Book excerpt: Khat. A harmless natural stimulant or a lethal epidemic sweeping through the international drugs trade? Khat is a natural substance that, in the Middle East, is as ubiquitous as coffee is in the West. It is hugely popular in some African and Arab populations. But critics contend that it is a seriously addictive stimulant that damages the cardiovascular system. In a groundbreaking study, the authors go behind the veil of the drug, questioning its availability and its effect on its Red Sea producers. Interwoven with case studies from Djibouti to Rome, The Khat Controversy goes deeper to explore contemporary issues relating to globalization, ethnicity and culture. With its popularity escalating in London, Rome, Toronto and Copenhagen, khat is fast becoming a problem in the West. The first study of this contested drug, The Khat Controversy provides a concise introduction to the issues surrounding khat usage and suggests how policymakers should address them. The Khat Controversy: Stimulating the Debate on Drugs has received an honorable mention for the African Studies Association's 2008 Melville J. Herskovits Award

Available in PDF, EPUB and Kindle. Book excerpt: The habit of Catha edulis Forsk (khat) chewing has prevailed for centuries among populations in the horn of Africa and the Arabian peninsula. Fresh leaves of Catha edulis are customarily chewed to attain a state of brain stimulation. The fact that cathinone has a closer structural similarity with amphetamine, and both share common pharmacodynamic features, led to the conclusion that cathinone is the most important active ingredient of Catha edulis, which causes the major pharmacological effects. Problems associated with repeated consumption of Catha edulis leaves are becoming evident. Literature surveys and clinical diagnostic studies revealed an association with prevalence of psychiatric morbidity. Catha edulis also leads to several peripheral effects that include increases in blood pressure and has been associated with the increased incidence of acute myocardial infarction. Catha edulis also accounts for a number of gastrointestinal tract problems, oesophagitis, gastritis and delay intestinal absorption, as well as the development of oral keratotic white lesions at the site of chewing.

Book excerpt: Addiction is increasingly being recognized as a major global public health issue, and an ever-growing number of medical specialties, psychological and social science training programs, and professional associations are including addiction as part of their training and continuing education curricula. The first edition of this book presented an overview of the spectrum of addiction-related problems across different cultures around the globe. Sharing the experience and wisdom of more than 260 leading experts in the field, and promoted by the International Society of Addiction Medicine, it compared and contrasted clinical practices in the field of addiction medicine on the basis of neurobiological similarities as well as epidemiological and socio-cultural differences. Building on the success of this inaugural edition, and taking into account the formal and informal comments received as well as an assessment of current need, this textbook presents general updated information while retaining the most requested sections of the first edition as demonstrated by the number of chapter downloads. It also provides a basic text for those preparing for the ISAM annual certification exam. Written by some 220 international experts, it is a valuable reference resource for anyone interested in medicine, psychology, nursing, and social science.

Book excerpt: Khat (Catha edulis Forsk) is a dicotyledonous evergreen shrub of the family Celastraceae that grows in parts of East Africa and Yemen. Fresh leaves and stem tips or twigs of the plant are chewed for their euphoric properties. The habit of khat chewing is increasing remarkably and its health effects are not much known to the users. The study is aimed at investigating the toxic effects of khat extract on male gonad, the testes by assessing the histological changes of the testes as well as changes in serum testosterone level and amount of glycogen in testicular tissue. The organic extract of khat leaves at different doses were chronically administered to different age groups of mice. By means of qualitative histopathological examinations and quantitative histomorphometric investigations, as well as through testosterone and glycogen assays, the toxic manifestations of the extract was assessed at various doses. The results were compared with the control groups and statistically analyzed.
