D'autre part elle ne permet pas de combattre les bactéries impliquées dans les pathologies buccales. Des antibiotiques sont prescrits dans le service d'Odontologie de l'hôpital Saint André à Bordeaux par des praticiens de parcours et d'expérience variés. Bien qu'il existe à l'heure actuelle des recommandations précises, ces prescriptions peuvent elles être praticien ou spécialité dépendantes, ou suivent elles ces recommandations? Pour répondre à cela, une étude rétrospective a été réalisée sur une année, chez des patients adultes par consultation des dossiers présents au sein de ce service en excluant les patients hospitalisés et ceux suivis en implantologie. Les résultats ont montré que la majorité des prescriptions sont conformes aux dernières recommandations de l'Afssaps de juillet 2011. L'amoxicilline en monothérapie reste l'antibiotique de référence tant en antibioprophylaxie qu'en antibiothérapie. D'autre part, la parodontite apicale aiguë est comme pour nos pays voisins la première raison de prescription d'antibiotique même si l'absence de signes généraux ne le justifie pas toujours. Les recommandations sont à suivre car elles sont une synthèse des données nouvelles de la science avec pour objectif d'orienter le praticien vers une bonne pratique et les bonnes références médicales.

Elle constitue une préoccupation mondiale de santé publique. Malgré les politiques mises en place depuis les années 2000, la France restait en 2016 le 3ème pays de l'Union Européenne le plus consommateur d'antibiotiques en secteur de ville avec 30,3 DDJ pour 1000 habitants par jour. Par son statut de prescripteur, le chirurgien-dentiste joue un rôle non négligeable dans cette consommation. Afin d'évaluer les connaissances et comportements des étudiants de chirurgie-dentaire, une enquête par questionnaire a été conduite entre novembre 2017 et janvier 2018 auprès des étudiants inscrits en 6ème année pour l'année universitaire 2017-2018 des Facultés de Chirurgie Dentaire de Lille et Nancy. 101 étudiants ont participé à l'étude. Cette étude a mis en évidence, malgré la confiance des étudiants dans leur prescription, des lacunes dans la connaissance des recommandations de prescription d'antibiotiques en pratique bucco-dentaire. Cette étude a également révélé l'intérêt des étudiants au sujet de la résistance bactérienne aux antibiotiques et leur inquiétude face à ce phénomène. Alors qu'ils ont jugé l'enseignement théorique comme le facteur influençant le plus leur prescription, ils ont le sentiment de ne pas être suffisamment formés et/ou informés. Le second facteur influençant le plus leur prescription sont les recommandations en vigueur. Une amélioration de la formation initiale et de l'accès aux recommandations sur les terrains de stage semblent donc nécessaires pour une juste utilisation des antibiotiques par les chirurgiens-dentistes de demain.

C'est pourquoi des recommandations ont vu le jour, dans le but d'optimiser le traitement des pathologies bactériennes tout en essayant de préserver leur efficacité (politique de réserve). Une étude a été menée auprès des étudiants de quatrième, cinquième et sixième années de la faculté d'odontologie de Clermont Ferrand afin de définir leurs habitudes de prescription en matière d'antibiotiques. Les résultats montrent que les recommandations de bonnes pratiques actuelles (2011) ne sont pas assez connues dans certains domaines. Cependant, il s'avère que plus les étudiants sont avancés dans leur cursus universitaire, plus les conduites à tenir selon ces recommandations sont respectées. L'impact de l'entrée en clinique, du CSCT et du stage actif pourrait être mesuré, sur les pratiques de prescriptions. L'amélioration de la qualité des prescriptions est un enjeu majeur, pour garantir aux patients une prise en charge optimale et durable.

Les recommandations de bonnes pratiques ont été élaborées dans le but d'optimiser le traitement des pathologies infectieuses en Odontologie tout en limitant leur utilisation au strict minimum. Evaluer les habitudes de prescriptions des antibiotiques chez les étudiants peut donc paraître judicieux. Les étudiants de DCE02, DCE03, et TCE01 de l'UFR d'Odontologie de Clermont-Ferrand ont donc participé à une étude pilote visant à tester leurs choix de prescription. Certains résultats interpellent. La non utilisation d'antibiotiques lorsque un geste technique est possible; la prescription d'une antibiothérapie de seconde intention; la prescription d'une antibioprophylaxie. Il apparaît que si une autre étude plus complète était envisagée, quelques modifications seraient à faire dans le protocole d'étude. Cette étude a permis de montrer que certains thèmes et situations cliniques sont moins bien compris par les étudiants. Le questionnaire pourrait être utilisé par les enseignants pour identifier les stratégies de traitements moins bien comprises et par la suite modifier leur enseignement en conséquence. Elle montre l'intérêt pour les recommandations officielles de se renouveler réguliérement au vu de l'évolution des études scientifiques à ce sujet. Elle nous permet d'envisager une étude plus importante qui permettrait de tester l'influence de l'expérience clinique et du stage actif sur les habitudes de prescription. Le but ultime est d'optimiser la prescription qui doit répondre à une situation clinique précise dans un contexte médical particulier en fonction des données acquises de la science en évitant les habitudes et de céder à la pression du patient.

Over the last decade, 11 countries in the Africa Region and 32 countries in other regions showed reductions of 50% or greater in either confirmed malaria cases or malaria admissions and deaths. Malaria control is making a major contribution to reducing mortality in children less than 5 years old and to attaining the health-related Millennium Development Goals.
