Program (SCHIP) finances health care to low-income, uninsured children whose family incomes exceed the eligibility limits under their state¿s Medicaid program, but who cannot afford other health insur. coverage. To participate in SCHIP, a state must submit a plan that describes how its program meets require. and must receive approval of the plan from CMS. On Aug. 17, `07, CMS issued a letter purporting to clarify statutory and regulatory requirements related to preventing SCHIP substitution for other insur. coverage in states wishing to cover children with effective family incomes in excess of 250% of the fed. poverty level. This report considered whether the letter is a rule under the Congressional Review Act.

Available in PDF, EPUB and Kindle. Book excerpt: Congress created the State Children's Health Insurance Program (SCHIP) to reduce the number of uninsured children in low-income families that do not qualify for Medicaid. States have flexibility in structuring their SCHIP programs, and their income eligibility limits vary. Concerns have been raised that individuals might substitute SCHIP for private health insurance -- known as crowd-out. This report examines the Centers for Medicare and Medicaid Services' (CMS) and states' efforts to minimize crowd-out. The auditor examined: (1) CMS's guidance to states for minimizing crowd-out and assessment of whether it should be a concern; and (2) states' policies to minimize crowd-out and how they assess whether it should be a concern. Charts and tables.

Available in PDF, EPUB and Kindle. Book excerpt: The Children's Health Insurance Program was crafted in a period of intense partisan and ideological controversy over health care entitlements to provide -creditable coverage- for American children below 200 percent of the Federal Poverty Level. This objective was widely supported, though achieved only by a compromise between the structural alternatives of a block grant, similar to the Maternal and Child Health Block Grant or an entitlement resembling Medicaid. According to David G. Smith, the CHIP compromise has been a successful experiment that far exceeded expectations, both in identifying and enrolling -targeted low-income children- and in earning political capital. He argues that beyond this core mission, the reauthorization of CHIPRA (Children's Health Insurance Program Reauthorization Act of 2009) invites a larger mission: going beyond enrollment of children to include assuring access, improving quality, and containing costs of health care for them. Extending this thrust, the author notes that CHIP could be used to establish children's health as a niche--much like care for the elderly--within the larger scheme of health care insurance for all. Several areas of successful performance needed for the program to be adjudged a success as well as its limitations are discussed in the book. These areas include initial implementation, enrolling kids, federal-state relations, and the uses and misuses of waivers to modify the program. A description of changes made by the CHIPRA reauthorization and the new Patient Protection Affordable Care Act (PPACA) is also included. This is followed by a consideration of lessons learned from CHIP's evolution and recommendations for future development. In short, this is a valuable and readable account for those interested in the current and future trends of health care for the young.

Available in PDF, EPUB and Kindle. Book excerpt: Medicaid, one of the largest federal programs in the United States, gives grants to states to provide health insurance for over 60 million low-income Americans. As private health insurance benefits have relentlessly eroded, the program has played an increasingly important role. Yet Medicaid’s prominence in the health care arena has come as a surprise. Many astute observers of the Medicaid debate have long claimed that “a program for the poor is a poor program” prone to erosion because it serves a stigmatized, politically weak clientele. Means-tested programs for the poor are often politically unpopular, and there is pressure from fiscally conservative lawmakers to scale back the $350-billion-per-year program even as more and more Americans have come to rely on it. For their part, health reformers had long assumed that Medicaid would fade away as the country moved toward universal health insurance. Instead, Medicaid has proved remarkably durable, expanding and becoming a major pillar of America’s health insurance system. In Medicaid Politics, political scientist Frank J. Thompson examines the program’s profound evolution during the presidential administrations of Bill Clinton, George W. Bush, and Barack Obama and its pivotal role in the epic health reform law of 2010. This clear and accessible book details the specific forces embedded in American federalism that contributed so much to Medicaid’s growth and durability during this period. It also looks to the future outlining the political dynamics that could yield major program retrenchment.
