Book excerpt: This open access book reviews the trends of persistent organic pollutants (POPs) in human milk and discusses the main findings of five global surveys that were coordinated by the World Health Organization (WHO) and the United Nations Environment Programme (UNEP) from 2000 to 2019. Human milk was selected as core matrix for human exposure under the Global Monitoring Plan for effectiveness evaluation of the Stockholm Convention on Persistent Organic Pollutants. Milk from well-defined groups of mothers was collected and mixed to form a representative sample per country. Datasets collected represent the largest global human tissues survey with a harmonized protocol, carried out in a uniform format for more than two decades. Altogether 69 countries participated in these studies between 2000 and 2015, and more than 40 countries participated in the study from 2016 to 2019. Divided into 5 parts, the book offers an authoritative overview of human milk biomonitoring; collates the harmonized sampling requirements and analytical methods for the identification and quantification of contaminants in human milk; examines the results of the WHO/UNEP-coordinated exposure studies, including the identification of selected chlorinated pesticides, dioxin-like compounds, industrial chemicals like polychlorinated biphenyls (PCB) and chlorinated paraffins, polybrominated POPs and PFAS, among others; and traces geographic, temporal and cross-substance trends and correlations, and human health risks. The book finishes by providing the reader with the summary of the main findings and outlook from these studies, in which the comparison of concentrations found for the wide range of POPs listed in the Stockholm Convention allowed the identification of possible needs for actions and follow-ups in different countries/regions. This book contributes to the understanding of exposure to hazardous chemicals and pollution as addressed by the UN Sustainable Development Goals on Good Health and Well-being (SDG 3) and will appeal to environmental and analytical chemists, researchers, professionals, and policymakers interested in learning more about contaminants in human milk. Despite much propaganda to the contrary, there can never be any serious question that pest-control chemicals and food-additive chemicals are essential to adequate food production, manufacture, marketing, and storage, yet without continuing surveillance and intelligent control some of those that persist in our foodstuffs could at times conceivably endanger the public health. Ensuring safety-in-use of these many chemicals is a dynamic challenge, for established ones are continually being dis placed by newly developed ones more acceptable to food technologists, pharmacologists, taxicologists, and changing pest-control requirements in progressive food-producing economies. These matters are of genuine concern to increasing numbers of governmental agencies and legislative bodies around the world, for some of these chemicals have resulted in a few mishaps from improper use. Responding to inconsistencies in the published data on concentrations of trace elements in breast milk, the study analyzes milk samples collected from women in Guatemala, Hungary, Nigeria, the Philippines, Sweden and Zaire in an effort to obtain reliable data on the quantities of minor and trace elements present in breast milk. The study was also designed to determine whether the concentrations of these elements varied significantly with the socioeconomic group, geographical origin, or nutritional status of the mothers. The book opens with a brief explanation of the biochemical functions of trace elements and the association of deficiencies with various disorders of growth and development. The second chapter explains the careful design of the study, which used standardized procedures for the collection of samples, a single reference analytical laboratory for each element, and appropriate analytical reference materials. Results are then presented for total dry matter and for antimony, arsenic, cadmium, calcium, chlorine, chromium, cobalt, copper, fluorine, iodine, iron, lead, magnesium, manganese, mercury, molybdenum, nickel, phosphorus, potassium, selenium, sodium, tin, vanadium, and zinc. For each element, information is given on any observed differences between study areas, variations in samples between urban and rural areas, and variations in concentrations according to time of year. A discussion of findings compares results of the study with data from the literature, assesses sources of variation in the elemental composition of human milk, and compares the observed daily intake of minor and trace elements with recommended intake. The book concludes that when minor and trace elements are determined under similar conditions in the breast milk of groups of mothers living in different parts of the world, environmental conditions play a major role in determining concentrations. Findings from the study should also prove useful in the formulation of recommendations regarding levels of trace elements in the diet of infants.

Book excerpt: Four years ago the National Institutes of Child Health and Human Development (NICHD) brought together a group of scientists to Belmont, Maryland to examine the status of human milk banking. During those deliberations, the idea was generated to organize a series of research conferences concerning human lactation and the composition and biological effects of human milk. The first one, organized by Robert G. Jensen from the University of Connecticut and Margaret C. Neville from the University of Colorado, dealt with methodologic issues. An additional meeting to explore the effects of maternal and environmental factors upon human lactation and the composition of human milk was organized by Margit Hamosh from Georgetown University and me, and was held in January, 1986 in Oaxaca, Mexico. Those meetings provided the foundation for the design of the present conference, 'The Effects of Human Milk Upon the Recipient Infant'. In addition to a grant from the NICHD, the conference was generously supported by Milupa AG from the Federal Republic of Germany; Wyeth Limited and Mead Johnson of Canada; and Ross Laboratories, Heinz USA, the Mead Johnson Nutritional Group, Wyeth International Limited, Gerber Products Company, the La Leche League International, Glaxo Incorporated and Sandoz Pharmaceutical Corporation from the United States.
