Book excerpt: Différents travaux ont abordés la question de l'attitude des médecins envers leur propre santé mais on en sait peu sur celle des internes. Pourtant ils éprouvent déjà les mêmes difficultés : stress, charge de travail...et ont eux-aussi la possibilité de s'autoprescrire. Une étude descriptive a été réalisée à l'aide d'un questionnaire auprès de tous les internes de médecine générale de la faculté de Poitiers inscrits en 2012/2013. 128 internes ont répondu à l'enquête. 14,1% fument du tabac, 5,5% consomment de l'alcool régulièrement. 93,3% des femmes ont déjà réalisé un frottis de dépistage du cancer du col de l'utérus. Pour 15,6% d'entre elles celui-ci remonte à de plus de 3 ans. La moitié des internes pratique une activité physique régulière et seulement 12,5% ont un IMC supérieur à la normal (23,7% des hommes et 7,8% des femmes). Même s'ils sont nombreux à avoir déclaré un médecin traitant, ils ne le consultent que très rarement. Ils s'estiment majoritairement en bonne santé mais près de 30% pensent que leur prise en charge médicale est moins bonne depuis qu'ils sont internes. Les principaux facteurs incriminés sont le manque de temps et la mobilité géographique au cours de l'internat. En matière de prévention, les comportements sont semblables voire meilleurs que ceux de la population générale par contre certains éprouvent déjà des difficultés à consulter et ont recours à l'auto-prescription, l'autodiagnostic et aux consultations informelles. Le Conseil de l'Ordre alerte sur la tendance des médecins à appréhender seuls leur propre santé. Ces comportements semblent débuter dès l'internat. Il serait donc important de sensibiliser les internes avant même leur prise de fonction aux dangers de l'autoanalyse et l'auto-prescription et leur apprendre à être tout simplement des patients.

Book excerpt: Les internes en médecine générale ont une double activité d'étudiants et d'agents de la fonction publique. Nous avons cherché à savoir si cette double charge de travail avait des répercussions sur leur prise ên charge de leur propre santé. Pour cela, en janvier 2015, un questionnaire a été distribué aux 176 internes civils de médecine générale de la faculté de Paris-Sud, au Kremlin Bicêtre (France). Les questions portaient notamment sur leurs habitus, leur parcours de soin,- le suivi médical, les modalités de prise en charge de leur santé, l'impact du stress et la prise de psychotropes. Le traitement des données a été effectué via Excel 2007. Les résultats qui portent sur les 124 répondants (parmi 176 internes des promotions de 1ère, 2ème et 3ème années) montrent que : 50% des internes avaient un médecin traitant déclaré mais celui-ci n'était jamais consulté par les hommes en cas de problème de santé et seulement par 23% des femmes ;le recours à l'auto-prescription était très fréquent ; la consommation de tabac, d'alcool et de cannabis croissait avec les années d'internat (atteignant respectivement 15%, 6% et 10% en T3) ; les rapports sexuels non protégés étaient le fait de 60% de l'échantillon et 48% des internes femmes avaient déjà eu recours à la contraception d'urgence ; la qualité du sommeil était ressentie comme mauvaise, notamment en premier (45%) et dernier semestre (20%), les internes ayant alors d'avantage recours aux hypnotiques (31%); enfin l'état psychologique se fragilisait tout au long du cursus de l'interne. Le manque de temps est invoqué comme le principal obstacle à la prise en charge de la santé, lié à une surcharge objective d'heures de travail comme agent de la fonction publique et à l'ampleur du travail universitaire demandé. Ce constat appelle des préconisations de prévention, parmi lesquelles une visite médicale obligatoire effectivement réalisée, une application stricte de la loi en regard des horaires de travail, la mise en place d'un groupe de soutien psychologique.

L'objectif principal de cette étude est d'analyser le comportement et le ressenti des internes de médecine générale concernant la prise en charge de leur propre santé. L'objectif secondaire de cette étude est de recueillir au besoin leurs propositions d'amélioration en termes de prévention, de formation et de soins. Matériels et méthodes : une étude qualitative a été menée par des entretiens individuels semi-dirigés auprès d'internes de médecine générale. Une analyse thématique par triangulation via le logiciel Nvivo 11a a ensuite été réalisée. Résultats : 12 internes de médecine générale (8 femmes et 4 hommes) ont participé à cette étude. Bien que leur accès aux soins soit facilité par leur statut, les internes de médecine générale étaient souvent les acteurs exclusifs de leur prise en charge médicale. Ils avaient un ressenti de bonne santé mais reconnaissaient négliger leur suivi et pratiquaient l'autoprescription régulièrement. Leurs comportements étaient similaires à leurs ainés et ce dès le début de leur formation comme si ces conduites étaient encrées dans la culture médicale. Leurs rapports à la maladie et à leurs confrères jouaient un rôle important dans leur manière de se soigner et les différenciaient du « patient lambda » auquel ils aimeraient pourtant pouvoir s'identifier. Certains internes de médecine générale exprimaient une fragilité psychologique et étaient en demande de soutien de la part de leurs ainés. Conclusion : l'interne de médecine générale doit être mieux formé à la gestion de sa santé mais également mieux accompagné, soutenu et sensibilisé aux dangers de l'autosuffisance.

The evidence instead presents an intersectoral agenda on the pre-condition of equitable access to health workers in the attainment of universal health coverage, along with a dynamic labour market understanding of the substantive impact on education, employment, jobs and innovation in the health and social care economy. The Global Strategy, therefore, enables governments and other relevant stakeholders to adopt a holistic, rather than fragmented, approach to ensuring that the health workforce contributes both to improved health and to broader socioeconomic development.
