This study examines their health-related behaviour and the interaction between this population and the American health care system. In order to understand the current health practices of Kampuchean refugees and the changes which have occurred since their resettlement, discussions of Kampuchean history and culture, traditional health practices and refugee experiences are also included. Cost, convenience, awareness of health care options and ability to cope with the American world are the critical factors which shape help-seeking. Family, friends and community leaders are the principal resources used by Kampuchean refugees who want health care. Definitions of illness have changed during the years Kampuchean refugees have been in the United States, and they are still changing. The health-related behaviour of Kampuchean refugees has changed much more than their conceptions of health and illness have. The presence of cultural intermediaries is critical to the provision of satisfactory medical care for refugees: they are crucial links between the refugee community and the American health care system. The principal recommendation which emerges from this paper is that interpreters and information services are needed to improve refugee health care. Refugees need to be taught how to use the American health care system.

Book excerpt: Abstract: This qualitative study sought to understand the perceptions of mental health and help-seeking in the second generation, their help-seeking behaviors, and the role that the Cambodian community enhances and inhibits mental well-being. Stigma, ignorance of mental health, and community pressure were identified as inhibitors of help-seeking, while strong community engagement, positive Cambodian identity, and increased mental health awareness in the second generation were identified as enhancing mental health. Respondents noted an increased comfort with discussing mental health issues with friends, family, and professionals compared to the first generation, and identified a number of positive coping strategies that they engage in to self-sooth. The findings from this study suggest that there is greater mental health awareness in the second generation compared to the first generation, and that their awareness is contributing to increased positive associations with mental health help-seeking in the Cambodian community. More support for the community is needed in the areas of developing strong community identity and engagement, which may in turn increase mental health literacy in the community as well.

Book excerpt: Background: Cambodian Americans have relatively poor mental and physical health outcomes compared to the general U.S. population and other Asian American subgroups. Additionally, many services such as screenings and mental health treatment are underutilized. Barriers such as cost, low-English proficiency, and acculturation stress impact their ability to access healthcare. However, there is a lack of research on the holistic healthcare experience of Cambodian Americans. Purpose: This study aimed to further explore the 1) health beliefs and values of Cambodian-Americans, 2) perceived factors that affect their healthcare experience, and 3) perceived factors that affect their health care seeking behavior. Methods: Purposive and convenience sampling were used by members of the Khmer Health Board to conduct 3 focus groups and 30 interviews with Cambodian Americans in the Seattle Metropolitan area for a total of 50 participants ranging from 19 to 75 of age. Results: A qualitative analysis identified challenges navigating the health system, socio-cultural influences, and historical trauma as major themes that informed Cambodian Americans health beliefs, healthcare experience, and healthcare seeking behavior. Implications: This study found that the experience of historical trauma not only contributes to health outcomes such as post-traumatic stress disorder, but can be a mediating factor in healthcare utilization. When also considering the preference participants showed towards wellness activities among peers, efforts to culturally-tailor health services should also consider community-level interventions and how to avoid potential re-traumatization of patients.

Using a mixture of qualitative and quantitative methods, this paper describes a quality improvement project that explores personal, cultural, and social factors that reduce symptoms of post-traumatic stress disorder (PTSD) among Cambodian American seniors residing in Minneapolis and St. Paul, MN. Leininger's Theory of Cultural Care Diversity and Universality along with her Sunrise Enabler guided project design, implementation, and evaluation. Also, data were collected and analyzed according to the Ethnonursing Method. Ten Cambodian seniors who were active in an Adult Day Care program and Buddhist religious ceremonies participated in the project. Face-to-face interviews were conducted with open-ended questions. A PTSD for Cambodians Checklist (PTSD-CC) was specifically designed to collect additional information. Qualitative and quantitative results showed that active involvement in Adult Day Care and Buddhist religious ceremonies was effective in reducing symptoms of PTSD among Cambodian seniors. Implications affirm that being active in the Adult Day Care program and Buddhist religious ceremonies can be considered alternative ways to intervene in mitigating symptoms of PTSD. These activities should be considered integrative treatments and added to Western modalities for reducing Cambodian patients' suffering from PTSD. Further studies are needed with a larger sample and in other locations to ensure the credibility of project outcomes.
