence tomography (FT). However, approaches such as integrating FT with other anatomic imaging modalities such as Magnetic Resonance Imaging (MRI) or computed tomography (CT) do not perform well if the fluorescent agent distribution within the medium cannot be defined in the anatomical image. There are also several techniques that attempt to modulate fluorescence signals using ultrasound to achieve higher spatial resolution. However, low modulation efficiency and extremely low signal to noise ratio (SNR) are the two primary factors that make the implementation of these techniques difficult.Consequently, the poor spatial resolution and subsequent low quantitative accuracy are the main obstacles preventing the widespread use of this powerful technique in pre-clinical and clinical settings. To overcome these limitations, the goal of this thesis is to develop an entirely new approach termed, "Temperature-modulated fluorescence tomography (TM-FT)", that can provide high resolution images at depths up to 6 cm without sacrificing the exceptional sensitivity of FT. In this innovative approach, FT is combined with temperature activatable fluorescence molecular probes (ThermoDots) and high intensity focused ultrasound (HIFU) which can provide localized heating of the tissue (only 3-5 °C) with high spatial resolution. The small size of the focal spot (~1.4 mm) allows imaging the distribution of these temperature sensitive agents with not only high spatial resolution but also high quantitative accuracy.This thesis will present the development of the first TM-FT system prototype including the instrumentation and system design, image reconstruction algorithm, and ThermoDots probe development. The feasibility of this method to provide superior spatial resolution and high quantitative accuracy is validated using simulations and experimentally demonstrated in phantom and ex vivo tissue.

NIRS was originally designed for the clinical monitoring of tissue oxygenation, and today it has also become a useful tool for neuroimaging studies (functional near-infrared spectroscopy, fNIRS). However, difficulties in the selective and quantitative measurements of tissue hemoglobin (Hb), which have been central in the NIRS field for over 40 years, remain to be solved. To overcome these problems, time-domain (TD) and frequency-domain (FD) measurements have been tried. Presently, a wide range of NIRS instruments are available, including commonly available commercial instruments for continuous wave (CW) measurements, based on the modified Beer–Lambert law (steady-state domain measurements). Among these measurements, the TD measurement is the most promising approach, although compared with CW and FD measurements, TD measurements are less common, due to the need for large and expensive instruments with poor temporal resolution and limited dynamic range. However, thanks to technological developments, TD measurements are increasingly being used in research, and also in various clinical settings. This Special Issue highlights issues at the cutting edge of TD DOS and diffuse optical tomography (DOT).
