Ces investissements auraient toutefois des retornbees substantielles : 65 millions de cas de retard de croissance et 265 millions de cas danemie chez la femme auraient ete evites en 2025 comparati vement a I' annee de reference 2015. En outre, sur 10 ans, au moins 91 millions d' enfants supplementaires auraient ete traites pour emaciation severe et 105 millions de nourrissons additionnels auraient ete beneficiaires d'un allaitement matemel exclusif au cours de leurs premiers six mois de vie. L'atteintede l'ensemble de ces cibles permettrait d'eviter au moins 3,7 millions de mortalites infantiles. Chaque dollar investi dans ce paquet d'interventions genererait des rendements econorniques de4 a 35 dollars, ce qui fait du financement de la nutrition precoce l'une des interventions de developpement les plus rentables. Certaines des cibles - particulierement celles qui sont axees sur la reduction du retard de croissance chez l'enfant et de I'anemie chez la femmeparaissent ambitieuses et demanderont des efforts concertes de financement, de mise a l' echelle et d' engagement continu; toutefois, l' experience tiree de plusieurs pays indique que leur atteinte reste possible. Les investissements au cours des 1000 premiers jours decisifs de la petite enfance sont inalienables, transportables et porteurs de retombees tout au long de la vie - non seulement pour les enfants directement concernes, mais aussi pour nous tous - sous forme de societes plus robustes, qui agissent comme moteur des economies a venir.

Countries across Europe understand what they are or what they should include differently. This study describes the experiences of nine countries detailing the ways they have opted to organize and finance public health services and train and employ their public health workforce. It covers England France Germany Italy the Netherlands Slovenia Sweden Poland and the Republic of Moldova and aims to give insights into current practice that will support decision-makers in their efforts to strengthen public health capacities and services. Each country chapter captures the historical background of public health services and the context in which they operate; sets out the main organizational structures; assesses the sources of public health financing and how it is allocated; explains the training and employment of the public health workforce; and analyses existing frameworks for quality and performance assessment. The study reveals a wide range of experience and variation across Europe and clearly illustrates two fundamentally different approaches to public health services: integration with curative health services (as in Slovenia or Sweden) or organization and provision through a separate parallel structure (Republic of Moldova). The case studies explore the context that explain this divergence and its implications. This study is the result of close collaboration between the European Observatory on Health Systems and Policies and the WHO Regional Office for Europe Division of Health Systems and Public Health. Book excerpt: Abstract: In the public sector in developing countries, leakage of public resources could prove detrimental to users and affect the well-being of the population. This paper empirically examines the importance of leakage of government resources in the health sector in Chad, and its effects on the prices of drugs. The analysis uses data collected in Chad as part of a Health Facilities Survey organized by the World Bank in 2004. The survey covered 281 primary health care centers and contained information on the provision of medical material, financial resources, and medicines allocated by the Ministry of Health to the regional administration and primary health centers. Although the regional administration is officially allocated 60 percent of the ministry's non-wage recurrent expenditures, the share of the resources that actually reach the regions is estimated to be only 18 percent. The health centers, which are the frontline providers and the entry point for the population, receive less than 1 percent of the ministry's non-wage recurrent expenditures. Accounting for the endogeneity of the level of competition among health centers, the leakage of government resources has a significant and negative impact on the price mark-up that health centers charge patients for drugs.

The explosion of the HIV/AIDS pandemic, the persistence of malaria as a major killer, and the resurgence of diseases like tuberculosis which were previously under control, have brought about changes in the health system, with implications for its governance, especially in view of the diminished capacity of the public health facilities to cope with a complex range of expanded needs. Government responsibilities and objectives in the health sector have been redefined, with private sector entities (both for profit and not-for profit) playing an increasingly visible role in health care provisions. The reasons for collaborative patterns vary, but chronic under-funding of publicly financed health services is often an important factor. Processes of decentralisation and health sector reforms have had mixed effects on health care system performance; while private health insurance markets and private clinics are pointers to a growing stratification of the health market, in line with the intensified income and social differentiation that has occurred over the last two decades.These developments call for health sector reforms.

Le rapport Améliorer la couverture effective en matière de santé examine une approche particulière visant à améliorer la couverture effective : les incitations financières sous la forme d’un financement basé sur la performance (FBP), une réforme globale qui comprend généralement la rémunération liée à la performance pour les agents de santé de première ligne ainsi que l’autonomie des établissements, la transparence et l’engagement communautaire. Ce rapport de recherche s’appuie sur un vaste ensemble d’études rigoureuses et de nouvelles analyses. Par rapport au statu quo, dans les pays à faible revenu dotés de systèmes de santé centralisés, le FBP peut entraîner des gains substantiels en matière de couverture effective. Toutefois, les avantages relatifs du FBP — la composante rémunération liée à la performance en particulier — sont moins clairs lorsqu’il est comparé à deux autres approches, à savoir : le financement direct des établissements, qui permet d’octroyer des budgets de fonctionnement aux services de santé de première ligne en accordant aux établissements l’autonomie en matière d’allocations budgétaires, mais sans rémunération liée à la performance ; et le soutien financier lié à la demande pour les services de santé (c’est-à -dire les transferts monétaires conditionnels et les vouchers). Bien que le FBP se traduise souvent par des améliorations marginales, combler les lacunes importantes de la couverture sanitaire effective n’est pas encore à la portée de nombreux pays. Néanmoins, d’importantes leçons et expériences tirées de la mise en oeuvre du FBP au cours de la dernière décennie peuvent orienter le financement de la santé à l’avenir. En particulier, pour réussir, la réforme du financement de la santé devra sans doute moins se focaliser sur la rémunération à la performance tout en conservant les éléments de financement direct des établissements, l’autonomie, la transparence et l’engagement communautaire.
