Book excerpt: In the 2010 Comprehensive Spending Review the Coalition Government announced its decision to achieve the internationally agreed target of providing 0.7 percent of Gross National Income as ODA from 2013. This will involve spending an additional 2.5 billion pounds in 2013-14 to make the total DFID budget 11.3 billion pounds in that year. There will be a large increase in spending on fragile and conflict affected states and it will be difficult to ensure that every pound is well spent in such war-torn environments. When scrutinising DFID's accounts the MPs were also surprised to discover that the Pope's visit was paid for in part by money supposed to be for overseas development aid (ODA). The Committee expects a response from the Government as to what the £1.85 million, transferred to the Foreign Office for the papal visit, was spent on and an explanation as to how this was ODA compliant. The Comprehensive Spending Review (CSR) announced reductions in DFID's running costs to 2% of the total budget. If achieved, this would make DFID the most cost-efficient development organisation in the world.This is to be achieved by a large reduction in back office administration costs (which excludes front-line staff) of £34 million over the CSR period. The International Development Committee supports the proposals to make savings in back office staff, but the MPs are warning that Ministers must ensure that reduced administration budgets do not affect the ability to deliver aid programmes on the ground. While declining as a share of total costs, running costs will increase in real terms over the next four years because the total budget will rise so much.

Defra is one of the smallest of Government Departments but it has faced among the most substantial budget cuts, which are set to continue up to 2016. Defra relies on a large number of arms length bodies to deliver many significant areas of policy. Ministers must clarify how further budgets cuts of over £300 million over the coming 2 years will impact on the funding provided to these agencies and the ability of the Department to respond to emergencies, exemplified by the flooding over Christmas and the New Year. Among the most significant challenges facing the Department in the next 12 months is implementation of the new Common Agricultural Policy, and there are risks that the introduction of a new digital system for delivering payments to farms will cause problems for those farmers who do not have access to rural broadband. The Committee found that the results of the Defra staff survey revealed an increased lack of confidence in the management and leadership of the Department, issues which must be addressed. Further challenges facing Defra in the coming year include policy relating to bovine TB and badger culling, its proposals on biodiversity offsetting, the introduction of plastic bag charging, and examining the role and potential of GM technology in food production.

Book excerpt: Despite the fact that the Department for Environment, Food and Rural Affairs has made improvements in its financial management, the NAO cannot yet conclude that the Department is achieving value for money in its financial management activity. This is because the spending watchdog expected faster progress in improving performance since it last reported in 2008 and a higher level of financial maturity, given the resources spent and the focus on financial management. The Department has undertaken a number of projects designed to strengthen its financial management. These have had some positive results, such as revised management reporting and improved forecasting, although the Department has not fully assessed all the benefits of these projects. Despite increasing the number of permanent, qualified finance staff and also offering financial skills training courses for non-financial staff, there are still weaknesses in financial capability. Financial skills could also be better integrated across the Department. The Department should also focus on improving its commercial skills, such as contract management. It should also develop a strategic model for engagement with its arm's-length bodies, to understand the risks that they face and opportunities open to them. Since 2002, Defra has consistently underspent against its Parliamentary estimate. The underspend in 2010-11 was £530 million (10 per cent). The Department has, however, improved its management of expenditure against its departmental expenditure limit, reducing its underspend in 2010-11 to £34 million (one per cent of expenditure).

The challenges facing Defra are first whether the reduced budget available to it is sufficient for its task, and second how to make the correct policy choices so as to allocate smaller funds effectively.

Although life expectancy and survival rates in the United States have improved dramatically over the past century, Americans live shorter lives and experience more injuries and illnesses than people in other high-income countries. The U.S. health disadvantage cannot be attributed solely to the adverse health status of racial or ethnic minorities or poor people: even highly advantaged Americans are in worse health than their counterparts in other, "peer" countries. In light of the new and growing evidence about the U.S. health disadvantage, the National Institutes of Health asked the National Research Council (NRC) and the Institute of Medicine (IOM) to convene a panel of experts to study the issue. The Panel on Understanding Cross-National Health Differences Among High-Income Countries examined whether the U.S. health disadvantage exists across the life span, considered potential explanations, and assessed the larger implications of the findings. U.S. Health in International Perspective presents detailed evidence on the issue, explores the possible explanations for the shorter and less healthy lives of Americans than those of people in comparable countries, and recommends actions by both government and nongovernment agencies and organizations to address the U.S. health disadvantage.
