Book excerpt: Lors de la délivrance de l'information, le médecin doit dans un premier temps évaluer la capacité du patient à comprendre le langage médical et dans un second temps s'adapter au niveau de compréhension de celui-ci. L'information médicale délivrée en consultation étant perçue différemment d'un patient à l'autre et ne pouvant être standardisée, l'objectif principal de notre étude était d'explorer la délivrance de l'information médicale par le médecin généraliste au patient lors de l'adressage aux urgences, selon le point de vue du patient. Méthode : Une recherche qualitative a été réalisée pour répondre à l'objectif de ce travail. Des entretiens semi-dirigés ont été réalisés de manière individuelle d'Avril 2023 à Septembre 2023. Ont été inclus des patients ayant consulté un médecin généraliste à son cabinet, médecin traitant ou non, dans le 75, et qui ont été adressés aux urgences au décours de la consultation. Les patients ont été recrutés directement par l'intermédiaire de médecins généralistes informés de l'étude exerçant dans le 75. Résultats : La relation médecin-patient était au centre de l'information délivrée par le médecin généraliste. D'un côté le patient avec la littératie en santé et sa capacité à tolérer I 'incertitude, et de l'autre côté, le médecin avec ses compétences scientifiques et son savoir-être, ont communiqué de façon verbale et non verbale avec comme base de cette relation : la confiance. Le patient s'en remettait au médecin et à ses connaissances médicales, il acceptait les limites du savoir médical et l'incertitude exprimée par le médecin. Aucun patient n'a fait part d'une insatisfaction concernant le contenu de l'information mais certains en ont évoqué la forme, c'est-à-dire, la façon de délivrer l'information. Conclusion : Liant le médecin à son patient, la confiance et la communication verbale, mais aussi non verbale, sont des facteurs essentiels dans la délivrance de l'information médicale. Le patient ne place pas les compétences scientifiques du médecin au premier plan des moteurs du développement de sa confiance, il privilégie la savoir-être du médecin. D'autre part, la formulation d'une incertitude par le médecin ne semble pas atténuer la confiance du patient dans l'information délivrée. Dans la même perspective, il accepte les limites du savoir scientifique du médecin, considérant celles-ci comme naturelles et indissociables de l'exercice de la médecine. Les compétences humaines et sociales du médecin sont donc essentielles dans la délivrance de l'information au patient dans le contexte particulier de l'adressage aux urgences.

MÉTHODE : Une revue de littérature a été menée afin de répertorier les différentes grilles déjà publiées. Puis une recherche qualitative a été effectuée par 5 focus groups auprès de médecins généraliste de Savoie, Haute-Savoie et Isère. Une analyse thématique des verbatim a été conduite par triangulation des chercheurs après retranscription intégrale des entretiens. RÉSULTATS : 15 grilles d'évaluation de la communication médecin-patient ont été publiées dans la littérature. L'analyse des données, à partir du point de vue des médecins généralistes, a fait émerger 7 thèmes : l'accueil, les habiletés générales de communication, l'empathie, la structure de la consultation, l'information et l'éducation du patient, le partenariat et la prise de décision partagée, et la fin de la consultation. CONCLUSION : Ce travail nous a permis d'élaborer une grille de 34 items. L'outil proposé nécessite une validation auprès d'experts et une recherche de concordance entre ses items et le point de vue des patients.

L'objectif principal de ce nouveau travail était d'étudier des sous-populations semblant avoir des spécificités de connaissance de précautions d'emploi (hypothèse de départ) avec étude des facteurs pouvant influencer cette connaissance, notamment la source d'information. Il était en outre question de comparer ces résultats avec ceux des différentes études sur l'observance et la connaissance des effets indésirables par une recherche bibliographique. Méthodes : Une enquête a été réalisée en consultation de médecine générale de juillet à septembre 2015. Après avoir renseigné ses données personnelles, chaque patient indiquait s'il pensait connaître les précautions d'emploi de ses médicaments. Si oui, il les détaillait. Les médecins cherchaient ensuite à faire préciser pour chaque médicament les précautions d'emploi concernant le mode de prise, l'alimentation, les conduites à tenir, les interactions médicamenteuses. Ensuite, pour cette nouvelle étude, des échantillons en fonction des classes thérapeutiques de l'ordonnance des patients ont été constitués afin de les analyser et les comparer. Une recherche bibliographique a été effectuée sur l'observance et la connaissance des effets indésirables pour comparer nos résultats. Résultats : Pour les 5 sous-populations choisies, il n'existait pas de différence significative selon nos différents critères de comparaison, en premier lieu duquel une différence significative de la connaissance des précautions d'emploi. Le nombre de sources d'information n'influe pas sur cette connaissance. Conclusions : Les sous-populations étudiées avec des effectifs restreints n'ont pas permis d'affiner les facteurs influençant la connaissance des précautions d'emploi des médicaments, notamment par l'étude des sources d'information. Cependant les résultats de notre étude semblent en corrélation avec ceux de l'observance, de la connaissance de l'ordonnance et des effets indésirables retrouvés dans la littérature, à savoir une faible connaissance de ces critères.

It additionally supports country efforts to implement WHO International Health Regulations (2005) and OIE international standards, to address gaps identified through external and internal health system evaluations, and to achieve targets of the Sustainable Development Goals.
