Book excerpt: Any discussion of the present success in management of urological cancers evokes a mixed response. Oncologists and urologists can enjoy the success with chemotherapy for testicular cancers but cannot forget the dismal results with any form of treatment, other than surgery, for renal carcinoma. But these are the less frequent urolegi cal tumours: what are the attitudes to the more common prostate and bladder cancers. Intensive study, many clinical trials and much debate lead us to the conclusion that we understand them better, we can tailor the treatment more appropriately to the individual patient but there remains some uncertainty as to the overall success that we have achieved. There have been no striking changes in the 5-year survival data. Clinicians tend to see their success in terms of their special interest. Radiotherapists point to their success in stage-reduction but what are we to do with the many patients whose tumour is unaltered by radiotherapy. Urological surgeons, and especially those who are still influenced by the shadow of Halsted, point to their success in excising the cancer but apart from that highly selected group, what are we to do for the very large number of patients for whom surgery is inappro priate. Bystanders can only watch and listen to the arguments for and against these views.

This book was released on 2012-12-06 with total page 181 pages. Available in PDF, EPUB and Kindle. Book excerpt: Chemotherapy for malignant disease has brought about many rapid and often spectacular improvements in the survival rate of some groups of patients. Yet enthusiasm for these successes has, in part, been offset by the problems in evaluating responses to treatment and by the disappointment of failing to check the progress of still so many other tumours. These reactions will be no surprise to the medical historian but perhaps the expectations from scientific progress nowadays sometimes demand more than can reasonably be expected. Another expectation is that any review of chemotherapy is com pletely up to date, even prophetic. Such is the expansion of the subject that new drugs and trial results are continually being reported but it is this very mass of information that creates its own problems and makes many clinicians despair of finding a balanced judgement on all of this information. This was the challenge accepted by Professor Spiers. He then gathered together a group of colleagues who are amongst the acknowledged leaders in the field of chemotherapy for urological tumours, all of whom have made important contributions to this subject. However, each chapter is not merely a record of the author's experience but encompasses an assessment of past and present practice as well as perspectives in diagnosis and management. Almost all chapters include reference to published work up to and including 1981.

Book excerpt: Urologists and medical oncologists have witnessed a rapid growth in systemic therapeutic options for treating genitourinary malignancies that increasingly integrates radiation therapy to primary cancers, nodal beds, and even metastatic sites. The culmination of these advances has been the creation of multidisciplinary teams that expertly provide comprehensive care to patients with urologic cancers. This book provides the framework to create such a multidisciplinary clinical team focused on the treatment of urologic malignancies with representation from urologists, medical oncologists, and additional specialists who work together to provide optimal team-based care. The book integrates advanced systemic therapeutics including immune-based and targeted therapies. Readers gain a better understanding of the benefit of multidisciplinary co-management through specific examples such as hormone sensitive metastatic prostate cancer, neoadjuvant chemotherapy for muscle invasive bladder cancer, and immunotherapy approaches to advanced bladder and kidney cancer. The book also discusses the integration of genomic tumor characterization and personalized medicine into surgical planning, tumor biopsies, and chemo-immunotherapy selection, emphasizing the expansion of new therapeutics for urologic malignancies and the changing definitions of disease progression. Urologic Oncology: Multidisciplinary Care for Patients is a comprehensive resource for urologists, medical oncologists, radiation oncologists, genetic counselors, fellows in urologic oncology and medical oncology, residents in urology and radiation oncology, advanced practice providers, practice managers, and pharmaceutical representatives.

Available in PDF, EPUB and Kindle. Book excerpt: The future of urology is changing from one based around surgery, to an outpatient system, concentrating mainly on medical mana- ment. Last year, in the UK, there were almost two million hospital consultations for urological symptoms and an estimated fve million consultations in general practice. With such a signifcant proportion of care being delivered for urology, 80% of which can be managed medically, we felt that a resource should be made available to providers that will serve as a quick and useful guide to the medical management of urological disease. The aim was to provide a fresh, practical, and concise overview of the key medical management issues every practicing clinician faces in an outpatient urology setting, on a daily basis. With these goals in mind, each chapter starts with a brief overview of the relevant anatomy/physiology and pathology of the condition in question, and then goes on to give an explanation of the drugs used, including mode of action, doses, side effects, and important interactions/contraindications. Finally, there is a brief but relevant section on review of medical literature and a clinical section on when a particular drug treatment should be used. A list of key points can be found at the end of each section.

Available in PDF, EPUB and Kindle. Book excerpt: Cancer forms a large part of urological practice. Prostate cancer is the most significant and is the second most common cause of male cancer deaths with some 300,000 cases in the USA annually. Cancer of the bladder is also common. While treatment has been primarily surgical, recent years have seen an explosion of new drug therapies for urological cancers. All urologists, whether in training or in practice, need an up-to-date survey of the literature and key findings in this important and controversial field. This book takes each organ in turn (prostate, kidney, bladder, testis and penis) and tackles the controversies head-on by inviting chapters on various (often rival) treatments for each disease. Each section is then concluded by a commentary from the editor which draws together the various strands, outlines the principal areas of consensus and disagreement. Jonathan Waxman is an oncologist who specialises in urological tumours and has developed new treatments for prostate, bladder and kidney cancer and for terminal care, which are now part of standard practice. He founded the Prostate Cancer Charity and helped establish an all-party parliamentary group to improve cancer treatment and research throughout the UK.
