Due to the progress in the technology of CBCT, for orthodontic clinical diagnosis, treatment and follow-up, CBCT supply much more reliable information compared to conventional radiography. The most justified indications for the use of CBCT in orthodontics are the existence of impacted and transposed teeth. For the management of the impacted teeth, CBCT enhances the ability to localize these teeth accurately and to assess root resorption of adjacent teeth. Patients with craniofacial anomalies like cleft palate cases, the abnormalities of the temporomandibular joint contributing malocclusion, evaluation of airway morphology in obstructive sleep apnea cases, patients needing maxillary expansion or planning orthognathic surgery in severe skeletal discrepancies are also listed among the indications of using CBCT in orthodontics. CBCT is useful in identifying optimal site location for temporary skeletal anchorage device. The use of CBCT for the assessment of treatment outcomes and evaluation of cervical vertebral maturation are still controversial issues. It should be kept in mind that before using CBCT, justification and evaluation of risks and benefits are needed. In order to minimize the radiation dose, the exam should include only the areas of interest.

Book excerpt: Objectives: To explore the nature and frequency of incidental findings in large field of view cone-beam computed tomography (CBCT). Additionally, to assess the agreement among orthodontic clinicians in the assessment of the impact of maxillofacial findings identified in CBCT imaging. Methods: A total of 427 consecutive CBCT radiologic reports obtained for orthodontic purposes were retrospectively reviewed. All findings were categorized into six anatomic categories for descriptive purposes. Additionally, using a sub-sample of these findings, the agreement between 3 orthodontists was assessed with respect to need for further follow-up and potential impact of the findings on orthodontic treatment. Results: A total of 842 incidental findings were reported in the 427 CBCT scans (1.97 findings/scan). The most prevalent findings were those located in the airway (42.3%), followed by the paranasal sinuses (30.9%), dentoalveolar (14.7%), TMJ (6.4%), surrounding hard/soft tissues (4.0%), and cervical vertebrae (1.3%) regions. Non-odontogenic findings represented 718 of the 842 (85.3%) findings. In terms of agreement when assessing clinical significance of the findings, subjects demonstrated "fair-to-good" inter-rater agreement regarding CBCT findings in terms of the need for further follow-up and the potential impact on future orthodontic treatment. Subjects demonstrated "excellent" intra-rater agreement in the assessment of CBCT findings regarding both need for follow-up and potential impact on future orthodontic treatment. Conclusions: This study demonstrates the high occurrence of incidental findings in large field of view CBCT scans in a sample of orthodontically referred cases. The majority are extragnathic findings, which can be normally considered outside the regions of interest of many dental clinicians, but which may still require follow-up and/or management. Specifically, incidental findings in the nasal-oral-pharyngeal and paranasal air sinuses are the most frequent. Subjects demonstrated higher levels of agreement for dentoalveolar findings compared with all other extragnathic regions when assessing clinical significance.

Book excerpt: In an asymptomatic sample of adolescent orthodontic patients without any history or symptoms of TMD, CDCT was utilized in conjunction with a centric relation wax bit to assess condylar position and the effect one condylar discrepancy may have on the contralateral joint. Twenty-one patients between 11-16 years of age were divided into four groups based on degree of CPI discrepancy in the horizontal and vertical plane, and iCAT images taken with use of an anterior wax bite. Digital linear measurements were recorded similar to the method used by Ikeda for anterior (AS), superior (SS), and posterior (PS) joint spacing for all 21 patients. Joint spacing on one condyle was moderate to highly correlated with joint spacing on the contralateral condyle. There were no significant differences between condyles within the same patient in any of the 4 groups. There were no significant differences between groups except for the SS spacing between Group 3 and Group 4. Within the CPI range 0-2.5mm studied, the effect of CPI values in one or multiple joints had no statistically significant effect on joint position as measured by the CR technique described. Determination of optimal joint space must address technique for registration of bite, inclusion criteria of patients, and method of measurement. Joint spacing mean values for this adolescent orthodontic population are: AS = 2.06mm (+/- 55), SS = 2.67mm (+/7 79), PS = 1.88mm (+/- .51).
