Book excerpt: Abstract: This research examines the cross-border utilization of Mexican health care services by Hispanic border residents. Using data from a community survey conducted in Webb County, Texas along the U.S.-Mexico border during the summer of 2011, personal, economic, insurance, and cultural factors were examined as possible predictors of the practice of seeking health care services in Mexico. The objectives of the study were to identify the strongest predictors explaining the use of Mexican health care services, and to determine whether the predictors varied depending on the services sought. The types of services studies were medical, dental, and pharmaceutical services. To observe general cross-border behavior regardless of the type of service, a fourth variable was constructed to determine use of at least one of these Mexican health care services. Overall results obtained from a series of binary logistic regressions suggest that lack of health insurance and cultural factors are the most significant predictors of cross-border patient mobility. Specifically, having any type of health insurance coverage, living in an English-dominated household, and residing longer in Webb County significantly reduce the odds of crossing to Mexico for health purposes. Other significant variables were having only a high school diploma and having children between the ages of 6 and 18 living in the household, both of which increase the likelihood of seeking health services in Mexico. Surprisingly, household income was not significant in any of the four models, a finding that suggests that cross-border patient mobility is not dependent on the individuals' financial capabilities. Predictors were the same for seeking most types of health services. In all, this study suggests that seeking health care in Mexico is a more affordable and culturally acceptable alternative for many Hispanic border residents who face barriers accessing health care domestically.

Book excerpt: US-Mexico border region area has unique social, demographic and policy forces at work that shape the health of its residents as well as serves as a microcosm of migration health challenges facing an increasingly mobile and globalized world. This region reflects the largest migratory flow between any two nations in the world. Data from the Pew Research Center shows over the last 25 years there has never been lower than 140,000 annual immigrants from Mexico to the United States (with peaks over 700,000). This migratory route is extremely hazardous due to natural (e.g., arid and hot desert regions) and human made barriers as well as border enforcement practices tied to socio-political and geopolitical pressures. Also, reflecting the national interdependency of public health and human services needs, during the most recent five year period surveyed the migratory flow between the US and Mexico has equaled that of the flow of Mexico to the US--both around 1.4 million persons. Of particular public health concern, within the US-Mexico region of both nations there is among the highest disparities in income, education, infrastructure and access to health care--factors within the World Health Organization’s conceptualization of the Social Determinants of Health, and among the highest rates of chronic disease. For instance obesity and diabetes rates in this region are among the highest of those monitored in the world, with adult population estimates of the former over 40% and estimates in some population sub-groups for the latter over 20%. The publications reflected in this Research Topic, all reviewed from experts in the field, addressed many of the public health issues in the US Mexico Border Health Commission’s Healthy Border 2020 objectives. Those objectives-- broad public health goals used to guide a diverse range of government, research and community-based stakeholders--include Non Communicable Diseases (including adult and childhood obesity-related ones; cancer), Infectious Diseases (e.g., tuberculosis; HIV; emerging diseases--particularly mosquito borne illnesses), Maternal and Child Health, Mental Health Disorders, and Motor Vehicle Accidents. Other relevant public health issues affecting this region, for example environmental health, binational health services coordination (e.g., immunization), the impact of migration throughout the Americas and globally in this region, health issues related to the physical climate, access to quality health care, discrimination/mistreatment and well-being, acculturative/immigration stress, violence, substance use/abuse, oral health, respiratory disease, and well-being from a social determinants of health framework, are critical areas addressed in these publications or for future research. Each of these Research Topic publications presented applied solutions (e.g., new programs, technology or infrastructure) and/or public health policy recommendations relevant to each public health challenge addressed.

Book excerpt: The intensification of U.S. border security since 9/11 seeks to stem unauthorized immigration, the illicit narcotics trade, and terrorism, but also affects daily life in U.S.-Mexico border communities. In addition to ongoing border security changes, the U.S. healthcare system is also undergoing major shifts with the implementation of the 2010 Patient Protection and Affordable Care Act. This dissertation uses ethnographic data collected in two South Texas Mexican American communities over 10 months from 2011-2013 to examine how border security and healthcare policies intersect to affect healthcare access for patients, their families and friends, and their providers. This dissertation explores how these dynamics shape the behaviors of both patients and providers. My research points to three main findings. First, border security policies unintentionally limit cross-border healthcare access for U.S. residents--both citizens and noncitizens--seeking cheaper healthcare in Mexico, but do so differently depending on immigration status. Second, I develop the concept of "Community Cultural Health Capital" (CCHC) to describe the health-relevant cultural capital South Texas residents use to navigate exclusion from mainstream U.S. healthcare resources. Focusing on access to prescription drugs in particular, I find patients and their caregivers use CCHC to access prescriptions despite significant barriers, such as lack of health insurance and geographic isolation. Third, I show how healthcare providers develop localized kinds of CCHC to facilitate patient healthcare access. I study primary care teams at community health centers to identify how provider teams mobilize CCHC to navigate patient access barriers and fill gaps in the U.S. healthcare safety net. This dissertation suggests that border security policies targeting illicit cross-border flows of people and narcotics interact with healthcare policies in unpredictable ways. Yet policymakers often fail to consider how border security policies affect access to healthcare. Similarly, medical sociologists often overlook resources patients and providers must mobilize to address structural barriers to healthcare. Including these dissertation findings in discussions of security and health policy will better account for the breadth of border policy consequences. Exploring this case also provides insight into the skills providers and communities develop to manage policy changes and improve exclusion from U.S. healthcare.

Today, about one-quarter of the U.S. population consists of immigrants or the children of immigrants. Given the sizable representation of immigrants in the U.S. population, their health is a major influence on the health of the population as a whole. On average, immigrants are healthier than native-born Americans. Yet, immigrants also are subject to the systematic marginalization and discrimination that often lead to the creation of health disparities. To explore the link between immigration and health disparities, the Roundtable on the Promotion of Health Equity held a workshop in Oakland, California, on November 28, 2017. This summary of that workshop highlights the presentations and discussions of the workshop.
