Until the 1970s, there was no generally accepted method of ensuring a scientific, critical approach to clinical decision making. And while traditional clinical authority was under attack, there was increasing concern about the way in which doctors made decisions about patient care. In this book, Jeanne Daly traces the origins, essential features, and achievements of evidence-based medicine and clinical epidemiology over the past few decades. Drawing largely on interviews with key players, she offers unique insights into the ways that practitioners of evidence-based medicine set out to generate scientific knowledge about patient care and how, in the process, they reshaped the way medicine is practiced and administered.

Book excerpt: All I Eat Is Medicine charts the lives of individuals and the operation of institutions in the thick of the AIDS epidemic in Mozambique during the global scale-up of treatment for HIV/AIDS at the turn of the twenty-first century. Even as the AIDS treatment scale-up saved lives, it perpetuated the exploitation and exclusion that was implicated in the propagation of the epidemic in the first place. This book calls attention to the global social commitments and responsibilities that a truly therapeutic global health requires.

Book excerpt: The California Institute for Regenerative Medicine (CIRM) was created in 2005 by The California Stem Cell Research and Cures Act (Proposition 71) to distribute $3 billion in state funds for stem cell research. The passage of Proposition 71 by the voters of California occurred at a time when federal funding for research involving human embryonic stem cells was uncertain, given the ethical questions raised by such research. During its initial period of operations, CIRM has successfully and thoughtfully provided more than $1.3 billion in awards to 59 California institutions, consistent with its stated mission. As it transitions to a broadened portfolio of grants to stimulate progress toward its translational goals, the Institute should obtain cohesive, longitudinal, and integrated advice; restructure its grant application review process; and enhance industry epresentation in aspects of its operations. CIRM's unique governance structure, while seful in its initial stages, might diminish its effectiveness moving forward. The California Institute for Regenerative Medicine: Science, Governance, and the Pursuit of Cures recommends specific steps to enhance CIRM's organization and management, as well as its scientific policies and processes, as it transitions to the critical next stages of its research and development program.

Available in PDF, EPUB and Kindle. Book excerpt: What Is Medicine? Western and Eastern Approaches to Healing is the first comparative history of two millennia of Western and Chinese medicine from their beginnings in the centuries BCE through present advances in sciences like molecular biology and in Western adaptations of traditional Chinese medicine. In his revolutionary interpretation of the basic forces that undergird shifts in medical theory, Paul U. Unschuld relates the history of medicine in both Europe and China to changes in politics, economics, and other contextual factors. Drawing on his own extended research of Chinese primary sources as well as his and others' scholarship in European medical history, Unschuld argues against any claims of "truth" in former and current, Eastern and Western models of physiology and pathology. What Is Medicine? makes an eloquent and timely contribution to discussions on health care policies while illuminating the nature of cognitive dynamics in medicine, and it stimulates fresh debate on the essence and interpretation of reality in medicine's attempts to manage the human organism.

Available in PDF, EPUB and Kindle. Book excerpt: Most of us want and expect medicine’s miracles to extend our lives. In today’s aging society, however, the line between life-giving therapies and too much treatment is hard to see—it’s being obscured by a perfect storm created by the pharmaceutical and biomedical industries, along with insurance companies. In Ordinary Medicine Sharon R. Kaufman investigates what drives that storm’s “more is better” approach to medicine: a nearly invisible chain of social, economic, and bureaucratic forces that has made once-extraordinary treatments seem ordinary, necessary, and desirable. Since 2002 Kaufman has listened to hundreds of older patients, their physicians and family members express their hopes, fears, and reasoning as they faced the line between enough and too much intervention. Their stories anchor Ordinary Medicine. Today’s medicine, Kaufman contends, shapes nearly every American’s experience of growing older, and ultimately medicine is undermining its own ability to function as a social good. Kaufman’s careful mapping of the sources of our health care dilemmas should make it far easier to rethink and renew medicine’s goals.

Available in PDF, EPUB and Kindle. Book excerpt: American Medicine: The Quest for Competence, the first book to explore in depth the meaning and politics of competence in modern American medicine, examines questions that lie at the heart of the contemporary debate about medical care. Based on Mary-Jo DelVecchio Good's recent ethnographic studies of three distinct medical communities - physicians in rural California, academics and students involved in Harvard Medical School's innovative "New Pathway" curriculum, and oncologists working on breast cancer treatment - the book demonstrates the centrality of the issue of competence throughout the medical world. The theme of competence, Good shows, provides common ground for discussing the power struggles between rural general practitioners and specialists, organizational changes within the halls of academia, and the clinical narratives of high-technology oncologists. A timely, provocative study that addresses one of the fundamental issues in contemporary medicine, American Medicine: The Quest for Competence is essential reading for medical professionals, educators, and students; medical anthropologists and sociologists; and health-care policymakers.
