Book excerpt: Une enquête a été menée auprès des médecins généralistes de la commune de Rueil-Malmaison (département des Hauts de Seine). Trente deux médecins ont répondu à un questionnaire sur leurs pratiques concernant l’hygiène des mains, la prevention des accidents d’exposition au sang, la gestion du matériel médical et des déchets de soins à risque infectieux. L’équipement de leurs locaux et notamment du point d’eau servant au lavage des mains a été observé au cours de cette enquête. Un guide de bonnes pratiques (sous forme d’une fiche) reprenant les grandes notions de base de l’hygiène, réalisé à partir des recommandations ministérielles, leur a été remis dans le but de les sensibiliser à ce sujet d’actualité. Concernant les habitudes d’hygiène, seulement 41 % des médecins affirment se laver les mains avant et après l’examen de chaque patient ; 30% déclarent respecter la procédure recommandée pour le nettoyage du matériel médical, et 3% utilisent un produit détergent-décontaminant adéquat. L’équipement des locaux ne répond pas aux recommandations puisque seulement 59% des médecins visités ont un lavabo dans leur salle de consultation. Il ressort de ce travail la nécessité de former les médecins, d’évaluer le risque infectieux au cabinet du médecin généraliste afin d’adapter les recommandations issues du milieu hospitalier à la pratique ambulatoire.

A discussion of the reasons why opioids continue to be underprescribed or difficult to obtain is followed by an explanation of the Single Convention on Narcotic Drugs.
