Book excerpt: Context: Recommended treatment of exertional heat stroke (EHS) includes whole body cold-water immersion (CWI); however, remote locations, spatial or monetary restrictions challenge CWI feasibility. Thus, the development of a modified, portable CWI method would allow for optimal treatment of EHS when restrictions apply. Objective: Determine cooling efficacy of modified CWI (tarp assisted cooling with oscillation; TACO) following exertional hyperthermia. Design: Randomized, crossover controlled trial. Setting: Environmental chamber (33.4 ± 0.8°C, 55.7 ± 1.9% relative humidity). Patients or Other Participants: Sixteen (9 males, 7 females) volunteers (26 ± 4.7y, 1.76 ± 0.09m, 72.5 ± 9.0kg, 20.7 ± 7.1%body fat) with no history of compromised thermoregulation participated. Interventions: Participants completed volitional exercise (cycling or treadmill) until a rectal temperature (Tre) {u2265}39.0°C. Following exercise, participants transitioned to a semi-recumbent position on a tarp until Tre reached 38.1°C or until 15 minutes elapsed during both control (no immersion; CON) and TACO (immersion in 151L of 2.1 ± 0.8°C water). Main Outcome Measures: Tre, heart rate (HR), and blood pressure (reported as mean arterial pressure, MAP), were assessed pre- and post-cooling. Statistical analyses included repeated measures ANOVA with appropriate post-hoc t-tests and Bonferroni correction. Results: Tre prior to cooling was not different between conditions (CON: 39.27 ± 0.26°C, CWI: 39.30 ± 0.39°C; P=0.62; ES=-0.09) whilst post-cooling Tre was decreased in TACO (38.10 ± 0.16°C) compared to CON (38.74 ± 0.38°C, t15=-8.84;P.

Book excerpt: Victims of exertional heat stroke (EHS) in whom treatment is delayed have higher rates of multi-organ failure and a greater number of fatalities. Death related to EHS is preventable, through immediate treatment via cold-water immersion (CWI). To date little is known about the influence of treatment delays on core cooling following EHS. Thus we sought to examine the effects of treatment delays on cardiovascular and thermal responses prior to, during, and following CWI treatment in individuals with exercise-induced hyperthermia. Our findings demonstrate that treatment delays resulted in a sustained level of hyperthermia and cardiovascular strain that significantly increased the time an individual is at risk to the potential lethal effects of EHS. Moreover, we report that cold water immersion treatment is powerful enough to overcome the adverse effects of treatment delays and rapidly reduce core temperatures while facilitating the re-establishment of blood pressure towards normal resting levels.

Individuals with a High AD/LBM (315 cm2/kg) had a 1̃.7-fold greater overall rectal cooling rate relative to those with Low AD/LBM (275 cm2/kg) during both CWI and TWI. Further, overall rectal cooling rates during CWI were 2̃.7-fold greater than during TWI for both the High and Low AD/LBM groups. Study findings show that AD/LBM must be considered when determining the duration of the immersion period. However, CWI provides the most effective cooling treatment for EHS patients irrespective of physical differences between individuals.

Book excerpt: Hot environmental conditions can reduce work productivity/exercise performance and increase the incidence of exertional heat illness. In preparation for hot environments, individuals commonly complete heat acclimation protocols that initiate physiological and perceptual adaptations to reduce thermal strain and improve physical capabilities. However,conventional exercise-heat acclimation interventions can be costly and impractical, as they require access to an artificial hot environment and often require control of core temperature during exercise. As such, there is a demand for a flexible heat acclimation intervention that can be easily incorporated into the daily training of athletes and military/occupationalpersonnel. Therefore, the aim of this thesis was to develop a novel and practical post-exercisehot water immersion heat acclimation protocol.Firstly, 6-days of submaximal exercise in temperate conditions followed by a hot water immersion induced hallmark heat acclimation adaptations during submaximal exercise intemperate and hot conditions and improved endurance exercise performance in the heat (Chapter 4). The initiated adaptations were also not specific to the clock-time of when heat exposures occurred (Chapter 5). Post-exercise hot water immersion also initiated hallmark heat acclimation adaptations in both endurance trained and recreationally active individuals (Chapter 6). In addition, the decay of the induced adaptations following post-exercise hotwater immersion is slow, with no observable loss of heat acclimation two weeks following the cessation of the protocol (Chapter 7).The studies presented in this thesis demonstrate that the novel post-exercise hot waterimmersion intervention provides heat acclimation and reduces thermal strain during exercisein the heat. Future research is required to optimise this technique to improve its incorporation into different military/occupational or athlete scenarios and assess the impact of the intervention on the incidence of exertional heat illness.
