Work at this institute and the Steno Memorial Hospital has been dedicated to research in diabetes for decades, and the insti tutions were appointed WHO Collaborating Centres for Re search and Training on the Pathogenesis of Diabetes Mellitus in 1983. It is worth noting that while addressing the hypothesis of the role of class II major histocompatibility glycoproteins in autoimmune diabetes (insulin-dependent diabetes, IDDM) a number of investigators established animal models in which class II molecules were expressed under the control of the rat insulin promoter. While generating interesting information on 100M, the finding of immunologic tolerance in such transgenic mice has attracted the attention of several basic immunologic laboratories for quite different reasons. It is now evident from studies in several countries that enterovirus infection accompanies or precedes the onset of diabetes in many individuals. These evidences include the isolation of coxsackievirus B4 from the pancreas of a child at diagnosis of type 1 diabetes, several epidemiological and prospective studies showing that some cases of type 1 diabetes are strongly associated with enterovirus infections, seasonal incidence studies, case-control studies that show an increased prevalence and levels of IgM antibodies to enterovirus in newly diagnosed patients. Other evidences involves several case studies in which diabetes develops after an enterovirus infections, increased T cell response to enterovirus antigens in type 1 diabetic patients, suggestions that enterovirus infections during pregnancy might initiate the events leading to childhood diabetes, and a higher frequency of enterovirus RNA in the serum/blood of type 1 diabetic patients at diagnosis. Book excerpt: The effect of Coxsackie B virus infections on pancreatic B cell function has been studied in laboratory mice. It has proved possible to induce a diabetes-like syndrome in a proportion of male, CD^ mice following infection with a Coxsackie B^ virus.Infection of Porton, DBA or CD^ mice with pancreas and heart adapted strains of Coxsackie virus resulted in moderate to severe acinar cell damage but islet tissue appeared generally unaffected histologically. Relative hypoglycaemia was noted in many of the infected mice and this was associated with somewhat raised serum insulin concentrations. Insulin release from pancreas slices in these animals showed an increased basal release but decreased release of insulin in response to glucose stimulation.Adaptation of Coxsackie' B^ virus to pancreatic tissue resulted in increased acinar cell damage but no tendency to hyperglycaemia was observed. The relative hypoglycaemia probably results from a combination of decreased food utilisation plus an effect on the islets due to the degenerating acinar cells causing increased insulin release.Coxsackie B. infection in CD, mice resulted in U 1 about 30% of animals becoming diabetic 15-20 days after infection. The diabetes was characterised by moderate hyperglycaemia with relative hypoinsulinism. Although serum insulin levels in the hyperglycaemic animals were slightly higher than controls the levels were inappropriately low for-Tithe height of the blood sugar. Isolated islet preparations indicated that basal insulin release was normal but response to 20 mM glucose was impaired.Histological evidence showed that the acinar tissue was only moderately damaged but several islets were surrounded by mononuclear cells and, in some cases, these cells had infiltrated the islet tissue. However, many islets appeared normal on light microscopy.In view of the tine of onset of hyperglycaemia and the apparent lack of massive islet cell damage the possibility arises that the diabetogenic effect of the virus is mediated by some immunological mechanism. The mechanism of insulin secretion may involve receptors on the cell membrane of the B cell and it is possible that an inappropriate immunological response to the Coxsackie virus may lead to damage of the B cell membrane.
