Within but little over a decade -a new science has arisen and a new theory of infec tious diseases has been established, while the advances made in many other branches of internal medicine have been hardly less remark able. Indeed it is not too much to say that a new era has be gun, one in which the rational treatment of disease engages the best thought of the best workers, supplementing, while not supplant ing, the study of pathological anatomy by which the preceding era was characterized. The science of medicine has been in great part recast - the tithe is now ripe for it to be rewritten. To this end the co-operation of many recognized authorities in Europe and America has been secured, the results of whose labors will be presented in the successive volumes of this series. In arranging the order of subjects it has been thought best to re serve the consideration of infectious diseases for the later volumes, in the hope that by the time they are published a solution of some of the problems in the pathogenesis and therapy of these affections, as yet but partially worked out, will have been reached. It at times extends pandemically, with a velocity and wide spread such as we see in no other disease at present known. Numerous phenomena of its epidemiological behavior border on the miraculous, and fill us with wonder and astonishment. On the other hand, influenza in single cases produces so manifold and often such remarkable morbid pictures, that the study of the disease, though demanding great labor, brings a rich reward eventually. In the closing years of this century influenza has given us occasion to test all the modern attainments of medical science. Physicians have labored whole-souled in the attack against this pestiferous disease and they have also worked with such ardor in the solution of the riddle which confronted them, and they have endeavored to fathom it with such an ideal working together in all directions, that it may be truly said that influenza has become the teacher in many branches of medicine, and it has demonstrated to the whole world such thoroughness of methods on the part of physicians and investigators that our profession has a right to feel proud of its members. Epidemiology. With the discovery of the microorganism of influenza by R. Pfeiffer in the year 1892, a milestone was reached in the history of this disease. The obscurity which shrouded the epidemics of influenza of former centuries as regards the etiology and the epidemic character of the disease, will, thanks to the researches of this savant, be lifted from future epidemics. In former times the occurrence of epidemics was referred to coincident natural phenomena, as the appearance of comets, volcanic eruptions, earthquakes, etc., and even Webster attempted to prove this association in his work on the history of epidemic diseases. A purely etiological division of the subject is still impossible, great as the progress of the past few years in this direction has been. Distinctions based upon acuteness and chronicity are hardly applicable to an affection like pleurisy, in which there are innumerable gradations between the two extremes. The character of the exudate, whether fibrinous, serofibrinous, or purulent, still remains the prominent factor which regulates both the prognosis of the disease and its practical management by the physician. Dry Pleurisy. Definition. - Dry or fibrinous pleurisy is an acute or chronic inflammation of the whole or a part of the pleural membrane, characterized by an exudation of fibrin upon the free surface of the latter. Etiology. Dry pleurisy is a very common disease. This is to be inferred not so much from its clinical frequency as from the fact that in autopsies it is almost an exceptional occurrence to find the pleura entirely free from adhesions. Smith, for example, in a series of one hundred and twenty post-mortem examinations in individuals who had died from all sorts of diseases, found adhesions in no less than eighty percent. That such adhesions are extremely frequent is certainly in accord with the general experience of most observers. As a primary affection, however, dry pleurisy is comparatively rare; Engster, among one hundred and sixty-three cases of apparently primary pleurisy, found only sixteen of the dry form. Three possible causes of primary dry pleurisy are commonly given, viz., cold, rheumatism, and tuberculosis.
