Le rt-PA, administré après un AVC ischémique diminue les séquelles si le protocole est appliqué dans les 3 premières heures. Nous avons réalisé une étude prospective sur les délais d'admission des AVC au CHU de Rouen pendant 4 mois. Les 131 patients sélectionnés sont admis aux urgences avec une médiane de temps à 6 heures. 35% se présentent en moins de 3 heures. 24.4% des patients ont réalisé le scanner cérébral en moins d'1 heure (médiane à 2 h 09). Le sexe masculin, les antécédents coronariens, l'appel du SAMU et le transport par VSAB sont les facteurs diminuant le délai d'admission. L'éloignement et la solitude augmente significativement les délais. Les victimes doivent parvenir à l'hôpital le plus rapidement possible. Le grand publique doit être éduqué pour reconnaître les signes d'alerte et encouragé à prévenir toujours le centre 15. Toute la filière de soins doit se redynamiser : l'AVC est une urgence au même titre que l'infarctus du myocarde.
