Book excerpt: Healthcare providers, especially those working in emergency and intensive care settings, are frequently exposed to traumatic experiences. These experiences affect their emotional, physical, psychological, or spiritual well-being. This integrated review analyzed the effectiveness of Critical Incident Stress Debriefing (CISD) as an intervention for emergency healthcare providers secondary to exposure of traumatic events in their clinical practice. Fifteen (15) articles and journals using the PRISMA model searching for evidence supporting the application of CISD in managing the effects of traumatic experiences by emergency healthcare providers were reviewed. The research showed that emergency healthcare workers mitigate stress in multiple ways. The non-mandatory and formalized CISD process is open to the staff involved and facilitated by trained professionals It is designed to offer an open, non-judgmental setting, where those involved may express their feelings with the goal to avoid internalization of the feelings and stress surrounding the incident, while reinforcng that the individual is not alone in experiencing this stress and emotions. Working in emergency medicine increases the likelihood of exposure to frequent critical incidents, which may lead to Post Traumatic Stress Disorder(s) (PTSD). This integrated review supports the use of CISD for helping healthcare providers working in emergency medicine cope with the challenge of being exposed to trauma and the related effects. Book excerpt: An identified problem that directly affects nurses and patient care is the exposure of nurses to workplace violence (WV) and trauma. Long-term consequences from lack of addressing this issue have far-reaching effects. Absenteeism, burnout syndrome (BOS), decreased morale, decreased cognitive functioning and level of care and job dissatisfaction are just a few (Gates, Gillespie, and Succop, 2011). According to a Joint Commission Sentinel Event Alert in 2010, actions by healthcare facilities to prevent violence in the healthcare setting need to be instrumented (The Joint Commission, 2010). Using a critical incident stress debriefing (CISD) tool has had a positive and lasting effect on the negative impact of violence and trauma at work. Immediate intervention during the first few hours or days following a traumatic event can provide the necessary support that is lacking. Studies show brief crisis intervention at the worksite had a positive clinical effect up to 2 years post treatment (Boscarino, Adams, and Figley, 2005). CISD models work to the benefit of both nurses and facilities. Consisting of a group of peers immediately responding to all critical events, a 2-year cost savings following the initiation of one program was $268,000 (Flannery, 1999). Management is not equipped to properly support staff or report incidents. Implementation, resources, training and reevaluation will make the CISD tool beneficial to facilities and nurses. Preventing the symptoms of post-traumatic stress disorder (PTSD) rather than reacting to the myriad consequences of its untreated sequelae is a proactive approach. Providing for the well-being and safety of front-line caregivers is an expectation. Having the tools, support and people to guarantee this process will benefit not only nurses and patients, but organizations as well. Several tools for evaluation of the CISD implementation are already in use at most facilities. They are the patient satisfaction survey (see Appendix 4) and the Healthcare productivity survey (see Appendix 1). PTSD symptoms can be measured using the critical incident stress management (CISM) (Everly and Mitchell, 1997) questionnaire (see Appendix 3a) and the PCL-S (Weathers, Huska, and Keane, 1991) tool (see Appendix 3b). Publicized findings will provide information necessary for more healthcare facilities to put into place a program to provide a healthy and safe working environment for all.

The nurses' code of conduct states that nurses should be aware of and, where necessary, take action to protect colleagues' health and safety; thus it is recognized that some form of support is needed yet there is little evidence that support groups are regularly provided (Spencer, 1994). It is essential that a practice of support should be supported organizationally. When resources are properly utilized and healthcare providers feel that it is acceptable and appropriate to grieve, they will be able to do so in their own manner and at their own time, knowing that they are cared for and supported by the health care system (Brosche, 2007). A formal interdisciplinary team can help guide and inspire nurses to transform grow, and blossom in body and spirit. The initiation of a critical incident stress management team at an acute care tertiary hospital benefits the staff in that it encourages the employee to seek help during critical incidents that affect normal coping mechanisms of an individual pulling forth a sense of empowerment, dignity, and respect." -- Abstract.
