Using estimates from the Insurance Component of the Medical Expenditure Panel Survey for 2011, this Statistical Brief discusses differences in state and local governments' employer-sponsored insurance costs across the nine census divisions--New England, Middle Atlantic, East North Central, West North Central, South Atlantic, East South Central, West South Central, Mountain, and Pacific. Within each census division, it also compares costs for state and local government employees to that for private sector employees. The Brief examines three general types of health insurance coverage--single coverage (employee only), employee-plus-one coverage (employee and one family member), and family coverage (employee plus at least one other family member). Both average annual premiums and employee contributions toward the premium are discussed. Only those differences that are statistically significant at the 0.05 significance level are discussed. See the "Definitions" section for a list of states included in each census division.

According to data from the Insurance Component of the Medical Expenditure Panel Survey (MEPS-IC), 65.4 percent of state and local government employees were enrolled in employer-sponsored health insurance plans compared to 53.6 percent of private sector employees. State and local government employees were also more likely to work where health insurance is offered than were private employees. Of the 19.6 million state and local government employees in 2009, 75.5 percent were eligible to enroll in employer-sponsored health insurance coverage compared to 69.6 percent of the 110.5 million private sector employees. This Statistical Brief focuses on state and local government enrollment rates, premiums, and employee contributions for employer-sponsored health insurance by census geographic division. These health insurance characteristics varied significantly across the United States for state and local government employees. Only those estimates that had a statistically significant difference from the national average at the 0.05 percent significance level are noted in the text.

Book excerpt: Employer-sponsored health insurance is the primary source of health insurance coverage in the United States for active employees. It is especially important to the 19.4 million state and local government employees. According to data from the Insurance Component of the 2006 Medical Expenditure Panel Survey (MEPS-IC), 99.9 percent of state and local government employees work where health insurance is offered and 65.5 percent of these employees are enrolled in employer-sponsored health insurance. Both these figures are significantly higher than the same values for private sector employees. In recent years, premiums and employee contributions for employer-sponsored health insurance provided by state and local governments have risen dramatically. For instance, annual employee contributions towards family coverage for state and local government employees rose from $1,267 to $2,058 (62.4 percent) between 2000 and 2006. Contribution costs are only a portion of the costs of health insurance for these employees. Other factors, such as whether an employee has a deductible, the size of deductibles, and the size of co-pays and coinsurance, also contribute to employees' out-of-pocket cost of care. This Statistical Brief examines the levels of these deductible, co-pay, and coinsurance for state and local government employees and how they vary by census division. Among the items examined are the percentage of enrollees who paid a deductible and the size of deductibles, what percentage of enrollees paid co-pays and the size of co-pays, and the average coinsurance percentage paid by enrollees. Only estimates that had a statistically significant difference from the national average at the 0.05 percent significance level are noted in the text.

According to data from the Insurance Component of the 2010 Medical Expenditure Panel Survey (MEPS-IC), approximately 113.2 million of the 128.0 million civilian employees (including private and state and local, but not federal employees) worked where the employer offered health insurance. Of those employees who worked where health insurance was offered, approximately 69.1 million were enrolled. Beginning in September 2011, the Rate Review program established under the Patient Protection and Affordable Care Act requires health insurers to justify premium rate increases of 10 percent or more before the increase can take effect. This Statistical Brief presents MEPS-IC estimates by type of coverage (single, employee-plus-one, and family) on average premiums for 2010 and changes relative to 2008 (the height of the economic downturn). Estimates are also presented by census geographic division for employees who enrolled in offered health insurance plans. A companion Brief, Statistical Brief #365, also describes recent changes in average premiums by census division but provides estimates separately for the public and private sectors. Only those estimates that had a statistically significant difference from the national average at the 0.05 percent significance level are noted in the text.
