Book excerpt: Most symposia on chronic inflammatory bowel disease during the last few years have focused on new aspects of aetiology and pathophysiology. However, based on such new aspects, changes in diagnosis and treatment have been developed over the last few years and some others are currently under investigation. This book, the proceedings of Falk Symposium 97, `Clinical Challenges in Inflammatory Bowel Diseases - Diagnosis, Prognosis and Treatment', held in Lugano, Switzerland, 18-19 April 1997, summarizes these changes and new developments, discusses their value and defines further new approaches. The topics include primary diagnosis, definition of different patient groups, special problems of these patients in daily life and very new therapeutic principles. This volume opens new possibilities of diagnosis and treatment of inflammatory bowel disease to physicians and provides stimulation for further development.

Available in PDF, EPUB and Kindle. Book excerpt: Physicians treating patients with Inflammatory Bowel Disease (IBD) often face challenges due to the disease itself, associated conditions, and systemic effects. Recently, new medications have been introduced that allow for more aggressive and effective treatment of IBD. With these medications come the risks of adverse side effects, and physicians must vigilantly monitor for any complications. Drs. Miguel D. Regueiro and Jason Swoger have created a unique, user-friendly book that describes how to prevent, recognize, and manage complications encountered in the care of patients with IBD. Clinical Challenges and Complications of IBD aims to provide practical and clinically oriented information to assist physicians with commonly encountered complications of the natural history of IBD, extraintestinal manifestations of IBD, general health and metabolic complications of IBD, and complications arising from both the medical and surgical treatments of IBD. Formatted to allow for quick access of the information, Clinical Challenges and Complications of IBD is organized into sections that allow physicians to both identify potential pitfalls in treatment, as well as focus on health maintenance and specific challenges, including pregnancy, nutrition, and psychological issues. Clinical pearls will be emphasized and each chapter will provide a bulleted summary, along with suggestions for future reading. Clinical Challenges and Complications of IBD is an invaluable guide for gastroenterologists, nurse practitioners, physician assistants, and gastroenterology trainees as it is a one-of-a-kind resource for recognizing and dealing with the challenges and complications in patients with IBD.

Available in PDF, EPUB and Kindle. Book excerpt: The inflammatory bowel diseases, of unknown etiology and for which there are no cures, continue to attract the attention and interest of gastroenterologists, internists and surgeons. International symposia are common and it is safe to say that there is at least one major symposium held somewhere in the world each year. This book encompasses the proceedings of two recent symposia held in Victoria, British Columbia, Canada. The symposia were the fifth and sixth international meetings focused on inflammatory bowel disease in Canada in the last eight years. Once again they were sponsored by Axcan Pharma, Inc. (formerly Interfalk Canada, Inc.) and endorsed by the Canadian Association of Gastroenterology. As has become traditional at such meetings the faculty was drawn from an international roster ofleaders in the field of inflammatory bowel disease and gastroenterology. The chapters of the proceedings provide a timely, up-to-date review of the major issues, including those within the realm of basic science and others dealing with clinical problems. The first symposium, 'Basic Research and Clinical Implications', was co-ordinated by John Wallace in association with Stephen Collins and Stephan Targan. The themes of this section were organized under the general topics of predisposing factors (genetics, animal models, infection, permeability, and immune deficits) and the pathophysiology of intestinal inflammation. The second symposium, Trends in Therapy' was organized by Lloyd Sutherland along with Franc;:ois Martin, Robin McLeod and Noel Williams.

Book excerpt: Ulcerative colitis and Crohn's disease remain a great therapeutic challenge to the medical community. In recent years knowledge about the pathogenesis of these diseases has progressed rapidly but the cause of the diseases remains completely unknown. It has become clear that dysregulation of the mucosal immune system is the basis for the chronic evolution of the diseases in a genetically susceptible population. Exciting new therapeutic approaches have been attempted in the last couple of years and cytokine and anti-cytokine treatments in particular seem very promising, especially in intractable disease. The format of the Falk Symposium 106 on `Advances in Inflammatory Bowel Diseases', held in Brussels, Belgium, June 18-20, 1998, was somewhat innovative as each session attempted to link the new insights into pathogenetic mechanisms with new therapeutic approaches, resulting in optimal information transfer. The classic therapeutic schemes were updated with a special focus on step-wise build-up of therapy.

