Book excerpt: Studies on the effect of recombinant human interferon (rIFN) on the growth of colonies of multi potential hematopoietic progenitors have demonstrated that IFN inhibits the growth of these cells in culture. The inhibitory effect of IFN was more pronounced in multilineage colony forming cells (CFU-GEMMT) of marrow cells of patients with chronic myelogenous leukemia (CML) compared with stern cells of normal marrow. T cell colony formation (CFU-TL) of phi_positive T cells obtained from primary multilineage hematopoietic colonies was inhibited by rIFN. Inhibition of T cell colony formation of Phi T cells was more profound by IFN compared with secondary T cells of healthy individuals. Southern blot analysis of secondary T cells from normal individuals revealed a germ line pattern when examined for T cell receptor (TcR) gene re arrangements using the cDNA of the beta chain. Subcloned Phi T cells of one patient with CML demonstrated gene rearrangements of the TcR. We also demonstrated the disappearance of phi_positive multilineage hematopoietic colonies when marrow cells were preincubated with IFN and subsequently cultured in the presence of IFN. The cytoconversion of the Phi chromosome in metaphases obtained from individually analyzed multilineage hematopoietic colonies suggests that IFN might allow the restoration of nonclonal hematopoiesis in patients with CML. Book excerpt: This book provides state-of-the-art reviews of key issues and recent developments relating to chronic myeloid leukemia (CML), acquainting the reader with advances in research, treatment, and promotion of public health. Among the management topics addressed are the choices, advantages, and pitfalls of first-, second-, and third-line treatments; the European LeukemiaNet recommendations; management of adverse effects of tyrosine kinase inhibitors (TKI); management of CML blast crisis; management of pregnancy in the context of CML; the role of hematopoietic cell transplantation; the current experience with TKI discontinuation; and the value of interferon α in improving the outcome of TKI treatment. Various aspects of relevance to treatment outcome are discussed, including prognostic scores, molecular monitoring (principles and interlaboratory standardization), and response-related predictors of survival. Furthermore, updates are provided on the increasing prevalence of CML and its implications and on the changing cost of care for CML, taking into account the forthcoming impact of availability of generic imatinib.

Available in PDF, EPUB and Kindle. Book excerpt: Current problems in research and treatment of leukemias as discussed at the symposium of the International Society of Haematology on leukemias in Dresden (Germany) are dealt with in detail in this volume. The characterization of leukemic cells and the evaluation of their function are themes, with the first hints emerging of the possibility of relating them to the intensity of treatment required. The cytokines, responsible for cooperation between cells, are also of great importance, and the beginnings of therapeutic applications can be discerned here. Not only are the cytokines themselves very interesting, but also the application of cells producing cytokines according to the range of macrophages found. The emphasis on cell-to-cell relationship is thus a main topic of the book. Of course, other treatment such as bone marrow transplantation and interferon therapy play an important part, too, and the latest results of chemotherapy are reported. A further essential area covered is the diagnosis and therapy of chronic leukemic diseases, the inclusion of which suitably rounds off the book. I am very grateful to the out standing specialists from both the West and the East who contributed to the symposium and this book: an important sign of collaboration and integration for the future.

This book was released on 2021-01-26 with total page 54 pages. Available in PDF, EPUB and Kindle. Book excerpt: The tyrosine kinase inhibitor (TKI) imatinib was the first treatment to specifically target cancer cells, rather than the relatively indiscriminate effects of conventional chemotherapy on any rapidly dividing cells. This concept of targeted treatment in cancer is one of the important advances in modern medicine in the last 30 years. Indeed, treatment with TKIs has transformed chronic myeloid leukemia (CML) from a cancer with a poor prognosis to one in which many patients can expect a normal lifespan. Success with the TKIs has prompted the question of whether it is desirable – or feasible – for patients to remain on treatment for long periods. While the TKIs are targeted, they are associated with considerable toxicity, and long-term treatment has important economic implications for health services and patients. Thus, the concept of treatment-free remission (TFR) has emerged for patients in deep clinical remission. Clinical research over the last decade has focused on whether treatment can be stopped, how to best monitor patients while off treatment, and how to intervene before a clinical relapse. As this research progresses, the tantalizing prospect of a cure for some patients seems increasingly feasible. This new Fast Facts title outlines this trail-blazing approach to the long-term management of patients living with CML in remission. It explains the concepts of molecular and hematologic relapse, the highly sensitive technologies that allow disease monitoring, and how TFR is best managed in practice. It is a concise educational resource, ideal for any healthcare professional involved in the treatment of patients with CML who wants to understand TFR, particularly clinical nurse specialists and pharmacists who increasingly help clinicians to run CML clinics. Table of Contents: • The concept of treatment-free remission • Measurement of disease burden • Clinical practice • Future directions

Book excerpt: This concise, clinically focused pocket handbook assembles and synthesizes the latest developments and trends in the diagnosis and treatment of CML and provides an authoritative and convenient summary of the latest progress in TKI trials, the molecular monitoring of CML responses, and the development of new therapies to overcome resistance and improve patient care. Chronic myeloid leukemia (CML) is a rare type of leukemia (1–2 per 100,000 people) but is the most common chronic myeloproliferative neoplasm. CML remains a key model for the improved understanding of the pathophysiology of a malignancy at a molecular level; CML was the first cancer to be associated with a recurring chromosome abnormality, which generates the Philadelphia (Ph) chromosome and its associated fusion gene BCR-ABL1. The clinical outcome for patients with CML has changed dramatically in the past 15 years and this has been due to the development of tyrosine kinase inhibitors (TKIs), compounds that inhibit the activity of the oncogenic BCR-ABL1 protein. A number of first-, second- and third-generation TKIs are now available for the treatment of CML, although a number of treatment challenges remain, not least the development of treatment-resistant CML. Parallel to the development of specific drugs for treating CML, major advances have been made in the field of disease monitoring and standardization of response criteria.

Book excerpt: Interferons were among the first cytokines to be studied clinically in recombinant form in the 1980s, beginning a new era of pharmacotherapy in internal medicine. Interferons were soon employed in the treatment of tumors and it was shown that those hematopoietic neoplasias which had been difficult to treat with chemotherapy responded well to interferon treatment. In the present volume, renowned international authors update the importance of interferons in various fields of internal medicine, dermatology, and urology. The book also contains a general introduction into the classification, structure, and mode of action of interferons. Further chapters discuss the range of side effects of these substances and point out possible focuses of further clinical interferon research.

Book excerpt: Molecular Pathogenesis and Treatment of Chronic Myelogenous Leukemia provides a comprehensive understanding of the recent molecular genetics of Chronic Myelogenous Leukemia (CML) and a characterization of the molecular targets for drug development. Included are therapeutic implications, diagnosis, and prognosis for understanding and facilitating daily practice in the management of patients with CML. Understanding of the pathogenesis and therapy of hematological malignancies such as CML has grown significantly in recent years. This development owe much to the progress in molecular biology and now makes a major contribution to diagnosis and to treatment that pharmacologically targets the molecular events of CML. Molecular targeting therapy with newly developed agents such as small molecules and antibodies for hematological malignancies are being discovered after years of research. Some of these, including tyrosine kinase inhibitors such as imatinib, nilotinib, and dasatinib, lead to increased survival rates and improved therapies. With the opinion leaders of basic and clinical research in the field as authors, this book reviews recent advances in the biology of CML.
