Book excerpt: Research has consistently shown that therapy is effective and that clients experience positive outcomes from engaging in therapy (Orlinsky et al., 2004). However, past research has shown that many clients prematurely terminate therapy often with negative outcomes (Archer, Forbes, Metcalfe, & Winter, 2000; Carpenter, Morrow, del Gaudio, & Ritzler, 1981; Pekarick, 1985). The purpose of this study was to explore an intervention designed to reduce premature termination rates using a comprehensive,standardized role induction procedure and a more sensitive definition of premature termination. Further, the impact of the role induction as it relates to readiness for change was explored. This study also examined the impact the role induction would have on clients' commitment to therapy. A total of 106 adult clients seeking services at a community mental health center participated in the study. Participants were randomly assigned to either the experimental or control group. Prior to the intake session, each participant viewed a video. The experimental group viewed the comprehensive role induction video and the control group viewed a video explaining the services at the counseling center. Next, all participants completed the self-report measures of commitment to therapy (i.e. EAC-B; Tinsley et al., 1980) and readiness for change (i.e. SOC; McConnaughy, Prochaska, & Velicer, 1983). The present data failed to show that a role induction to counseling significantly impacts premature termination, commitment to counseling or readiness for change among clients seeking counseling at a rural community mental health center. Overall, the data failed to support previous research findings that seemed to indicate a role induction to counseling could reduce the occurrence of premature termination and increase a client's commitment to therapy (Reis & Brown, 2006; Walitzer, Dermen, & Connors, 1999).

Book excerpt: Given the high prevalence of client unilateral termination from psychotherapeutic services, elucidating client reasons for ending therapy is an important activity for researchers. Three studies were designed to shed light on reasons for both premature and appropriate termination from the perspective of adult clients and therapists: 1) In Study 1, I examined data from the Canadian Community Health Survey, Cycle 1.2, to establish base rates of client reasons for psychotherapy termination in Canada, along with their demographic and clinical correlates, 2) In Study 2, I used training clinic data to examine client and therapist perspectives of reasons for termination, working alliance, and barriers to treatment participation in mutual versus unilateral terminators, and 3) In Study 3, I collected data from Canadian clinical psychologists to examine their perspectives of client reasons for early versus later termination, and their use of engagement strategies to reduce client-initiated unilateral termination. In Study 1, 43.1 percent of respondents reported terminating therapy for reasons other than feeling better or completing treatment. In general, individuals with low income and diagnosable mental disorders had significantly increased odds of premature termination. Study 2 revealed that when clients made unilateral decisions to end therapy, therapists were only partially aware of either the extent of clients' perceptions of their success in therapy or with their dissatisfaction with therapy. Although working alliance and barriers to treatment participation were rated as lower in the context of unilateral termination by both clients and therapists than in the context of mutual decisions to terminate therapy, all clients, in general, rated the early alliance and barriers to treatment as higher than did their therapists. In Study 3 psychologists assigned differential importance to reasons for termination depending on whether termination was before versus after the third session. Theoretical orientation (CBT versus other) did not influence views of reasons for termination, but influenced use of some engagement strategies. Results are discussed in terms of research and clinical implications.

Book excerpt: Premature termination is a pervasive barrier to effective implementation of outpatient psychotherapy that frequently results in decreased treatment gains for clients and lowered morale for therapists. Unfortunately, despite its high prevalence and cost, premature termination remains poorly understood. The current study addressed some gaps in the literature using a national online survey design that permitted investigation of a broader range of potential predictors, exploration of more specific reasons for premature termination, and examination of longer term treatment outcomes than has been possible in most previous research. Participants were 278 workers from Amazon.com's Mechanical Turk, an online labor market regularly used for social science research. Participants completed an online survey about their treatment history, their most recent outpatient therapy experience and therapist, termination status, reasons for terminating prematurely (if applicable), treatment satisfaction, therapeutic outcome, and demographics. Over half of the participants reported prematurely terminating their most recent episode of therapy. Results revealed that premature termination of previous therapy episodes, a weak therapeutic alliance, and primary or comorbid depression were the best predictors of premature termination. These predictors were highly accurate in distinguishing premature terminators from treatment completers. Results indicated that being a woman, identifying as non-heterosexual, seeking treatment from a hospital outpatient psychiatric clinic, and having a therapist low in perceived multicultural competence were also associated with increased risk of premature termination. However, these predictors of premature termination did not remain significant when controlling for other variables. The three most common reasons for premature termination were environmental obstacles, dissatisfaction with services, and lack of motivation for therapy. Finally, with respect to therapeutic outcomes, treatment completers reported greater problem improvement, greater satisfaction with therapy, and less current functional impairment than premature terminators. However, contrary to expectations, no differences in outcomes were found between early premature terminators (five or fewer sessions) and late premature terminators (at least six sessions). Clinical implications, limitations of the study, and recommendations for future research are also discussed.
