Les enfants malades sont exposés, entre autre, au risque d'accident vasculaire cérébral. Cette complication concerne les populations SS et SpO thalassémiques. L'objectif de cette étude rétrospective était de faire un état des lieux du dépistage des complications vasculaires cérébrales chez l'enfant drépanocytaire en Martinique, de 2004 à 2007. Nous avons étudié 124 enfants âgés de 2 à 18 ans, drépanocytaires SS ou SpO thalassémiques sans antécédent d'accident vasculaire cérébral suivis au Centre Intégré de la Drépanocytose. La proportion retrouvée des principales complications aiguës de la maladie correspondait aux données de la littérature. Nous avons ensuite étudié la population des enfants ayant au moins eu une consultation médicale entre 2004 et 2007 : 105 enfants. Parmi ces enfants, 78% avaient bénéficié d'un dépistage de la vasculopathie cérébrale par échographie Doppler transcrânienne (EDTC). 15% présentaient des EDTC pathologiques et 7,5% avaient une vasculopathie confirmée. Un des facteurs associés retrouvé était l'atopie. Une prise en charge thérapeutique adaptée permettait d'éviter la survenue d'accidents vasculaires cérébraux.
