Chacun de ces axes est défini et situé dans la littérature scientifique et le contexte légal québécois et canadien. Après une description détaillée de l'organisation spécifique de la pharmacie de l'hôpital Sainte Justine, les travaux réalisés pendant ce stage sont présentés. Dans chacun des cinq axes, des mesures issues du modèle québécois et applicables en France sont proposées pour l'amélioration des pratiques.

Les processus en place y sont résumés et associés aux résultats de l'auto-évaluation. Les résultats de l'autoévaluation et les différents audits menés permettent de prioriser les actions d'amélioration à mettre en oeuvre. On remarque que le concept de soins pharmaceutiques, qui est bien intégré au Québec, commence à apparaître dans le référentiel de certification de la Haute Autorité de Santé. D'autres Pratiques organisationnelles requises ou normes d'Agrément Canada pourraient également être adoptées par la Haute Autorité de Santé. Les pharmaciens français devront se préparer à ces perspectives d'évolution de la pharmacie hospitalière.
