Book excerpt: Suicide is a national public health problem with 48,344 estimated United States (US) deaths in 2018, making it a top-10 leading cause of death. Veterans are 1.5 times more likely to die by suicide than the general population, after adjusting for age and sex. In 2018, Veterans represented 8% of the US adult population but accounted for 13.8% of suicide deaths. Thus the Department of Veterans Affairs (VA) has made suicide prevention a top priority. Many VA initiatives focus on identifying and treating Veterans determined to be at elevated risk for suicidal behaviors. These initiatives include maintaining a Veterans Crisis line as well as preventions programs through the Veterans Health Administration (VHA), such as the Recovery Engagement and Coordination for Health - Veterans Enhanced Treatment (REACHVET) program, Caring Contacts to Veterans, yearly screenings for suicide risk, and hiring Suicide Prevention Coordinators at each Medical Center. These VHA-specific initiatives may account for reduced suicide rates among Veterans who use VA health care compared with those who do not. However, two-thirds of Veterans do not use the VA for health care. Community-based approaches to suicide prevention outside of VA health care settings may provide opportunities to reach Veterans. The National Strategy for Suicide Prevention released by the Office of the Surgeon General, the National Action Alliance for Suicide Prevention, VA's National Suicide Prevention Strategy and the President's Roadmap to Empower Veterans and End a National Tragedy of Suicide (PREVENTS) Executive Order all call for a public health approach to suicide prevention. Population-based approaches targeting individuals across the spectrum of suicide risk may serve as adjunctive or complementary strategies to clinical interventions to help address this public health problem.

Subsequent epidemiologic studies conducted on military and veteran populations that served in the operations in Afghanistan and Iraq provided scientific evidence that those who fought were in fact being diagnosed with mental illnesses and experiencing mental healthâ€"related outcomesâ€"in particular, suicideâ€"at a higher rate than the general population. This report provides a comprehensive assessment of the quality, capacity, and access to mental health care services for veterans who served in the Armed Forces in Operation Enduring Freedom/Operation Iraqi Freedom/Operation New Dawn. It includes an analysis of not only the quality and capacity of mental health care services within the Department of Veterans Affairs, but also barriers faced by patients in utilizing those services.

Book excerpt: This systematic review updates evidence on the accuracy of methods to identify individuals at increased risk for suicide, and the efficacy/effectiveness and adverse effects of healthcare service interventions in reducing suicide and other suicidal self-directed violence. Important areas of ongoing research and current evidence gaps on suicide prevention are also addressed. This report includes studies relevant to healthcare services provided to Veterans and military personnel in the United States (US), and updates 3 previous Department of Veterans Affairs (VA) Evidence-based Synthesis Program (ESP) reviews on these topics. Veterans and military personnel represent 20% of all known suicides in the US. Rates of suicide increased during the wars in Afghanistan and Iraq, and between 2000 and 2010, the suicide rate among Veterans rose higher than the rate among civilians. Female Veterans are at especially high risk relative to other women. These trends have led to new initiatives within the VA and military to address suicide prevention. During the year prior to suicide, an estimated 77% of individuals make contact with primary care and 32% with mental health care clinicians, providing opportunities for suicide risk assessment and intervention. However, screening for suicide risk in general medical practice is not part of standard care in the US. Efforts to prevent suicide in individuals at high risk, such as those with recent suicide attempts, generally include treatment of underlying conditions and psychotherapy. In addition to individual-level approaches to suicide prevention, initiatives have been implemented at organizational, health system, and community levels. However, despite the existence of many types of services, very few studies demonstrating their efficacy and effectiveness have been published. As a result, their influence on suicide prevention remains unclear.

Book excerpt: Suicide is the tenth leading cause of death in the United States (U.S.), with nearly 100 suicides occurring each day and over 36,000 dying by suicide each year. Among Veterans and current military, suicide is a national public health concern. Recent estimates suggest current or former military represent 20 percent of all known suicides in the U.S. and the rate of suicides among Veterans utilizing Veterans Health Administration (VHA) services is estimated to be higher than the general population. The enormity of the problem has led to several major public health initiatives and a growth in research funding for suicide prevention. Despite recent suicide prevention efforts, the suicide rate in the U.S. has changed relatively little over the past 100 years. The body of research on suicide prevention approaches has been reviewed previously by Gaynes and colleagues, and Mann and colleagues, and recent, similar work exists in the form of draft self-harm guidelines from the National Institute for Health and Clinical Excellence (NICE) in 2011. As requested by the Veterans Affairs (VA)/Department of Defense (DoD) Evidence Based Practice Working Group (EBPWG) on suicide prevention, we examined recent research on suicidal self-directed violence as defined by Crosby et al. 2011. We update the work of Gaynes et al. and Mann et al. by systematically reviewing relevant literature that was not included in either report, and was published in 2005 through November 18, 2011. Though the focus of the report is on suicide prevention, we include as outcomes any type of suicidal self-directed violence, defined as “Behavior that is self-directed and deliberately results in injury or the potential for injury to oneself. There is evidence, whether implicit or explicit, of suicidal intent.” The key questions were: Key Question #1. What is the effectiveness of specific interventions for reducing rates of suicidal self-directed violence in military and/or Veteran populations? Key Question #2. What lessons can be learned from suicidal self-directed violence prevention intervention research conducted outside of Veteran or military settings that can be applied to Veteran and/or military populations? Key Question #3. What is the effectiveness of referral and follow-up services (e.g., strategies designed to provide referrals, improve referral follow-through and attendance, etc.) for reducing rates of suicidal self-directed violence in military and/or Veteran populations? Key Question #4. What lessons can be learned from research on suicidal self-directed violence referral and follow-up services conducted outside of Veteran or military settings that can be applied to Veteran and/or military populations?

It is well documented in the literature that there are high rates of psychological disorders among military personnel serving in Operation Enduring Freedom in Afghanistan and Operation Iraqi Freedom in Iraq as well as among the service members' families. For service members' families, the degree of hardship and negative consequences rises with the amount of the service members' exposure to traumatic or life-altering experiences. Adult and child members of the families of service members who experience wartime deployments have been found to be at increased risk for symptoms of psychological disorders and to be more likely to use mental health services. In an effort to provide early recognition and early intervention that meet the psychological health needs of service members and their families, DOD currently screens for many of these conditions at numerous points during the military life cycle, and it is implementing structural interventions that support the improved integration of military line personnel, non-medical caregivers, and clinicians, such as RESPECT-Mil (Re-engineering Systems of Primary Care Treatment in the Military), embedded mental health providers, and the Patient-Centered Medical Home. Preventing Psychological Disorders in Service Members and Their Families evaluates risk and protective factors in military and family populations and suggests that prevention strategies are needed at multiple levels - individual, interpersonal, institutional, community, and societal - in order to address the influence that these factors have on psychological health. This report reviews and critiques reintegration programs and prevention strategies for PTSD, depression, recovery support, and prevention of substance abuse, suicide, and interpersonal violence.

This monograph attempts to set out the basic framework for suicide prevention strategies. It provides details in formulating and evaluating suicide prevention programs. The public health approaches suggested offer a multilayer intervention model, which has been adopted by a number of developed countries for setting up national prevention strategies. Also included are details of some well-established suicide prevention programs worldwide and in the Western Pacific Region.
