The introduction of direct CT sections in horizontal and more recently in frontal or modified frontal planes, the use of reconstruction to indirectly obtain sagittal, parasagittal, and frontal CT images, and the visualization of the ventricular system, sulci, or cisterns with injection of metrizamide have led us to prepare this monograph. The full benefit of CT scanning can only be obtained from an accurate three-dimensional concept of anatomic structures of the brain including sulci, cisterns, ventricles, and deep nuclei. This may be achieved by studying in detail serial sections of the skull and brain in multidirectional planes. CT scanning, the single most important noninvasive diagnostic innovation in recent years, has widely changed the practice of neuroradiology. Indeed, neuroradiology remains a most fascinating field in the study of anatomy of the brain in vivo. The first part of this book is devoted to sagittal and parasagittal sections, the second part to frontal and modified frontal sections, and the final part to horizontal and modified horizontal sections of the skull and brain. Experience in neuroradiology has confirmed the outstanding value of computed tomography (CT) for the diagnosis of space-occupying lesions within the skull and orbit. It might be assumed, then, that the second edition of this book would simply represent a numerically expanded continua tion of the popular first edition. That is not the case, however. Advances in imaging techniques have promp ted the creation of a new book whose expanded title reflects its more comprehen sive nature. The added illustrations, the revised text, and the expanded circle of editors and contributors document this. Since publication of the first edition, a new modality, magnetic resonance imaging (MRI), has become an established neuroradiologic study. We felt it was essential to include this new modality in our book and explore its capabilities as an adjunct or alternative to CT scanning. Because of the high acquisition costs of MRI and the still small number of MR units currently in operation, we have relied in part on images furnished by other institutions and private practitioners, to whom we are indebted. Many problems relating to MR, both in terms of equipment and image interpretation, have yet to be resolved. There is no denying that we still have much to learn.

Book excerpt: The current book represents a distillation of the experience gained in diagnosis of intracranial tumors with computed X-ray tomography at the University Hos pitals of Berlin, Mainz, and Miinchen. To what purpose? Standard radiological techniques such as pneumoencephalography with lumbar puncture and cerebral arteriography with puncture of the common carotid artery are invasive proce dures which entail a certain amount of risk as well as discomfort for the patient. Furthermore, diagnoses made with these procedures rely primarily on indirect signs of an intracranial space-occupying lesion - such as displacement of the air-filled ventricles or of normal cerebral vessels. Only a few types of tumor are demonstrated directly with these techniques. In contrast, computed tomography demonstrates the pathology directly in almost all cases, and this with a minimum of risk and discomfort. In addition, normal intracranial structures are demonstrated, so that the tumor's effect on its surroundings can be evaluated. Today, almost a decade after HOUNSFIELD'S revolutionary invention, diagno sis of brain tumors without computed tomography is almost unthinkable, if not in fact irresponsible.

This unique position makes it particularly important for them to understand the basic mechanics, indications, and contraindications for the available orbital and neuroimaging studies (e.g., CT and MR imaging), as well as any special studies that may be necessary to fully evaluate the suspected pathology. It is equally important for them to be able to communicate their imaging questions and provide relevant clinical information to the interpreting radiologist. Since the publication of the original edition of this American Academy of Ophthalmology Monograph in 1992, new techniques and special sequences have improved our ability to detect pathology in the orbit and brain that are significant for the ophthalmologist. In this second edition of Monograph 6, Johnson, Policeni, Lee, and Smoker have updated the original content and summarized the recent neuroradiologic literature on the various modalities applicable to CT and MR imaging for ophthalmology. They emphasize vascular imaging advances (e.g., MR angiography (MRA), CT angiography (CTA), MR venography (MRV), and CT venography (CTV) and specific MR sequences (e.g., fat suppression, fluid attenuation inversion recovery (FLAIR), gradient recall echo imaging (GRE), diffusion weighted imaging (DWI), perfusion weighted imaging (PWI), and dynamic perfusion CT (PCT)). They have also included tables that outline the indications, best imaging recommendations for specific ophthalmic entities, and examples of specific radiographic pathology that illustrate the relevant entities. The goal of this Monograph is to reinforce the critical importance of accurate, complete, and timely communication--from the prescribing ophthalmologist to the interpreting radiologist--of the clinical findings, differential diagnosis, and presumed topographical location of the suspected lesion in order for the radiologist to perform the optimal imaging study, and ultimately, to receive the best interpretation.