Available in PDF, EPUB and Kindle. Book excerpt: Inflammatory bowel disease (IBD) encompasses a spectrum of autoimmmune diseases which include ulcerative colitis, Crohn’s disease, and indeterminate colitis. Ulcerative colitis is a relapsing nontransmural inflammatory disease restricted to colon, which may be classified as proctitis, left-sided colitis, or pancolitis depending on the extent of involvement. Some patients also develop ileal inflammation (backwash ileitis), which occasionally complicates its differentiation from Crohn’s ileocolitis. Crohn’s disease is defined as a relapsing transmural inflammatory disease of gastrointestinal mucosa that may affect the entire gastrointestinal tract (GIT) from the mouth to the anus. There is discontinuous involvement of various portions of the GIT and may get complicated by formation of strictures, abscesses, or fistulas. It closely resembles intestinal tuberculosis both clinically and on investigations. GI tuberculosis is often suspected on the basis of its clinical, radiological, and endoscopic features, but histological or microbiologic proof of the disease is often difficult to achieve. Therefore, the majority of patients are treated with antitubercular drugs on presumptive diagnosis only. Similar clinical, endoscopic, radiological, and histological features of GI tuberculosis and Crohn’s disease pose a serious challenge to the treating physician. The rate of misdiagnosis of these conditions using conventional diagnostic measures has been reported to be around 50–70%. Thus differentiation and diagnosis of these conditions is difficult but very important for correct treatment of the patients. Recently some serological tests have come up for diagnosing Crohn’s disease and ulcerative colitis, which may help to distinguish these disorders from each other and from GI tuberculosis. Besides diagnosis, treatment for IBD is also an emerging field in which active research is going on. The traditional drugs used in treatment of IBD include steroids and aminosalicylates. Recent studies have evaluated the role of azathioprine, methotrexate, and cyclosporine. Apart from improving efficacy of therapy in short term, these agents have raised debate on complications of therapy, long-term efficacy, and cost. Management of IBD during pregnancy and fistulising Crohn’s disease are other areas of concern.

Available in PDF, EPUB and Kindle. Book excerpt: One of the most vivid memories from my medical school training was seeing my first surgical operation on a patient with Crohn’s disease. The senior surgeon at Mount Sinai Hospital in New York City, the same institution at which Burrill Crohn, Leon Ginzburg, and Gordon Oppenheimer had first described the disease “terminal ileitis,” had - doubtedly done countless operations on patients with inflammatory bowel disease in the past. Yet as we both gazed down into the patient’s open abdomen, at the “creeping fat” that seemed to be wrapping its sticky fingers around the young man’s intestines, he stated, “this is the m- tery of Crohn’s disease—no two patients are ever the same. ” What is it about the inflammatory bowel diseases, Crohn’s disease, and ulcerative colitis, that we find so intriguing? Is it the young age of the patients, many who are younger than even the medical students - tending to them? Or is it the elusive etiology, the theory of a “mystery organism” that has yet to be identified? Perhaps it is the familial pattern of disease, where many patients have relatives with similar diseases, yet in some instances only one of a pair of identical twins is affected. Regardless of the cause, these chronic diseases with a typically early age of onset, result in a long-term commitment of the patient, their fa- lies, friends, health care providers, researchers, employers, and even health care insurers and other health-related industries.

Available in PDF, EPUB and Kindle. Book excerpt: In the Western world around 360 in every 100,000 individuals have inflammatory bowel disease (IBD), a relapsing-remitting autoimmune disease that affects the gastrointestinal tract. Its impact on individual functioning across physical and psychosocial domains is significant and psychological distress is a common feature, with research suggesting that active IBD is associated with one of the highest rates of depression and anxiety of all chronic illnesses. Despite the high prevalence of mental health co-morbidities in IBD, psychological illness remains largely undertreated, with studies showing that 60% of IBD patients experiencing mental health problems do not receive adequate help. In this book, Knowles and Mikocka-Walus bring together world experts who practice integrated and holistic approach in their care for IBD patients, to provide an overview of research across a range of topics associated with the biopsychosocial treatment of IBD. Each chapter provides an up-to-date comprehensive consolidation and evaluation of the current literature alongside recommendations for practice. Key themes include: current understanding of the interrelationship of the neurological and biological aspects of IBD common concerns and issues individuals with IBD face exploring challenges across individual life-stages current evidence for psychosocial interventions recommendations for future directions of biopsychosocial work. Psychological Aspects of Inflammatory Bowel Disease: A biopsychosocial approach is a key resource for researchers, practitioners and academics considering psychosocial aspects of the disease and psychological interventions. It will also appeal to health psychologists and mental health practitioners working with clients with IBD, as well as gastroenterologists interested in a comprehensive and holistic approach to IBD management.
