Book excerpt: The objective of the study was to assess the long-term effect of the Expanded Food and Nutrition Education Program (EFNEP) on participants' diet quality using 24-hr dietary recall (24HDR) data collected from a sample of English and Spanish speaking EFNEP participants from January 2018 through October 2019. Healthy Eating Index (HEI) scores collected at 4 time points – pre-EFNEP (Week 1 of a 9-week nutrition education class series), post-EFNEP (Week 9), 6-months post-EFNEP, and 12-months post-EFNEP – were analyzed using Generalized Estimating Equations (GEE) linear regression and paired t-tests. This study was limited due to its small sample size, little variation in participant demographics, and missing data. Total HEI – estimated due to the missing data – improved substantially by 4.8 points from pre-EFNEP to the 12-month follow-up. HEI subcomponent scores improved in total fruit, whole fruit, and fatty acids whereas scores decreased in total vegetables, green beans, and sodium. Further research is essential to demonstrate the long-term impacts of EFNEP on participants’ dietary behaviors and thus, the efficacy and value of evidence-based nutrition education programs targeting low-income families.

Book excerpt: The purpose of this study was to examine the behavior changes, barriers to preparing healthy meals, quality of life and internal locus of control with two nutrition education programs EFNEP and OFL. Data for this study were collected by surveying participants in the EFNEP and OFL Program before and after participation in the programs. The research was guided by the following questions. (1) Do participants change behavior after participating in EFNEP or OFL?; (2) Are participants able to overcome barriers that prevent them from preparing healthy meals after participating in EFNEP or OFL?; (3) Do participants demonstrate an increase in their quality of life after participating in EFNEP or OFL?; (4) Do participants experience changes in their perceived control of their environment after participating in EFNEP or OFL?, and (5). What effect, if any, does the method of program delivery have on change in behaviors, overcoming barriers to healthy meals, a change in locus of control and quality of life after participating in EFNEP or OFL? Among the 268 respondents, there were 141 respondents from the OFL Program and 127 from EFNEP. The majority of the participants were female, African-American, between the ages of 21-30 and had a high school education or less. Data analysis revealed participation in EFNEP and OFL nutrition education programs influences behavior change with low-income clients and perceived quality of life improved for respondents after participating in the two programs. Participation in EFNEP and OFL did not impact clients in overcoming barriers in preparing healthy meals, and participation in the programs did not seem to have an influence on the perceived control of one's life. Changes in behavior were not dependent on the method of delivery for the two programs EFNEP and OFL.

Book excerpt: Food insecure individuals do not always have access to adequate food for a healthy lifestyle and are at high risk of detrimental health outcomes. Researchers hypothesize that food insecurity leads to changes in dietary practices, including greater overall food purchase in times of adequate resources and purchase of low-cost, unhealthful foods when resources are constrained. Most measures of food insecurity do not measure changes in dietary practices and dietary quality. Research findings suggest education that provides alternative strategies to manage resources and improve dietary practices can improve food insecurity. We assessed the relationship between 1) food security and ability to afford foods and 2) the impact of Expanded Food and Nutrition Education Program (EFNEP) in a low-income, multi-cultural population in Massachusetts. We used a pre/post-education survey design, including the USDA six-item Food Security Module (FSM), food-affordability questions and EFNEP behavior checklist. EFNEP participants experienced high rates of food insecurity with over 40% of participants classified as food insecure (N=80). Pre-EFNEP, individuals in households with low food security were less likely to report being able to afford healthy foods (51.5%) and fruits and vegetables (57.6%) throughout the month compared to those in households with high (80.9%) and marginal (78.7%) food security (P=0.007, P=0.051). Individuals in households with marginal, low and very low food security were less likely to report being able to afford the same kinds of food throughout the month compared to individuals in high food secure households (56.4% vs. 84%) (P=0.022). Individuals in food insecure households reported running out of food before the end of the month more often than their food secure peers (P=0.013). Post-EFNEP, a greater proportion of participants fell into the high and marginal categories of food security (60.0% to 71.7%, P=0.065). Our results indicate that food insecure households have a harder time affording healthy foods throughout the month, leading to poor diet quality that possibly contributes to poor health outcomes. The food affordability questions may capture changes in dietary practices in food insecure populations throughout the month. EFNEP can provide skills and knowledge to at risk populations to improve abilities and combat food insecurity.

Book excerpt: This project assesses the effectiveness of the Expanded Food and Nutrition Education Program (EFNEP) within the context of supermarket access. EFNEP is a national community nutrition education program that strives to give participants the tools to live healthier lives. Analysis was performed on participants from 16 Arkansas counties that completed EFNEP during 2013 or 2014. The program outcome is measured in terms of the change in Healthy Eating Index (HEI) as calculated from 24-hour diet recalls at program entry and exit. Supermarket locations were obtained from the USDA Food Nutrition Service's SNAP Retail Locator and represent the food environment near the midpoint of our two-year study period. Each participant's census block of residence was characterized as being supermarket accessible or non-accessible based on the availability of supermarkets within one mile (ten miles) of the center of urban (rural) census blocks. Linear regressions are used to model changes in HEI scores as a function of program graduation, defined as completing eight or more EFNEP lessons. Our models are estimated with educator fixed effects and include controls for nutrition assistance, age, gender, educational attainment, race, and ethnicity. The key finding is that the effect of graduation on HEI was higher for participants with access to supermarkets. This finding holds across urban and minority subsamples and is robust to measurement of program exposure as graduation or in terms of lessons completed. The implication is that limited access to affordable and healthy foods is a crucial barrier that may impact goals of EFNEP and other educational interventions. Moreover, understanding the role of the food environment enables educators to tailor curriculum to the constraints facing lower-income audiences.
