Book excerpt: Background & Objectives: Financial burden on households due to healthcare is high in India but only small segments of the population are covered with health insurance. This study has two objectives: 1. to investigate the factors affecting enrollment in voluntary health insurance, and 2. to examine the characteristics of enrollment in terms of income-related inequality and adverse selection problem. Data and Methods: This is a cross sectional study. Mixed method approaches - qualitative and quantitative methods were used. In-depth interviews were conducted among officials of insurance companies, insurance consultants (insurance agents), insured and uninsured people. Subsequently, household surveys among insured and uninsured were carried to examine the significance of the determinants on enrollment. Results: In addition to income, education and health risk, the level of awareness of people about insurance and the significant role of insurance consultants matter in the enrollment in voluntary health insurance. The enrollment in health insurance is characterised by income-related inequality in enrollment, and ii) no adverse selection. Conclusion: We identified several demand and supply side factors negatively and positively affecting the enrollment. This study suggest that, in addition to developing a competitive insurance market with the availability of attractive health insurance products and ensuring reimbursement about eligible claims, appropriate policy measures such as promoting people's awareness on insurance and introducing better incentive structures for insurance consultants are needed to increase enrollment in health insurance.

Logit regression model was used to identify the determinants affecting the enrollment of Voluntary Health Insurance Scheme. Descriptive statistics were applied to analyze the utilization of health care service between health insured group and health un-insured group. Data (n=2268) collected randomly from 11 provinces were used to identify the determinants affecting the enrollment of Voluntary Health Insurance Scheme in this region. The results showed that the determinants affecting the enrollment of Voluntary Health Insurance Scheme in Red River Delta Region in Vietnam were age, marital status, educational level, family size, location, consultation times and income. There was not much difference in number of visits to health care facilities between insured and un-insured groups.

Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

The study found that there is a significant role of 'Insurance Agents' and 'Self Help Groups' in the enrolment. In addition, role of insurance awareness of people, non-availability of comprehensive insurance products, lack of trust over insurers over reimbursement play important role in limiting the scale-up of health insurance in the country. This study proposes a community-public-private partnership for achieving universal healthcare coverage for India. This book targets an audience with multi-disciplinary backgrounds, and will be useful not only for students of health economics and insurance but also for researchers and development policy practitioners with interest in healthcare and development policies.

Book excerpt: Ghana National Health Insurance Scheme (NHIS) was established in 2003 as a major vehicle to achieve the country’s commitment of Universal Health Coverage. The government has earmarked value-added tax to finance NHIS in addition to deduction from Social Security Trust (SSNIT) and premium payment. However, the scheme has been running under deficit since 2009 due to expansion of coverage, increase in service use, and surge in expenditure. Consequently, Ghana National Health Insurance Authority (NHIA) had to reduce investment fund, borrow loans and delay claims reimbursement to providers in order to fill the gap. This study aimed to provide policy recommendations on how to improve efficiency and financial sustainability of NHIS based on health sector expenditure and NHIS claims expenditure review. The analysis started with an overall health sector expenditure review, zoomed into NHIS claims expenditure in Volta region as a miniature for the scheme, and followed by identifictation of factors affecting level and efficiency of expenditure. This study is the first attempt to undertake systematic in-depth analysis of NHIS claims expenditure. Based on the study findings, it is recommended that NHIS establish a stronger expenditure control system in place for long-term sustainability. The majority of NHIS claims expenditure is for outpatient consultations, district hospitals and above, certain member groups (e.g., informal group, members with more than five visits in a year). These distribution patterns are closely related to NHIS design features that encourages expenditure surge. For example, year-round open registration boosted adverse selection during enrollment, essentially fee-for-service provider mechanisms incentivized oversupply but not better quality and cost-effectiveness, and zero patient cost-sharing by patients reduced prudence in seeking care and caused overuse. Moreover, NHIA is not equipped to control expenditure or monitor effect of cost-containment policies. The claims processing system is mostly manual and does not collect information on service delivery and results. No mechanisms exist to monitor and correct providers’ abonormal behaviors, as well as engage NHIS members for and engaging members for information verification, case management and prevention.

The prevailing view seems to indicate that UHC require not just more money, but also a focus on changing the rules of the game for spending health system resources. The book does not attempt to identify best practices, but rather aims to help policy makers understand the options they face, and help develop a new operational research agenda. The main chapters are focused on providing a granular understanding of policy design, while the appendixes offer a systematic review of the literature attempting to evaluate UHC program impact on access to services, on financial protection, and on health outcomes.
