Book excerpt: Le tabac, avec 16 millions de fumeurs en France, est un réel problème de santé publique. Avec 66 000 morts directes par an, le tabagisme expose aussi les non-fumeurs à des pathologies graves ou mortelles. Parmi-eux les nourrissons représentent une population vulnérable et leur exposition entraîne un risque accru de pathologies infectieuses, respiratoires, cancéreuses voire de mort subite. Les fumées issues d'une cigarette comptent 4000 substances toxiques dont 60 cancérigènes. Elles peuvent se répandre au delà d'un espace clos et persister sur les surfaces pendant plusieurs mois après qu'une cigarette ait été consumée. L'aération ou la ventilation du logement d'un fumeur ne permet pas d'évacuer l'ensemble des particules dangereuses. Fumer dehors est la méthode de restriction du tabagisme passif la plus efficace mais n’empêche pas totalement les risques sur la santé des personnes exposées. Une évaluation des connaissances sur le tabagisme passif du nourrisson a été réalisée auprès de 110 parents d'enfants en bas âge venant consulter en cabinet de médecine générale. L’enquête, par le biais d'un questionnaire, a montré une insuffisance des connaissances des parents sur le sujet. Une moins bonne connaissance a été observée chez les fumeurs et notamment chez les gros fumeurs. L'âge, le sexe ou la localisation du tabagisme d'un fumeur n'a pas entraîné de différence dans la note obtenue au questionnaire. L'étude reflète des lacunes importantes concernant les connaissances du tabagisme passif chez les parents d'enfant en bas âge. Des campagnes d'information ainsi que des lois plus restrictives doivent être mises en place afin de lutter contre l'exposition des nourrissons.

Afin d’évaluer les connaissances des parents, 10 questions leur était posées. Il s’agissait secondairement d’analyser la catégorie socio-professionnelle des parents et l’exposition éventuelle de l’enfant au tabagisme passif, puis de proposer un moyen d’information qui puisse satisfaire le plus grand nombre. Les parents ont globalement de bonnes connaissances en ce qui concerne les conséquences du tabagisme passif sur la santé de leurs enfants avec une note moyenne au questionnaire de 8,97 sur 10 (écart-type : 1,30), la point le plus méconnu étant le risque de mort subite pour le nourrisson exposé. Les parents appartenant aux catégories socio-professionnelles les plus défavorisés ont une note significativement plus basse au questionnaire (p inférieur à 0,05). Ces derniers exposent également de façon significativement supérieure leurs enfants au tabagisme passif. Ces résultats mettent une nouvelle fois en lumière les inégalités sociales en matière de santé et soulignent le rôle essentiel du médecin généraliste en matière de prévention au sein de son cabinet. L’analyse des réponses a permis de proposer une brochure d’information à destination des parents insistant sur les points les moins bien connus.

Dans le monde, près de la moitié des enfants respirent régulièrement un air pollué par la fumée de tabac dans les lieux publics et 17,6% des enfants en France en 2018 étaient exposés au tabagisme passif à domicile. Les connaissances des parents sur les conséquences du tabagisme sur la santé des enfants sont inégales en fonction de la catégorie socioprofessionnelle. Notre étude avait pour objectif d'étudier le lien entre le fumeur et sa cigarette dans le contexte de la parentalité, de comprendre comment des parents qui connaissent les effets du tabagisme sur eux-mêmes et leurs enfants pouvaient continuer à fumer malgré tout et d'étudier le vécu et les représentations des parents fumeurs ou ex-fumeurs autour de ce thème. Matériel et méthodes : nous avons réalisé une étude qualitative par entretiens semi-dirigés, exploratoires et individuels. Les personnes incluses devaient avoir plus de 18 ans, être parents d'enfants nés après 1991, être fumeurs ou ex-fumeurs. Les interviewés étaient sélectionnés par bouche-à-oreille avec un accès de proche en proche au fil des entretiens et au sein d'un cabinet de médecine générale en milieu précaire. Les entretiens ont eu lieu au domicile des parents pour permettre à ceux-ci de sentir face à un chercheur plutôt qu'un médecin. Le cadre conceptuel de ce travail est celui de la grounded theory. Le canevas des entretiens a évolué au fur et à mesure des entretiens, laissant toujours une libre place à l'évolution spontanée des thèmes abordés par l'interviewé. Les entretiens ont été analysés avec un codage axial du verbatim, à l'aide du logiciel N'vivo. Le nombre d'entretiens à réaliser était basé sur l'arrivée à saturation des données. La durée totale des entretiens était de 926 minutes soit 15,5 heures. Résultats : entre le 17 août 2017 et le 1er octobre 2018, seize entretiens ont été réalisés. L'analyse a fait émerger trois grands concepts : moi, mes proches, la société. Le concept du Moi a fait émerger la problématique du manque, des usages individuels de la cigarette, les représentations des effets du tabac, la question de la fatalité du fumeur à vie, le lien d'amour et de haine entre la cigarette et le fumeur, les fumeurs qui jouent avec le risque en connaissant les conséquences de leur tabagisme. Le concept de Mes proches a exploré la problématique de la parentalité avec ses difficultés et les modifications de consommation du fumeur devenu parent, ainsi que les représentations de la transmission du tabagisme. Et enfin la problématique de la Société a fait émerger des concepts comme la honte et la culpabilité du parent fumeur. Conclusion : être fumeur et parent est complexe, entre amour inconditionnel de ses enfants et amour de la cigarette. L'amour pour son enfant, aussi grand soit-il, n'arrive pas à casser le couple fumeur cigarette. D'autres études pourront explorer spécifiquement les représentations des femmes enceintes ou des parents d'enfants malades du tabagisme passif.

Une analyse thématique des verbatims a été réalisée. Résultats : cette étude nous montre que les parents fumeurs ne sont pas suffisamment informés, et que trop d'enfants sont encore exposés au tabagisme passif. Les parents qui font l'effort de questionner et d'écouter les intervenants médicaux, ont su en tirer une conduite à tenir permettant de limiter au maximum le tabagisme passif chez l'enfant, ils ont tous proposé des mesures de sanction pour la protection des enfants face au tabac. Les parents ayant une consommation importante de tabac, exposent plus leurs enfants, et ont été moins bien informés, de plus pour eux le tabac n'est pas le plus grand responsable des maladies des enfants. Conclusion : les parents ne sont pas suffisamment informés, certains n'ont pas conscience du danger que représente le tabagisme passif pour la santé des enfants. Il est important que les professionnels de santé, ainsi que les professionnels de la petite enfance, informent plus ces parents fumeurs.
