Utilization of health services for children plays an important role towards achieving the Millennium Development Goal in the reduction of child mortality. In Nepal, several studies reveal disparities in child health outcomes across socio-economic levels and regions of the country. Health inequalities found in the relationship between socio-demographic determinants and the utilization of child health services has not been under rigorous examination. Objectives: The primary goal of the study is to examine empirical evidence of the socio-demographic determinants of child health services utilization, such as geographical access, household income, and mother's age and education level. Methodology: Using 1996 and 2001 Nepal Demographic and Health Survey (NDHS) data, both cross-sectional and fixed-effects logit models are used to analyze the effect of the determinants health services utilization for children suffering from diarrhea and/or respiratory illness. Results: Results show inequalities in child health utilization by sex of the child, maternal education, and across household income quintiles. Overall, health use increased over time and income; however, a smaller increase is observed for the top two income quintiles. Male health service utilization improved over time across all income groups; female health use also improved with the exception of the top quintile. Conclusions: In order to improve child survival, it is important to understand the determinants of healthcare use so that appropriate policies can be developed to maximize health services utilization. This study undertakes such an analysis to understand determinants of health services utilization for children in Nepal. This study demonstrates that sex of the child, household income, mother's education, partner's employment, and distance to health provider play a role in determining health services utilization for children under-5 years in Nepal. An important health equity implication of the association is the design of health policies and programs that reach children most at risk by increasing coverage to ensure including or targeting programs towards them. The analysis highlights the usefulness of the NDHS 1996 and 2001 data as tools to conduct health services research, and in developing policies to address health inequalities and improve access to underutilized health services.

Investments in the continuum of care can have significant returns for improved and equitable access, health, poverty, and health systems. 3. There is a large difference in how RMNCH conditions affect different income groups; investments in RMNCH can lessen the disparity in terms of both health and financial risk.

This volume also argues that the idea of social determinants of health (SoDH) is useful to study PEM in children; however, cultural factors associated with food habits cannot be overlooked when studying PEM in children in Nepal because it if found that children's nutritional status is significantly determined by the families' food habits and culturally practised belief systems.

Book excerpt: This study seeks to establish the relative importance of service access and quality on utilization of preventive health services in the western and middle-western Hill region of Nepal. Access was measured in terms of travel time to the nearest health post and coverage by outreach workers. The quality of static services was defined in structural terms: physical infrastructure, number of staff, availability of drugs and holding of special maternal and child health clinics. The initial analysis showed that no single indicator of quality was of overriding importance and therefore an overall quality index was constructed. After adjustment for access and for socioeconomic characteristics of families and communities, a very pronounced relationship between overall structural quality of the nearest health post and service uptake persisted. The adjusted odds of using some form of antenatal service were 6.6 times higher in the catchment areas of high quality posts than in areas served by low quality posts. The corresponding figure for receipt of BCG vaccination is 8.1. By comparison, the effects of travel time to the nearest health post are modest. Uptake of services is about twice as high when there is a health post in the community. Regular monthly visits by outreach workers also had a marked effect on service utilization. These results suggest that investment in the quality of health posts is more important than further increases in their number and that a further expansion of outreach services is a priority.

Book excerpt: The determinants of health care use among rural, low-income mothers and their children were assessed using a multi-state, longitudinal data set, Rural Families Speak. The results indicate that rural mothers' decisions regarding health care utilization for themselves and for their child can be best modeled using a simultaneous systems approach to negative binomial regression. Mothers' visits to a health care provider increased with higher self-assessed depression scores, increased number of child's doctor visits, greater numbers of total children in the household, greater numbers of chronic conditions, need for prenatal or post-partum care, development of a new medical condition, and having health insurance (Medicaid/equivalent and HMO/private). Child's visits to a health care provider, on the other hand, increased with greater numbers of chronic conditions, development of a new medical condition, and increased mothers' visits to a doctor. Child's utilization of pediatric health care services decreased with higher levels of maternal depression, greater numbers of total children in the household, if the mother had HMO/private health care coverage, if the mother was pregnant, and if the mother was Latina/African American. Mother's use of health care services decreased with her age, increased number of child's chronic conditions, income as a percent of the federal poverty line, and if child had HMO/private health care insurance. The study expands the econometric techniques available for assessing maternal and pediatric health care use and the results contribute to an understanding of how rural, low-income mothers choose the level of health care services use for themselves and for their child. Additionally, the results would assist in formulating policies to reorient the type of health care services provided to this vulnerable population.
