Book excerpt: Cellular Basis of Chemical Messengers in the Digestive System contains the proceedings of an international symposium on the cellular basis of chemical messengers of the digestive system held in Santa Monica, California, on January 16, 17, and 18, 1980. The papers explore the entire spectrum of problems related to the cellular aspects of chemical messengers in the digestive system, with emphasis on which amines and peptides serve these functions and in which neurons and endocrine-paracrine cells each kind of chemical messenger is found. This book is comprised of 28 chapters divided into six sections and begins by considering the nature of gut peptides and their possible functions. The discussion then turns to the diffuse neuroendocrine system and the phylogeny of the gastroenteropancreatic neuroendocrine system. Methods such as immunocytochemistry, electron immunohistochemistry, and electron microscopy autoradiography are then described. The following chapters focus on the function and morphology of endocrine-paracrine cells; immunochemical characterization of peptides in endocrine cells and nerves; Langerhans islets as the neuro-paraneuronal control center of the exocrine pancreas; and regulation of metabolism by gastroenteropancreatic peptides. The final section presents experimental results, including in vitro studies of canine pseudo-islets and of the mechanism of gastrin release. This monograph will be of interest to physiologists and other practitioners in the field of medicine.

Book excerpt: The mammalian gastrointestinal mucosa is a rapidly self-renewing tissue in the body, and its homeostasis is preserved through the strict regulation of epithelial cell proliferation, growth arrest, and apoptosis. The control of the growth of gastrointestinal mucosa is unique and, compared with most other tissue in the body, complex. Mucosal growth is regulated by the same hormones that alter metabolism in other tissues, but the gastrointestinal mucosa also responds to host events triggered by the ingestion and presence of food within the digestive tract. These gut hormones and peptides regulate the growth of the exocrine pancreas, gallbladder epithelium, and the mucosa of the oxyntic gland region of the stomach and the small and large intestines. Luminal factors, including nutrients or other dietary factors, secretions, and microbes that occur within the lumen and distribute over a proximal-to-distal gradient, are also crucial for maintenance of normal gut mucosal regeneration and could explain the villous-height-crypt-depth gradient and variety of adaptation, since these factors are diluted, absorbed, and destroyed as they pass down the digestive tract. Recently, intestinal stem cells, cellular polyamines, and noncoding RNAs are shown to play an important role in the regulation of gastrointestinal mucosal growth under physiological and various pathological conditions. In this book, we highlight key issues and factors that control gastrointestinal mucosal growth and homeostasis, with special emphasis on the mechanisms through which epithelial renewal and apoptosis are regulated at the cellular and molecular levels.

Book excerpt: Endocrinology, as a discipline, was a late arrival in the corpus of established subjects. Its growth in recent years has been prodigious, extending from mor phology to molecular levels. Most of the major endocrine glands were noted by the early anatomists, although the adrenal glands were not described until 1563 by Bartholomaeus Eustachius (1520-1574). On the other hand, elucidation of the function of these glands was extremely slow. Key work by A. A. Berthold (1849), although overlooked at the time, showed that comb atrophy in castrated fowl was prevented by testis transplantation. The idea that glands produced substances reach ing the bloodstream directly and not via excretory ducts stemmed from Claude Bernard, who first used the term internal secretion in 1855. The clinical observa tions of Thomas Addison at Guy's Hospital-published as a monograph in 1855 entitled The Constitutional and Local Effects of Disease of the Suprarenal Capsules -were seminal. However, the stimulus of this early research did not bring imme diate widespread further investigations. Upon the discovery of secretin in 1902, Bayliss and Starling considered the term "internal secretion" to be clumsy, and the term "hormone" was coined (from OQ[!UW-1 excite or arouse) and it was first used by Starling in his Croonian of 1905.

In statistical terms, gastric cancer is therefore the most formidable of cancer types, and its control is a pressing issue. Recent evidence indicates that the conversion of normal cells to so-called "clinical cancer" is the prerequisite for a multistage process which is intimately associated with an accumulation of multiple gene alterations including both oncogenes and tumor-suppressor genes. Gastric cancer is no exception, in that it reveals multiple gene changes whose scenario differs, depending on their occurrence in intestinal-type or diffuse-type gastric cancers.
