Research has revealed illness parameters and well-sibling demographic factors that may impact the psychosocial adjustment of well-siblings. The present study examined well-sibling age and gender, illness severity, and time since diagnosis to predict well-sibling quality of life. Coping was used to predict well-sibling adjustment after controlling for significant demographic or illness parameters that were predictors of well-sibling adjustment. Types of coping examined included approach and avoidance coping styles, total number of coping strategies, and a relative coping ratio of avoidance coping styles. Potential differences in well-sibling adjustment according to informant were also examined, including parent-report and self-report. Participants included 43 well-siblings of children with cystic fibrosis, aged 7 to 19 years, who presented in a children's hospital's cystic fibrosis clinic. Well-siblings completed assessments quality of life and strategies for coping with cystic fibrosis. Parents completed demographic questionnaires and quality of life assessments for both the sick-child and the well-sibling. Results found that well-sibling age and total number of coping strategies used were significant predictors of adjustment. Well-sibling adjustment as measured by quality of life was more impaired than previous studies found that examined psychopathology, and well-sibling self-report was significantly more impaired than parent-report of adjustment. These data emphasize the importance of obtaining quality of life ratings directly from the well-sibling rather than relying on parental report. Future research should incorporate measures of well-siblings' knowledge of the illness and well-siblings' ratings of severity of the illness. Additionally, future intervention studies should seek to incorporate coping curriculum to help improve well-sibling adjustment.

Recent data are revising our notions of chronic illness. Evidence is mounting that chronic psychiatric disorders reflect, in part, abnormalities of brain structure and function. In this sense, they are, in part, medical disorders. On the other hand, a number of traditionally labeled medical disorders produce a broad range of psychological symptoms and are exquisitely sensitive to psychosocial influences. Families undergo a complex process of adaptation during which their response to stress and their fundamental beliefs about learning and parenting change. Previous researchers differ substantially in their findings on how individuals are affected by having a sibling with a chronic pediatric illness. Some studies assert that chronic illness, though a stressor for healthy siblings, results in benefits such as higher levels of empathy, coping skills, and family cohesiveness. Other researchers find more negative effects such as elevations in academic and behavioral problems and declinations in parental support, and quality of life. Because the literature lacks consensus on how individuals adapt to illness in a sibling, the present research is designed to address both positive and negative effects including empathy, depression, anxiety, and the health of the sibling relationship. The sample for this research is comprised of individuals currently attending college. At present, college students as a specific demographic group have not yet been investigated as siblings of those with pediatric illnesses. Recruiting students from the University of Dayton, 30 participants with a sibling who has a chronic illness and 31 participants with healthy siblings completed a series of questionnaires. The series included a demographics questionnaire along with measures of empathy, depression, anxiety, and sibling closeness. Results indicate that individuals with a sibling who has an illness experience higher levels of empathy. Further, participant gender was determined to be a predictor of empathy in these individuals while age of the sibling at diagnosis and sibling closeness were not. No differences in depression, anxiety, and sibling closeness existed between groups. These findings partially support the premise that pediatric chronic illness has long term effects on healthy siblings, but more research is needed to further develop our understanding of the effect of chronic illness on familial relations across time.

This book is an attempt to describe how quality of life is affected at different stages of the disease process. Comprehensive reviews are provided of the impact on the child's physical activity, social life, and school and educational achievements. Special consideration is given to children with leukemia (one of the more common cancers) and brain tumors. Cancer does not just affect the child but every member of the family. Consequently there is coverage of the effects on parents and also healthy brothers and sisters. To the extent that improvements in survival have been achieved by national and international collaboration between clinicians, it is concluded that efforts to improve the quality of these children's lives is dependent on collaboration between clinicians, nurses, and behavioral scientists at national and international levels. This book should provide an impetus for such collaboration.
